Talent Show Permission Slip










Permission slips must be returned to the Main Office no later than Friday, December 21st.
Any current Midland School student, grades Pre-K through 8th is eligible to enter
Performance may include singing, dancing, instrumental performances, drama, comedy, magic or
skits deemed appropriate for the family atmosphere of the PTA
You may perform with a group or by yourself
Participants can only perform once
Each performance can be up to two minutes long.
Students must supply their own music and/or props.
Music must be supplied at the time of audition on a CD and left with us..
Proper attire is a MUST. Props and attire will be reviewed for appropriateness at time of audition
and/or rehearsal.
All participants and attendees must act responsibly and be on their best behavior at all times
Grades PreK-3rd will audition on Friday, January 18th, 2013
Grades 4 through 8 will audition on Friday, January 25th, 2013
