“In granting this sanction, it is understood and agreed that Utah Swimming and USA Swimming shall be free and held harmless from any
liabilities or claims for damages arising by reason of injuries to anyone during the conduct of the event.” Whether entries are submitted
electronically or on paper, by the action of entering the swimmer in the meet the submitter whether the swimmer, coach, parent or team
representative, certifies that the swimmer is a registered member of USA Swimming and that all times stated in/onthe entry are true and
correct. The submitter further assumes responsibility for any false or incorrect times, and understands that all listed entry times ar e subject to
Utah Swimming Time Verification Procedures, and that a fine of $20.00 shall be paid to Utah Swimming, Inc. for each incorrect time, unless
corrected before the entry deadline. It is understood that an automatic call before the review section shall be the result for a third entry time that
cannot be verified. The submitter further agrees that a penalty of $100.00 shall be assessed to anyone entering a swimmer in a meet
sanctioned by Utah Swimming, Inc. who is not registered with USA Swimming at the time of entry, upon it being proven that the swimmer was
not so registered.

Course:
• 25 yard pool with Six (6) lanes, Colorado System 5 and Pads
• The competition course has not been certified in accordance with 102.2.2C (4)
• Start End Water Depth Lanes: 1-5: 4 feet @ 1 meter from wall; 4 feet @ 5 meters from wall
• Start End Water Depth Lanes: 1-5: 5 feet @ 1 meter from wall; 6 feet @ 5 meters from wall
• Turn End Water Depth Lanes: 4 feet @ 1 meter from wall; 4 feet @ 5 meters from wall
Rules, Unaccompanied Athletes & Warm-up Procedures:
• 2011 USA Swimming Rules will apply.
• A mandatory scratch down may apply to the Mini Heart Attack meet if the time line exceeds 2 ½ hours
and to the Heart Attack meet if the time line exceeds 4 hours for the 12 and under events. If a scratch
down is necessary, swimmers will be reimbursed for said scratched events.
Unaccompanied
athletes: As per USA Swimming rule 202.3.2, all USA Swimming athletes must be under
•
the supervision of a USA Swimming member coach during warm-up, competition, and warm- down. If you
do not have a coach with you, please check with the meet director or referee before entering the pool. A
coach will be assigned.
• Any swimmer entered in the meet, unaccompanied by a USA Swimming member coach, must be certified by
a USA Swimming member coach as being proficient in performing a racing start or must start each race from
within the water. It is the responsibility of the swimmer or the swimmer’s legal guardian to ensure compliance
with this requirement.
• Warm-up: All warm-ups must be done under the supervision of a USA-S official. Coaches have the
responsibility of insuring that all their swimmers are instructed in warm-up procedures and that the swimmers
follow all Utah Swimming safety procedures. All swimmers will enter the pool from a 3 point sitting position. No
pushing, splashing, running or any other general horseplay. Swimmers engaging in such activities will be asked
to leave the pool. During the last 20 minutes of warm-up lanes 2 and 4 will be opened for diving starts in the
competition pool.
Eligibility:
This meet is open to all 2012 registered USA Swimming athletes who have not achieved a 2009-2012 National
“BB” or better time. NT’s are allowed. The swimmers age as of Thursday, February 23, 2012 shall determine the
age for the entire meet. No deck registration will be accepted; swimmers must be registered at time of entry.
Entries Information and Deadline:
 Individuals may enter up to THREE (3) individual events. No Times (NT’s) will be accepted.
 Meet Charge: $12.00 per swimmer
 All fees must accompany entries and are nonrefundable. One team check is recommended. Make checks
payable to: Utah Valley Aquatics.
 All entries must be received (in the hands of the below designated person) by 7:00 PM, Friday, February
17, 2012. Late entries will not be accepted. All entries will be subject to the meet entry limits regardless
if they are received before the entry deadline.

Meet entries will be limited to the first 75 swimmers that are not affiliated with the UVRays. UVRay
swimmers are not included in this meet limit. This will be subject to the four hour rule. Entries will be
accepted on a first come/first serve basis.
Please submit all entries by e-mail using Hy-tek software, or using the uniform entry form available at the
Utah Swimming website (www.swimutah.com). Teams entering via email must supply a signed hard
copy with verification of times. Entries will not be accepted without time verification and one uniform
entry sheet signed by the person responsible for entry submission. Said person will be responsible for
all fines and penalties resulting from inaccurate times and unregistered swimmers. Teams using the
official entry form must make sure that each form is signed and time verification is included on each
entry.
Awards:

Ribbons 1 through 6th place for Age Groups: 6 & Under, 7- 8, 9-10,11-12. Events may be swum as combined age
groups but will be awarded according to the preceding age groups.
Heart Attack cookie: given to every participant.
Scratch Procedure:
Scratches will be accepted up to a half an hour prior to the beginning of each session or when called for by the
Clerk of the Course. There shall be no penalty for a swimmer missing an event other than the swimmer will not be
allowed to swim that event.
Results:
Results will e-mailed after the completion of the meet.

Seating and Deck:
Please be courteous and clean up team area after the swim meet. Spectator seating will be in the bleachers above
the pool on the south side. The team areas for swimmers will be on the south and west side of the pool. The north
side will be for staging and starting and east end for timing. Please make sure to respect walkways.
Officials:
We welcome help from any officials your club may have. We will waive the surcharge and entries fees of a stroke
and turn judge’s child(ren) for the meet upon confirmation of the pledge to officiate the entire meet. Turn in names of
officials who will work entire meet and the names of their swimmer(s) with meet entries for fee waiver.
