LIFE INSURANCE & ACCIDENTAL DEATH & DISMEMBERMENT SERVICES
This addendum is issued to make the following revisions to this solicitation and is
hereby incorporated and made a part of RFP Number 110D-14F.
Proposers shall sign and include this addendum as part of their submission and also
acknowledge the receipt of this addendum by stating that acknowledgement on the
outside of the sealed envelope containing the proposal.
 The Employee/Spouse and Retiree/Spouse census data worksheets will be
made available as Excel spreadsheets via email upon request. Your request for
this information should be sent to [email protected] .
If you have already emailed a request for this information in Excel format your
original request will be fulfilled.
The due date and time remain the same.
