Dear Friends,
Thank you for making 2015 a remarkable year
for the Liberty Hospital Foundation!
2015 marked the 31st year of service for our
organization. With your support, we were able
to significantly expand our
programs and services
and grow our outreach to
the entire Liberty Hospital
community.
This year alone, we invested
$250,000 into programs
that benefit the patients, employees, and
community of Liberty Hospital. In this Annual
Report, you’ll see highlights from key programs
and ideas of what is on the horizon for 2016
and beyond. Also included are our annual
Honor Roll of Donors and a special section
recognizing tribute gifts.
While we are encouraged by the increased
impact through new programs serving patients
in need of long-term solutions for affording
medications and healthcare, as well as our
community grant program - we know we need
to strive to improve the health and wellness of
even more people moving forward.

Hospital Birthing Center have

KYLEIGH’S GIFT
Over the next year, we plan to provide more
support to families at Liberty Hospital facing
an infant or pregnancy loss - including gifts
of memorial candles, care packages, and
memory boxes.
MEDICATION ASSISTANCE
In 2015, the Medication Assistance Program
(MAP) expanded services to all Liberty Hospital
affiliated clinics and physician practices, as
well as Liberty and Liberty North High Schools.
In 2016, our goal is to offer more critical
assistance for patients to maintain their health
after they leave Liberty Hospital - and thrive in
our community.
PROFESSIONAL EDUCATION
In 2016, the High School Scholarship program
will grow and provide six $2,500 scholarships.
Five Liberty Hospital employees will enter their
third year of a full scholarship to the online RN
to BSN program at William Jewell College.

In 2016, we will enter into phase two of our
recent beautification efforts on the TreeHouse
gardens and patio. A new Butterfly Garden
will be planted and revitalized memorial and
commemorative opportunities will be available
along the Walking Trail.

COMMUNITY
GRANTS
Over the past year, nearly 400 tribute gifts totalling $40,000
have been made to the Foundation. Gifts were designated
to based on donor request to: TreeHouse at Liberty Hospital,
Kyleigh’s Gift, Hospice, Walking Trail, Auxiliary, Oncology,
and Women’s Health. The Foundation also provides
commemorative bricks, benches, and trees along the
Walking Trail as well as other naming opportunities.
