Dear Parent/Guardian,
We are very pleased to share with you information on our annual trip for our Baseball Campers. We have scheduled an
exciting, fun and instructional opportunity to visit the Renegades Baseball Stadium in Wappingers Falls, NY on Monday,
July 13th to watch a minor league ball game. Our experienced Coaches will accompany the group and provide ongoing
discussion of the game in addition to supervision. Transportation will be provided by our camp vehicles, with Chappaqua
Coach Bus drivers. We will leave from camp after normal arrival in the morning and return on time for regular dismissal.
Summer Trails will provide: round trip transportation and a ticket to the game. Please bring a brown paper bag lunch and
bottled water. If you are not enrolled in camp for this day and want to be included please contact the office to learn how
to register. We encourage you to return this form immediately as seating is limited. Feel free to call the office with any
questions.

I give my child permission to attend the baseball trip at the Renegades Stadium on Monday, July 13th. I authorize Jamie
Sirkin, the Camp Director, or her designated representative to act on my behalf in authorizing emergency medical, dental,
or surgical care and hospitalization for the above named minor during the period of my absence. This document will be
presented to a physician, dentist or appropriate hospital representative at such time as necessary.
