The inspection of the 1 Electric Sign was performed on TAXA dock, TAX AIRFREIGHT 1255 INDEPENDENCE DR Neenah WI 54956 . Present during
this inspection were Brenda. There were forklifts as well as palletjacks at this site.
The commodities were manufactured by Everbrite Inc., with the brand name A & W . The Electric Sign had model number E002103P, with the
serial number 10101222378.
The Sign was packed in a wooden crate constructed with 1' x 4" lumber. There were no inner packing materials. and there was no damage to the
crate.
The metal cover on the back side of the sign base is partly detached and the metal is bent,see photo # 8. The metal base covering is bent and
torn on the back right edge, see photo # 9.
