Earn AIA Vitality Points for completing a Vitality Health Check at selected Guardian Health & Beauty stores with Pharmacists.
Who may use this Benefit?
All AIA Vitality members qualify for this Benefit.
What you pay
The cost of the Vitality Health Check is $15 (inclusive of GST). You will receive a $15 voucher to spend at Guardian Health &
Beauty stores upon completion of the the Vitality Health Check (subject to the terms and conditions indicated on the voucher).
What is the Vitality Health Check?
The Vitality Health Check is made up of the following components:

Weight and height measurement

Blood pressure test

Blood glucose test
The test can be carried out at selected Guardian Health & Beauty stores with Pharmacists.
The pharmacist will give you feedback on your results. You will be awarded AIA Vitality Points for completing the check and bonus
points if your readings are within the healthy range. AIA Vitality will suggest personal goals to help you improve or maintain your
readings. Log on to our website AIAVitality.com.sg after your test to capture your results so that your AIA Vitality Points can be
awarded and see how AIA Vitality can help you improve where you need to.

How the Benefit works
Step 1: Make a booking
1.
You can have the Vitality Health Check done at selected Guardian Health & Beauty store with Pharmacists across
Singapore. Refer to the list of selected Guardian Health & Beauty stores at the end of this Benefit Guide.
2.
Call the Guardian Health & Beauty store of your choice to make a booking for the Vitality Health Check.
3.
Your booking date, time and price will be confirmed
4.
Go for your Vitality Health Check; remember to take your AIA Vitality membership card with you.
Step 2: Capture your results and receive your AIA Vitality Points
1.
After your Vitality Health Check, the pharmacist will give you a report with your Vitality Health Check results.
2.
Log on to AIAVitality.com.sg to capture these results (www.aiavitality.com.sg/memberportal/points_submission).
3.
You will need to upload an image of your results as a part of this process.
4.
You will be awarded your AIA Vitality Points once the results are uploaded.
AIA Vitality Points
You will earn 750 AIA Vitality Points for completing each component of the Vitality Health Check, and an additional 750 AIA Vitality
Points for each component if your measurements are within the healthy range.
This means you can earn a total of 4,500 Vitality Points if your BMI, blood pressure and blood glucose are within the healthy range
stated below.

You need to capture your results on the AIA Vitality member portal after your Vitality Health Check in order to have your points
awarded. AIA Vitality Points will be reflected on your AIA Vitality Points Statement 24 hours after the results are captured.
In addition, you may be awarded AIA Vitality Points when you have your BMI and blood pressure measured during the Vitality
Fitness Assessment. This is provided you have not already been awarded any AIA Vitality Points for these measurements earlier in
the membership year

Important points to remember

There is no limit on the number of Vitality Health Checks you can do in a year, but you will only be awarded points once per
membership year for completing the Vitality Health Check.

It is your responsibility to log onto AIAVitality.com.sg and capture your results in order to receive your AIA Vitality Points.
You will need to upload an image of your results as a part of this process.

You need to take your AIA Vitality membership card with you for the Vitality Health Check.

You can use the Guardian Health & Beauty vouchers to purchase any product at Guardian Health & Beauty stores
nationwide, except prescription medicines.

Your entitlement to, and enjoyment of, this Benefit is subject to the rules herein and the AIA Vitality Terms and Conditions
(see www.aiavitality.com.sg/memberportal/terms_and_conditions). In the event of any conflict between this Benefit Guide
and the AIA Vitality Terms and Conditions, the AIA Vitality Terms and Conditions shall prevail.

By signing up for a Vitality Health Check under this Benefit, you acknowledge that you have read these rules, the AIA
Vitality Terms and Conditions (seewww.aiavitality.com.sg/memberportal/terms_and_conditions), including the disclaimers
below, and that you accept and will be bound by the terms hereof.

In the event of dispute, the decision of AIA Singapore and Guardian Health & Beauty shall be final.
