THE NEW HAMPSHIRE LOTTERY COMMISSION® NEW ENGLAND PATRIOTS™ 2016 SCRATCH GAME TICKETS SECOND-CHANCE DRAWING RULES 1. Any non-winning ticket for the $5 PATRIOTS™ 2016 instant scratch game (game number 1311) may be entered for one of 5 (five) drawings as detailed below. 2. To be eligible, the ticket must: Be a non-winning ticket (does not contain any winnable prizes); Have the entrant’s name, mailing address, and daytime phone number legibly printed in the space provided on the back of the ticket. A mailing label is also acceptable; Be signed by the entrant in the space provided on the back of the ticket; Not be creased, folded or otherwise damaged, or altered in any way to give it an unfair advantage in the drawings; and Be submitted to the Lottery headquarters, prior to the deadlines noted below, at the following address: Entry New Hampshire Lottery Commission 14 Integra Drive Concord, NH 03301 Any ticket that is damaged, does not completely and clearly identify the entrant’s contact details, or has not been signed by the entrant will be discarded. The Commission shall not be held responsible for entry errors by entrants. 3. There will be 5 drawings total, one held at 10AM EST on each of the following dates at New Hampshire Lottery Commission (NH Lottery) headquarters: October 20, 2016; November 17, 2016; February 9, 2017; April 20, 2017; and June 22, 2017*. 4. Eligible entries must be received by 4:00pm EST on October 17, 2016; November 14, 2016; February 6, 2017; April 17, 2017; and June 19, 2017*. Tickets received after these deadlines will be entered into the next available drawing; tickets received after 4:00pm on June 19, 2017 will be set aside for destruction. Entries are good for one drawing only. Tickets not drawn for a prize at any drawing will be removed for destruction prior to any subsequent drawing. 6. The first eligible ticket selected at each of the drawings listed in (4) above shall be awarded Away Game Trip, comprising of: Two (2) roundtrip domestic commercial air/train fare; Two (2) away game tickets; and One (1) double occupancy hotel accommodation. 7. The next 10 eligible tickets selected at each of the drawings listed in (3) above will be awarded Lower Level 2-Pack Home Game Day Packages, comprising of: Two (2) home game tickets; One (1) parking pass; and Two (2) Patriots tailgate passes. 8. The final 10 eligible tickets selected at each of the drawings listed in (3) above will be awarded Upper Level 4-Pack Home Game Day Packages, comprising of: Four (4) home game tickets; One (1) parking pass; and Four (4) Patriots tailgate passes. 9. Game tickets awarded at the October and November drawings will be for games held during the 2016 NFL season. Game tickets awarded at the February, April and June drawings will be for games held during the 2017 NFL season. 10. The tailgate passes, awarded in accordance with (7) and (8) above, entitle holders to sit-down dining and/or food and beverage service. Bar services to include beverages, linen and tableware, copies of the Patriots™ Gameday Magazine for each guest, appearances by Patriots Cheerleaders, on-site security and medical staff. 11. At the conclusion of each drawing described in (3) above, five (5) alternate tickets will be drawn. 12. No person may win more than one (1) prize per drawing date. In the event that two or more tickets are drawn with the same entrant’s name, only the first ticket drawn will be eligible to win a prize; the subsequent ticket(s) will be discarded and another will be drawn in its place. 13. $5 PATRIOTS™ 2016 instant scratch game may continue to be sold after the final drawing. 14. Federal taxes estimated at 25% of the value of each package that is valued at $600 or greater will be borne by the NH Lottery, and will send winners IRS Forms 1099 in January of the year following the respective usage of the awarded prize. Any other taxes or fees will be responsibility of the prize winners. Page 2 of 4 pages August 2, 2016 15. The prizes will be as described above and there will be no substitution by cash. 16. Game tickets will be for games determined by the New England Patriots only and cannot be exchanged for different games. 17. Winners will be notified by mail, e-mail or telephone, using the contact information on the drawn tickets, and claim form. Prizes will be dispatched by The New England Patriots to the NH Lottery for distribution using a common carrier. Prizes sent to winners from the NH Lottery will use certified mail and/or Federal Express with a signature required. 18. To claim a prize, each winner shall complete a NH Lottery Prize Claim Form and return same within 30 (thirty) days* of the forms being mailed from the NH Lottery. Failure to comply will result in the prize package being forfeited and the prize will be awarded to the first available alternate winner. *NOTE: If a game is scheduled to be held within the 30 days noted above, then the winner will be allowed only 14 days to submit all required paperwork. 19. The NH Lottery reserves the right to amend the drawing rules as necessary without prior notice no less than 10 (ten) days before any drawing. 20. The NH Lottery offers no warrantees or guarantees of product quality and that availability and quality of the prize and its conformance to advertised representations is the sole responsibility of the promotional partner. 21. The NH Lottery shall not be responsible for any prizes lost, damaged or stolen during shipment, pickup or use. 22. The NH Lottery shall not be responsible for injury or loss of life resulting from any prize awarded. 23. Participants release and hold harmless the Commission and its agents and employees from any liability arising out of participation in the drawings. The Commission is not responsible for any negligence of any person, or any claim, liability, injury, property loss, or other loss to participants or their companions arising from or in connection with any event or occurrence related to these drawing. 24. The Commission, at their sole discretion, may disqualify an entry, winner, or designee from participating in any aspect of the drawing if they deem or suspect that such person has engaged in or has attempted to engage in any of the following: Act in violation of these Official Rules, Terms of Use, or Privacy Statement ; Act with intent to annoy, harass or abuse any other person; Behave in any unsportsmanlike, inappropriate, uncooperative, disruptive, fraudulent, potentially fraudulent, or unusual behavior or activity; or Participate in any activity deemed to be generally inconsistent with the intended operation of the drawing. The Commission reserves the right to prosecute and seek damages from any such individual to the fullest extent of the law. Page 3 of 4 pages August 2, 2016 25. CHILD SUPPORT INTERCEPT – Pursuant to RSA 284:21-v, the NH Lottery is required to review all lottery prize claims subject to federal tax reporting with a child support database provided by the Department of Health and Human Services (DHHS). In the event that a winner of any prize package described above owes child support, the winner shall have up to 21 days from the draw date to settle the total arrearage due as determined by DHHS to the DHHS, after which settlement the winner will be awarded the prize. Failure to settle any arrearage shall result in that winner’s prize package being forfeit and the package will be issued to the first available alternate winner. The original winner(s) owing Child Support shall receive no other prize or compensation from the NH Lottery relating to this game. Page 4 of 4 pages August 2, 2016
