A health risk assessment that shows steps you can
take toward improving your health.
Annual Wellness Visit
A benefit covered by Network Health, it’s a check-up
with your doctor. The visit plays an important role
in disease prevention and also earns you healthy
rewards points redeemable for gift cards.
Wellness Programs and Online Tools
to help you learn about making healthy lifestyle
choices in areas such as stress, exercise,
weight, nutrition, tobacco cessation and
more. Use the Fitbit® online tracker
(or mobile app) to record your food intake
and wirelessly record your steps.

My Account can request a FREE
pedometer (a $59 value). Go to
My Account and register. Select
Strive, click Order
Pedometer & Allow Access.
Participants can also
upgrade to a different
Fitbit device.
