Welcome to CCYM! We are glad to have you on our team, and we believe that God has
called you to this team to help in our effort to reach out to others in the name of Jesus Christ!
Our office needs you to work through this packet for the “nuts and bolts” of our ministry. This
packet has important documents that need to be filled out and sent back. Whether you
are a new member to CCYM or a returning member, it is important to carefully go through
this packet.

CCYM MEDICAL/PERMISSION FORM FOR 2016-2017

CCYM PARTICIPATION FEES
Due to the nature of our programming, we have instituted a one time CCYM Participation Fee of $150 per
person. This will cover registration, meals, and housing for any special events that we have (YAC, Project
Serve, Retreat, etc.) This is a non-refundable fee regardless of whether or not you attend the events,
or resign from CCYM.
Payment Schedule: (Make Checks Payable to East Ohio Conference Fund 9700)
$150 with online paperwork due by July 8, 2016
If you need to pay the fee in payments:
$50 with online paperwork due by July 8, 2016;
$50 by October 8, 2016, $50 by January 8, 2017.
Fees can also be paid online after creating your CCYM Profile on Ministry Tracker
(see instructions below)
If fees are not up to date, you will not be able to attend YAC 2016.

Get “Pumped” Up for the year; Grow together; and, Deepen our Love for Jesus Christ
FINAL CUT OFF FOR REGISTRATION IS Friday, JULY 8th
What to bring: Sleeping bag, pillow, bible, notebook, comfortable clothes (no spaghetti straps,
leggings, short shorts), bathing suit (one piece), towel, toiletries.

CCYM 2016-2017 CODE OF CONDUCT*
WE ARE THE 2016-2017 East Ohio Conference Council on Youth Ministries, and as such we represent our families, the
people of our Districts, local churches and agencies who elected or appointed us to be CCYM representatives. To honor
God and those persons we represent we are challenged to live at the highest Christian moral standard as possible at all
times.
It is expected that this CODE will be a guide for CCYMer’s daily life EVEN apart from CCYM related events and meetings.
The benefit of following this code is an increased depth to our community and to each person’s personal life. Following the
code provides for, and creates, an atmosphere in which all persons are cared for and respected in the highest manner
possible while honoring God.
The Community Promise: as a member of CCYM, I promise to adhere to the following Code of Conduct for all CCYM
related activities, and will use the Code as a guide for my everyday life.
a.
I will be at all meetings and events of CCYM. If I am unable to attend a meeting or an event I will submit a written
reason as to why I am unable to attend. Further, I will honor those who elected me by making every effort to be in
attendance. I understand that I must be at the meeting for 70% of the waking time in order for it to count as a full
meeting. I understand that missing more than two meetings/events/activities will result in my removal from
CCYM. I have received the calendar of these dates.
b.
I am a member of a leadership team, and as such I will seek to come to CCYM meetings with an open-mind, a listening
ear, a loving heart, and a willing spirit.
c.
I am a member of a leadership team for the United Methodist Church and as such I will seek to grow as a spiritual
leader through increasing my commitment to personal prayer and Bible Study. I will join with others to encourage,
pray for, and support all CCYM members in deepening their commitment throughout the year.
d.
I will also seek to grow spiritually through attending worship regularly at my local church (regular means 3 out of 4
Sundays a month you will be in church except when health or family activities cause you to miss). The church listed on
my profile is the church where I regularly attend. I understand that, at least, a bi-monthly call will be made to my church
to ensure my participation.
e.
During meetings, overnighters, and events, I will abide by the standards/procedures/expectations of CCYM adult
leaders who are entrusted with my safety and my care. If I do not, I understand that my parents will be called and
asked to come get me from the event/meeting/activity.
f.
I will not leave any CCYM meeting or event without a signed written note from a parent/guardian. I will have a signed
written note from my parent/guardian for each meeting if I will be riding with anyone other than my parent/guardian from
meetings/events. Further, I will seek to show respect for my family by keeping them informed of all CCYM meetings,
activities and events.
g.
I will leave each place better than I found it.
h.
As a teen member of CCYM I will not use drugs and alcohol; I will not engage in sex; will not use profanity; and, I will
not chew tobacco or smoke.
i.
As a responsible member of CCYM, my Christian values, beliefs and morals will be evident to those around me,
including those I correspond with via face to face, email, texting or social media, through my actions and words.
j.
As a member of CCYM I will do everything I can, with God’s help, to witness to the power of God in my life as a teen
witness for Jesus Christ by the way I talk, listen, and respond to others.
k.
I will respond promptly to all CCYM correspondence and communication. By taking these mailings seriously, I
am taking responsibility for my participation on this Conference Council.
I agree, as a 2016-2017 CCYM Member, to abide by this Code of Conduct. I understand that failure to abide by this Code of
Conduct will result in, at least, a consultation with the Associate Director for Youth Ministry and Conference Youth
Coordinators, and, at most, possible removal from CCYM.

*Our Conference Child Protection Policy calls on Conference Agencies to create safe, wholesome, and caring environments for
our children and youth. As with many Local School codes of conduct for students in extracurricular activities, this Code of Conduct is
designed to help create that safe environment. It is also designed to ensure our parents/guardians that creating this environment, for their
children, is a priority with the East Ohio Conference. (A copy of the signed code will be sent back to the CCYM member).
