Achieving a Network of Integrated Community-Based Medical Homes
OCHP is leading a MDC-wide comprehensive planning process to develop a roadmap that will become the guide for
leveraging both additional capital and operating dollars to expand our health delivery system. The Goal is to turn
Community Health Systems into full spectrum portals to comprehensive health and services.
Leveraging – $25M in GOB for Primary Care Facilities capital funds to generate nearly 150,000 added
primary care visits (over 3 years) from the County’s existing 7 Federally Qualified Health Centers (FQHCs)
