Prosthetic Fabrication Requirements
For accurate prostheses and fast service, complete information is vital. Please follow the
guidelines listed below:

Provide accurate schemas and measurements.

Use procedures taught at the universities.

Include completed measurement forms.

Specify all materials to be used. Be specific.

Send negative impressions with clearly marked anatomical landmarks and
trim lines.

Plaster modifications: draw trim lines on the cast or negative impression
and supply all important measurements.

If you request Fabtech Systems to order needed parts to complete a device, supply us with the item’s Part Number and Supplier of that part.
Pricing
Prices do not include Prosthetic parts to be installed. We carry a wide assortment of componentry, some at a discounted rate, others at retail cost plus 15%. If we are required to order a
specific part, that part is billed at retail cost plus 15% and any shipping charges up to a maximum of $500.00. Items over $500.00 may be requested to be ordered on the customer’s account. Some items must be ordered by the customer or on the customer’s account for the

PAYMENT TERMS
Payment terms are Net Thirty (30) days.
FINANCE CHARGES
Past due accounts are subject to 1 1/2% per month finance charge–an effective annual rate of 18%. A
$30.00 charge will be applied to returned checks.
PAST DUE ACCOUNTS
Customers with past due bills over sixty (60) days, at our discretion may be placed on credit hold and
placed into our “Green Flag” accounts management. Past due bills over (90) days, will be placed into
a collections process.
COLLECTION COSTS
Companies will be held liable for any and all costs associated with the collection of any past due funds
and attorney fees arising from collection efforts made.
NEW ACCOUNTS
Credit terms are available for approved accounts. For all new accounts we require three trade
references and information about the bank with which you do business (see form supplied). Credit
must be approved before orders will be shipped on open status. Visa and MasterCard payments are
accepted.
RETURNS
All returns must have an authorization number. Please call us before returning a product. Regular
stock items must be in saleable condition and in the packaging that it was sold in. Returns must be
requested within 8 days or one week of receipt of order. An original or a copy of the invoice or packing
slip must accompany all returns.
DAMAGED MERCHANDISE
Contact us to report any problems. Please report damage to the carrier upon receipt of the goods.

Prices and terms are subject to change without notice.
PRODUCT WARRANTIES
We will honor manufacturer warranties of items we stock or distribute. All other items and their perspective warranties will go through the company that provided the item.

DISTRIBUTION PRODUCTS PRICING
All distribution products are discounted to central fabrication customers at a special rate.
These discounts are based on terms. Discount percentages may change with out notice! All
other special order parts are billed at 15% plus shipping.
SHIPPING
Products ship F.O.B. Everett, Washington, via UPS Ground.
SHIPPING INSURANCE
UPS covers items for a $100.00 maximum, for loss or damage. Additional insurance can be
provided at customers expense and request. Additional insurance is always recommended.
Fabtech Systems will not be held responsible for lost or damaged products.
SPECIAL SHIPPING PROGRAM AND PRICING INCENTIVES
The following price list is a retail price list for Fabtech Systems Central Fabrication services.
Discounts and shipping incentives are available based on volume and Net30 terms. Call for
details.
Free UPS Ground program. US lower 48 states. UPS Ground only. Net30 days. Up to $15.00
in actual shipping costs.
CENTRAL FABRICATION WARRANTY
We honor all manufacturer warranties of items we stock and distribute, and will assist you
with any returns. All central fabrication craftsmanship is warranted for 90 days, starting at
ship date.
