__________ I agree to pay a $25 per student ($50 for family) non-refundable registration fee for the above
student(s) upon registration and understand that his/her placement in the designated grade class will not be
confirmed until the fee is paid in full. NOTE: Refer a new family to St. Peter School or preschool and, if
accepted, YOU will receive a $25 voucher to cover the cost of your registration fee.

each school year ends on May 12. Recording hours for the next school year begins on May 13.

WHAT ARE THE REQUIREMENTS OF THE PROGRAM?
Each K – 5 family is required to participate in and reach a minimum of 20 volunteer hours per year for each child. Hours
are earned by participating in various fundraisers and volunteer work in the school. Some of the things you can volunteer
for are:
 Lunch/recess Monitor (30-45 minute intervals)
 Program volunteer help such as, Pepperoni Sale pick-up help, Tamale Pick up help
 Special dinners/events help
 Auction or other committee help
 Stuffing envelopes for an event announcement
 Room representative
 Office help
 Joining and participating in our Marketing/Enrollment Committee
 Attending Home and School Parent meetings
 Other events as the school year progresses
NOTE: A volunteer request form will be available in the beginning of the school year.
NOTE: THIS COMPLETED AND SIGNED FORM IS REQUIRED AT THE BEGINNING OF THE
SCHOOL YEAR TO BE ABLE TO PARTICIPATE IN THE POINTS PROGRAM.
VERIFICATION: Please sign in/out for volunteer activities in the office so tracking hours can remain
accurate.
