Part A – List of beneficiaries who are entitled
If the person who has died was domiciled in Singapore, and did not make a valid nomination, we will
distribute their CPF money and money from their estate in line with the rules of distribution as set
out in the Intestate Succession Act (Cap. 146).

PART B - TABLE OF DOCUMENTS
Please submit a photocopy of the following documents together with the completed Online
Application Form.
Table 1 – Documents we must have
1) Death certificate of the Deceased
2) NRIC, passport, social security card (for US nationals), election card (for Indian nationals) of all
beneficiaries including the person who sent us the online application form
3) The front page of your personal bank passbook or bank statement of all beneficiaries showing the
account number (for GIRO payments). (We will not accept a joint account. We will send a cheque
instead.)
4) A declaration or testimonial from the proper authority giving details of the legal heirs of the person who
died (if Deceased was not domiciled in Singapore)

1) Decree nisi absolute (if the person who died was previously divorced)
2) Death certificate of the husband or wife (if the person who died was a widow or widower)
