Woodbridge High School “Enchanted Forest” – Winter Formal Dance Saturday, February 7th, 2015 8pm – 11pm Knott’s Berry Farm – Boardwalk Ballroom Our care and concern for the safety and welfare of our Woodbridge High School students has prompted the use of this agreement. A dance permission slip for EACH WHS student must be submitted. Guests from other schools MUST complete information on back of this page. Guests pay the “ID Card Only” price. To purchase a ticket for the dance, students must have their WHS ID. You will need your WHS ID to be admitted into the dance. Tickets may be purchased in Activities during break or lunch only. ** Clear your financial obligations so that you can buy a ticket** Monday, Jan 12 – Friday, Jan 16 Tuesday, Jan. 20 - Friday, Jan. 23 Monday, Jan. 26 –Thursday, Jan.29 Tuesday, Feb.3 - Wednesday, Feb. 4 ASB Card: $60 per ticket ASB Card: $60 per ticket; ASB Card: $65 per ticket; ASB Card: $70 per ticket; ID Card Only: $65 per ticket ID Card Only: $65 per ticket ID Card Only: $70 per ticket ID Card Only: $75 per ticket *** Ticket sales will end at 1:15 p.m. on Wednesday, February 4th. No Exceptions!!*** This agreement is to clarify expected behavior prior to and during any school activity. The consequences are delineated in accordance with the IUSD Substance Abuse Policy. A Breathalyzer will be on the premises and used at the discretion of the WHS staff. To be admitted to the dance, WHS students must have their student ID cards. Guests may use a student ID or driver’s license. Tickets are non-transferable. The dance will be held rain or shine. Anyone in violation of the Dress code will not be admitted. FORMAL Dress Attire is required. No costumes and No Jeans will be allowed. Dress/skirts must minimally be at least hand length No backless garments that extend below the waist. No bare midriffs/excessively low cut tops Dress/Skirt slits may be no higher than fingertip length No exposed undergarments/see through dresses/bare sides No bandanas, chains, or canes If clothing becomes altered during the dance, students may be asked to leave. No refunds will be given. Rave paraphernalia is not allowed at any time during the dance. Zero Tolerance Policy. Any student found to be “under the influence”, or to have consumed, or in possession of alcohol or any other controlled substance will be suspended for 5 school days and excluded from all school activities for an additional 10 school days. Three days of substance-abuse counseling is mandatory. DANCING GUIDELINES No inappropriate touching (hands on waists or shoulders only) No front to back touching No prolonged public displays of affection No straddling legs Inappropriate dancing such as “freaking” or “crowd surfing” will result in the following consequences: First violation = ID confiscated to be claimed from administrator after the dance, documented in discipline file Second Violation = Immediate removal from dance, documented in discipline file Students will not be admitted to the dance after 10:00PM. Once students leave the dance, they may not return. Latest arrival time to the dance will be two hours after opening. No refunds if the arrival time is missed, no re-admittance once student has left. Students are expected to be picked up from the dance no later than 15 minutes after the dance is over. To honor the time of the WHS staff, we ask parents to plan accordingly. In case of an infraction involving a minor, a parent will be contacted to pick up their student from the dance. In the event that they can’t be reached, the incident will be referred to the Irvine Police Department. **COMPLETE THE INFORMATION ON THE BACK OF THIS CONTRACT** Failure to comply with requests of any Staff member will result in immediate dismissal from dance without a refund. Non-Woodbridge students must obtain an administrator’s signature even if over 18. Guests must be under the age of 21 and in at least 9th grade. **Non-Woodbridge guests who have completed high school must provide a copy of a valid stateissued photo ID to purchase a ticket and must bring the ID in order to be admitted to the dance. We are asking parents not to provide alcohol for students and to monitor the activities of their son or daughter before and after the dance to avoid any unfortunate incidents.
