VIRGINIA UNIVERSAL SERVICE PLAN
A.
GENERAL
The Virginia Universal Service Plan (VUSP) is an offering designed to help qualified customers pay
for their Local Exchange Service. Such qualified customers are charged a reduced rate for their
local telephone service.

REGULATIONS
1.
The Virginia Universal Service Plan is available to qualified customers and is provided via a
residence dial tone individual line.
2.
The Virginia Universal Service Plan is Local Exchange Service and, as such, is subject to the
regulations governing Local Exchange Service in this tariff. The rates specified herein for
the Virginia Universal Service Plan apply in lieu of the rates for Local Exchange Service,
except as may otherwise be specified following.

In order to qualify for the Virginia Universal Service Plan, a customer must be certified by
the appropriate state agency under the auspices of the Virginia Secretary of Human Resources.
Certification will be based on criteria established by the Virginia State Corporation
Commission. Customers may be required to verify continued eligibility on an annual basis. The
Telephone Company is responsible for conducting periodic verification of continued eligibility
of its customers. The Telephone Company will comply with any continued eligibility
verification procedures established by State and/or Federal requirements.

When the Telephone Company receives notice from the appropriate state agency that a customer is
no longer qualified for the Virginia Universal Service Plan as outlined in 3. preceding, the
Telephone Company will then notify the customer that the Virginia Universal Service Plan will
be disconnected or changed to another class of residential service.

The Virginia Universal Service Plan will not be provided to a customer residing where there is
other Local Exchange Service on the same premises. In addition, with the exception of VUSP
Toll Restriction Service, a Virginia
Universal Service Plan customer may not subscribe to any additional services.
6.
A Virginia Universal Service Plan customer may subscribe to VUSP Toll
Restriction Service at no charge. VUSP Toll Restriction Service is a service available to
Virginia Universal Service Plan customers only. It allows the completion of local calls, calls
to the operator for the completion of collect and third number billed calls, calls to 911 and
to other N11 service codes, calls to 800/888 service telephone numbers, and local directory
assistance calls. VUSP Toll Restriction Service prohibits interstate and intrastate toll
calls, 700/900 calls, audiotex calls, collect and third party calls billed to the VUSP
customer’s line, busy line verification and interrupt services, interstate and intrastate long
distance directory assistance calls, as well as the use of Connect ReQuest®.
VUSP customers who selected Call Restriction Service prior to May 1, 1998 may continue to subscribe
to Call Restriction Service at no charge.

2.
The Virginia Universal Service Plan is Local Exchange Service and, as such, is subject to the
regulations governing Local Exchange Service in this tariff. The rates specified herein for
the Virginia Universal Service Plan apply in lieu of the rates for Local Exchange Service,
except as may otherwise be specified following.

In order to qualify for the Virginia Universal Service Plan, a customer must be certified by
the appropriate state agency under the auspices of the Virginia Secretary of Human Resources.

No deposit will be required to establish Virginia Universal Service Plan
Service if the customer voluntarily elects VUSP Toll Restriction Service.
8.
Virginia Universal Service Plan Service will not be interrupted or
disconnected for failure to pay toll charges. The Telephone Company,
however, reserves the right to initiate VUSP Toll Restriction Service, if
the VUSP customer has an unpaid toll balance of more than five days past the
due date.
9.
The Federal Subscriber Line Charge is waived for Virginia Universal Service
Plan customers.
