Patient Portal Instructions Login Screen Please go to https://portal.qrshs.com/ or georgetowninternalmedicine.com and click on the patient portal link Logging In/Creating an Account If you are returning to the Patient Portal please log in using your username and password. If this is your first time, please create an account to access the portal. Create New Account Link for lost Password and Username If you are returning and you forgot your password or username, please follow the links directly under the Login button. Patient Portal Instructions Create New Account Please input your account information noting that all fields with and asterisk (*) are required and cannot be blank. Check the box beside Terms and Conditions, then select You will automatically go to the Apply to a Practice page. Signup Patient Portal Instructions Apply to a Practice Enter the Practice Code given to you and select Check a box in the Apply column and select Select a box Search Apply to Practice Patient Portal Instructions Apply to a Practice Please click Patient Information to continue to input your information into the portal Click Patient Information Patient Portal Instructions Complete Portal Information Fill out each form as completely as possible. Always save your information before going to the next screen. Navigate by using the arrow buttons at the bottom of each page. Save your Information Go from page to page with arrows Patient Portal Instructions Creating a Linked Account If your family only has one email address or if you need a portal account for a minor, click Create Linked Account Create A Linked Account Patient Portal Instructions Create Linked Account Please input the account information for the LINKED account noting that all fields with and asterisk (*) are required and cannot be blank. Check the box beside Terms and Conditions, then select Signup You will automatically return to the dashboard of YOUR account. You can view the file by clicking on the filename, you can download the file by clicking Download File To delete the file click the delete file button Patient Portal Instructions Communicating with Your Practice To send an electronic message to your provider or their practice, click Communications from the main menu to access the Messaging Center.
