PURPOSE The City of South Beloit (City) intends to enter into a contract with a qualified, professional contractor (Contractor) for the surface repair, preparation, and resurfacing of the tennis courts and Basketball court located at South Beloit City Park (400 Winnebago Ave) pursuant to the requirements and considerations contained within this Request for Proposals (RFP). 2. SCOPE OF WORK The concrete tennis/basketball court is approximately 188 feet wide by 121 feet long (22,748 sqft). a. Generally, work shall consist of: i. Cleaning court surface to remove loose debris ii. Patching bad spots and low spots iii. Repairing cracks 0.063 inches (1/16”) wide and larger iv. Applying one (2) coat acrylic resurfacer v. Applying two (2) coats of tennis court paint vi. Striping to meet U.S.T.A. standards vii. Repair and paint fencing b. Compliance: i. All work shall comply with the standards and specifications provided with this RFP. *Subject to change at discretion of the City. c. Pricing: i. Pricing shall be provided as indicated on the Bid Form. ii. All bids shall include Illinois prevailing wage rate. d. Special Conditions: i. The bid price shall not include materials or labor required to repair existing conditions which were not apparent upon initial visual inspections. f. Billing and Payment: i. Contractor shall submit a single invoice for this project with the exception of approved change orders. g. Construction Schedule: i. Within twenty (20) working days of Notice of Award, Contractor shall declare a start date for the project. Work shall begin on that date and be completed within twenty (20) working days. The City may charge $100.00 per day for each day this project is not completed past the due date unless inclement weather causes the delay. ii. A Notice of Award issued by the City of South Beloit shall serve as a Notice to Proceed for this project. h. Installation Warranty: i. Contractor shall guarantee all work against defective workmanship for a period of two (2) year from the date of completion. Ordinary wear and tear and unusual abuse or neglect will not be subject to warranty work. 3. GENERAL CONDITIONS Proposals must be received at City Hall (519 Blackhawk Boulevard- South Beloit, IL 61080) by 10:00 a.m., June 8, 2016. It is the responsibility of the Contractor to ensure that proposals arrive at the correct location and time. a. Proposals shall be submitted in a sealed envelope clearly marked: South Beloit Tennis/Basketball Courts b. Proposal pricing shall be valid for 90 calendar days. c. The Contractor awarded this contract shall be required to comply with all Local, State, and Federal requirements related to the contract. d. PROPOSAL REQUIREMENTS Proposals shall contain one (1) signed original Bid Form, a detailed description of the proposed materials and applications, and documentation of relevant experience. 5. EMPLOYMENT DISCRIMINATION BY RESPONDENT PROHIBITED During the performance of a contract awarded pursuant to this RFP, the Contractor agrees as follows: a. The Contractor will not discriminate against any employee or applicant for employment because of age, race, religion, color, sex, or national origin. Contractor agrees to post in conspicuous places, available to employees and applicants for employment, notices setting forth the provisions of this nondiscrimination clause. b. The Contractor, in all solicitations or advertisements for employees placed by or on behalf of the Contractor, will state that such Contractor is an equal opportunity employer. c. Notices, advertisements and solicitations placed in accordance with federal law, rule or regulation shall be deemed sufficient for the purpose of meeting the requirements of this section. 6. INSURANCE REQUIREMENTS a. The Contractor shall provide a certificate of Liability insurance. (The contractor(s) will need to name the city as an additional insured and meets the following liability limits if awarded a contract:$500,000 per claimant and $1 Million per occurrence). b. The Contractor shall indemnify and hold harmless the City against and from all liability, claims, damages, demands and costs, including attorney fees of every kind and nature and attributable to bodily injury, sickness, disease or death or to damage or destruction of property resulting from or in any manner arising out of, or in connection with, the projects and their performance of the work under this contract. 7. AWARD OF CONTRACT After receipt of proposals, the City shall consider the following factors for the selection of the Contractor: a. Relevant past experience. b. Demonstrated understanding of requested service and delivery of qualified, professional workmanship and materials. c. Pricing. The City of South Beloit reserves the right to reject any or all proposals, to waive informalities or irregularities in the proposals received, and to reject non-conforming, non-responsive, or conditional proposals, and to accept a proposal in whole or in part which, in the City’s judgment, best serves the interest of the City of South Beloit and its citizens. SPECIFICATIONS PREPARATION Clean court surface to remove all loose dirt, mildew, and debris. Machine sand or power wash as necessary. PATCHING In rough areas, remove loose debris and patch with acrylic patch binder. In low areas that hold 0.125 inches (1/8”) depth of water for more than one (1) hour, fill and level with acrylic patch binder. All patches shall be sanded level with surrounding surface. CRACKS Fill all cracks 1/16 of an inch or greater with acrylic crack filler. All crack repairs shall be sanded level with surrounding surface. PRIMER The entire court surface shall be primed with a concrete primer. RESURFACER Over the entire court surface, apply one (1) coat of acrylic resurfacer. PAINT Apply two (2) coats of tennis court-grade paint plus silica sand to provide for a mediumspeed playing surface. COLOR The interior court surface shall be blue and the perimeter areas shall be light green. STRIPING The playing lines of the tennis court shall be marked according to Unites States Tennis Association standards. Lines shall be white in color.
