Ectopia lentis may occur after trauma or may be associated
with ocular or systemic disease. As ectopia lentis may be the
first sign of a more serious systemic disease, it is important
to determine the etiology for appropriate patient management. Additionally, ectopic lentis can cause major complications within the eye, including significant refractive shifts,
pupillary block glaucoma, retinal damage, and blindness.

Ectopia lentis — Dislocated

lens into the vitreous
secondary to trauma.
The most significant symptom of ectopia lentis is reduced
visual acuity. Common visual disturbances include poor
near vision due to loss of accommodation, poor distant
vision due to astigmatism or myopia, and monocular
diplopia. The degree of visual acuity reduction will vary
with the degree of dislocation, the type of dislocation, and
any other concurrent ocular defects. Minimal subluxation
may lead to little or no loss of visual acuity, while disruption
of the zonules, leading to increased curvature of the lens,
may result in lenticular myopia and astigmatism.
Surgical management of
ectopia lentis remains a challenge,
as inherent defects in the lens
capsule makes implantation of
an intraocular lens difficult. As a
result, many patients are managed
with lensectomy/vitrectomy with
optical rehabilitation with a contact lens. Indications for
lensectomy include, but are not limited to:
• Lens-induced glaucoma
• Lens in the anterior chamber, especially if the lens
is touching the corneal endothelium
• Lens opacity that is mature or hypermature
• Evidence of lens-induced uveitis
• Inadequate visual acuity not correctable by refraction
• Imminent complete luxation of the lens
• Retinal detachment
MEDICAL
CORNER
Multiple surgical techniques exist for surgical correction of
ectopia lentis, each with its own limitations and associated
complications. Techniques include:
• Lensectomy/vitrectomy with a contact lens or spectacles
• Artisan iris clip lens
• Scleral-sutured posterior intraocular lens
• Implantation of in-the-bag intraocular lens with a
capsular tension ring

IMAGE
His livelihood is based largely on the art of capturing
images through a lens – a video camera lens. While he
controls the footage he shoots to produce marketing
videos for his clients, what he could not control were
his own two natural ocular lenses that allow him
to see! Gary Heath, a video production professional,
fell victim to a rare
condition in which
the natural lenses

As a self-employed,
uninsured professional, one of Gary’s primary services
is that of developing, shooting, and producing marketing videos for a number of Houston’s small businesses.
Dr. Ou informed him of the HEA Foundation and its
commitment to helping working people that fall into
the “insurance gap,” those that do not qualify for
government assistance, and do not have the resources
to pay for a private health plan.
When Gary first started noticing a deterioration in
sight he visited a local optometrist in hopes of finding
corrective lenses or some other solution. The optometrist, fearful of what she saw during her exam, quickly
referred Mr. Heath to a specialist – Dr. Richard Ou, an
ophthalmologist with Houston Eye Associates. Dr. Ou
confirmed the findings and explained: “The lenses
in both of Gary’s eyes had completely dislocated. His
vision was equivalent to pictures taken by a camera
missing the front focusing lens. To correct this, we must
replace his lenses and secure the new lens by placing

measures so I’m most grateful to Dr. Richard Ou and
the HEA Foundation for providing me with the
necessary eye surgery I needed to give me new lenses.
Now I am able to continue supporting myself and
family through my video and related production services,” states Gary. “My lenses are permanently in place
and my fluctuating vision loss has been eliminated.

HEA Foundation is a 501(c)(3) non-profit organization committed
to preserving and restoring vision to working poor adults and
children on the verge of losing their eyesight by providing
complete medical and surgical eye care services at no cost.
Physicians and medical professionals donate their office time
and surgical services while the HEA Foundation pays for
the surgical center fees, vision-saving medications, ocular
prosthetics, cornea tissue and any other ancillary service
required for restored vision.

new President of the Houston Eye Associates
Foundation (HEA Foundation). On behalf of
the HEA Foundation, I thank you for your
support as we continue preserving and
restoring the vision of hundreds of adults and
children every year. Your generous donations
together with the services provided by our
physicians and staff positively impact many

new President of the Houston Eye Associates
Foundation (HEA Foundation). On behalf of
the HEA Foundation, I thank you for your
support as we continue preserving and
restoring the vision of hundreds of adults and
children every year. Your generous donations
together with the services provided by our
physicians and staff positively impact many

Ectopia lentis — Dislocated

secondary to trauma.
The most significant symptom of ectopia lentis is reduced
visual acuity. Common visual disturbances include poor
near vision due to loss of accommodation, poor distant
vision due to astigmatism or myopia, and monocular
diplopia. The degree of visual acuity reduction will vary
with the degree of dislocation, the type of dislocation, and
any other concurrent ocular defects. Minimal subluxation
may lead to little or no loss of visual acuity, while disruption
of the zonules, leading to increased curvature of the lens,
may result in lenticular myopia and astigmatism.
Surgical management of
ectopia lentis remains a challenge,
as inherent defects in the lens
capsule makes implantation of
an intraocular lens difficult. As a
result, many patients are managed
with lensectomy/vitrectomy with
optical rehabilitation with a contact lens. Indications for
lensectomy include, but are not limited to:
• Lens-induced glaucoma
• Lens in the anterior chamber, especially if the lens
is touching the corneal endothelium
• Lens opacity that is mature or hypermature
• Evidence of lens-induced uveitis
• Inadequate visual acuity not correctable by refraction
• Imminent complete luxation of the lens
• Retinal detachment
MEDICAL
CORNER
Multiple surgical techniques exist for surgical correction of
ectopia lentis, each with its own limitations and associated
complications. Techniques include:
• Lensectomy/vitrectomy with a contact lens or spectacles
• Artisan iris clip lens
• Scleral-sutured posterior intraocular lens
• Implantation of in-the-bag intraocular lens with a
capsular tension ring

videos for his clients, what he could not control were
his own two natural ocular lenses that allow him

As a self-employed,
uninsured professional, one of Gary’s primary services
is that of developing, shooting, and producing marketing videos for a number of Houston’s small businesses.
Dr. Ou informed him of the HEA Foundation and its
commitment to helping working people that fall into
the “insurance gap,” those that do not qualify for
government assistance, and do not have the resources
to pay for a private health plan.
When Gary first started noticing a deterioration in
sight he visited a local optometrist in hopes of finding
corrective lenses or some other solution. The optometrist, fearful of what she saw during her exam, quickly
referred Mr. Heath to a specialist – Dr. Richard Ou, an
ophthalmologist with Houston Eye Associates. Dr. Ou
confirmed the findings and explained: “The lenses
in both of Gary’s eyes had completely dislocated. His
vision was equivalent to pictures taken by a camera
missing the front focusing lens. To correct this, we must
replace his lenses and secure the new lens by placing
