Kindly note it has been decided to invite quotations in two bid i.e. Technical and Price bid separately through eprocurement. The offer is to be submitted through our website http://ofbeproc.gov.in . The offer in form of hard copy will not be
accepted. The eligibility and qualification criterion of tender is enclosed as Annexure-I. The scan copy of the relevant documents
should be enclosed with technical bid. The quotation in price bid shall be filled as per price bid format (Annexure-II) and template.
The EMD of amount ` 65,370/- and tender fee of ` 100/- should be submitted with technical bid otherwise the offer will
be ignored. The scan copy of EMD and tender fee should be attached with technical bid. The EMD and tender fee should reach in

Annexure-A
1. NAME OF WORK: Labour contract for cleaning,house-keeping,material handling, and
maintenance of gardens and lawns of GCF Hospital
2. PERIOD OF WORK: 365/313 working days
3. TOTAL LABOUR REQUIRED: Unskilled labourers-14, Dhobi (Unskilled labourer)-01,
Mali (High-skilled labourer)-01 and Supervisors (Semi-skilled
labourers)-01
4. SCOPE OF WORK / JOB CONTENT: [as per Annexure-B]
TERMS AND CONDITIONS:
I. WORK EXECUTION
1. Contractor, his supervisors and his workmen will have to act upon as per the directives given to them by the
factory representative (site-in-charge) from time to time in respect of work awarded to him. Contractor’s
Supervisor should meet the site – in – charge at least twice in a day and apprise him with the progress of the
work and take necessary guidance. Records/entries/instructions of such guidance will be made in a register to
be maintained by the contractor/supervisor which will be countersigned by the site-in-charge or nodal officer
nominated.
2. The contractor has to provide effective supervision during the period of contract.
3. The contractor will visit, survey and study the site and the quantum of work before submitting his quotation.
4. The contractor has to prepare and submit the planning schedule, programme chart, deployment details, list of
tools and equipment in consultation with the officer-in-charge of the respective area for which the contract is
concluded, before commencement of the contract.
5. The work related to all types of monitoring, supervision, administration and execution of the contract lies with
contractor. For any faults, damage, complaints, the contractor will be held responsible and shall be liable to
penalty as decided by the management.
6. The contractor must ensure that the workers are deployed for only 06 days a week with 01 days off in a week.
7. The contractor shall maintain the following Registers :
a)
Muster Roll in Form-XIV
b)
Register of Wages Form-XVII
c)
Register of deduction for damages or loss in Form-XX
d)
Register of Fines in Form-XXI
e)
Register of Overtimes in Form-XXIII
f)
Wages slips in Form-XIX should be issued to contract workers one day prior to the date pf payment
of wages.
g)
Register of persons employed in Form-XIII
h)
Employment Card to be issued to Contract Workers in Form- XIV
i)
Register of advances in Form-XXII
8. There will be periodical surprise checking of the services provide by the contractor for the subject job by the
representative of General Manager. In case of default, the contract will be short closed forfeiting the
performance security deposit.
9. Workers will be paid wages as per prevailing rate of minimum wages of respective schedules. Workers should
be paid their wages in presence of designated authority nominated by the factory. Such certificate should be
forwarded with claim bill for release of payment.
II. SECURITY & PVR FORMALITIES:
1. The contractor and his workmen shall follow factory’s security rules in force from time to time. General
Manager reserves right to disallow any of the contractor’s workmen from being deployed in the Factory and
estate area without assigning any reason. The contractor will submit PVR for all the workers engaged by him
including 20% extra to cater for the absenteeism before the start of contract.
2. The Security/Authorized section will maintain register for the Contractor’s workmen and obtain their
signature while coming “IN” and going “OUT” or as the case may be and user section will also maintain
register in the same way. Certificate of attendance will have to be enclosed duly countersigned by I/C user
section while submitting the bill for the payment and respective attendance registers to be produced whenever
required by audit authorities.
3. If any employee of the contractor is found to have committed misconduct or misbehaviors, the factory at its
sole discretion, may direct the contractor to remove such employee and the contractor shall remove such
employee(s) without questioning the decision of the factory.
4. The personnel deployed by the contractor will not have any business connection/ dealings with any
agency/agencies directly or indirectly associated with the Factory. The personnel deployed by the contractor
shall not engage themselves in any undesirable activities whether within the factory or in the estate area.
III. SAFETY TERMS:
1. Contractor has to make his own arrangement for consumables/tools/safety equipment and personal protective
equipments/material handling equipments etc. required for use by his workmen to carry out the assigned task.
The cost of consumable materials will be paid to the contractor as per terms and conditions of the contract.
However, the contractor shall provide tools, safety equipment, personal protective equipments, material
handling equipments etc. by his own arrangement and the charge for these items will not be paid separately.
Contractor will ensure that his workmen always adhere to safe working practices.
2. All the Contractor workmen will be provided with red/maroon colour jacket with the contractor’s name
written on the back and ensure that they wear the same while entering factory/work place.
3. No housing accommodation to the personnel deployed by the contractor will be provided by the fact
IV.TERMINATION & PENALTY CLAUSE:
1. In case of unsatisfactory performance/services, the contact will be liable to be terminated at
any time giving 30 days notice and forfeiting the Performance Security.
2. General Manager reserves the right to suspend the contract at any time without assigning any reasons.
3. The average attendance of contract labourer during the month should be atleast 80% otherwise the
penality of Rs 5000/- shall be imposed on the contractor in that particular month.
V.ARBITRATION: In case of any dispute between the factory and the contractor, it will exclusively
be referred to a Sole Arbitrator to be appointed by the DGOF/OFB. The decision of such sole
arbitrator shall be final and binding on both the parties to the dispute.
VI. LEGAL CLAUSE:
1. Workmen below the age of 18 years and above 60 years should not be employed by the contractors.
The working hours be generally restricted to 8.00 hrs a day excluding lunch break of 1 hour.
2. The employees of the contractor should not have any right/claim whatsoever for seeking employment in the
factory.
3. The contractor & workmen engaged by the contractor should not have any adverse police record and should
have good character. Clear PVRs should be submitted by the contractor to the factory management before
engaging any of their workmen. The contractor will be responsible for good conduct and behavior of himself
and his men deployed in the factory.
4. The factory management will not be held responsible for any accident or injury to the men of the contractor
and for converge of any risk involved or liability arising out of any accident etc. which will have to be born by
the Contractor. The contractor will be responsible for any accident including loss of life or Injury suffered by
any of his workmen during discharge of the duty and also responsible for payment of compensation to their
employees as per labour laws. If need arises Factory management will recover such amount form the
contractor to effect payment to the affected contractors employee.
5. The contractor will have to abide by labour laws including Payment of Wages Act, Minimum Wages Act, and
Contract Labour Regulation Act. , Workmen’s Compensation Act.(wherever applicable), Factories Act and
other legislations and notifications of Government of Madhya Pradesh made applicable form time to time.
The provisions of these labour laws shall be made applicable to the Contractor’s workmen as the case may be
during the period of contract.
6. The contractor, his supervisor and workmen will ensure that no damage is caused to any of the government
property of the factory. In the event of any damages caused, the cost of damages would be compensated by
contractor or would be deducted form the bill/security deposit of the contractor based on market value.
7. Contractor has to submit a copy of license granted to him under Contract Labour Regulation and abolition Act.
1970 (and the Rules made there under by the authorities) within a period of 30 days of the award of the
contract.
8. Apart from above important notes/ directives, the contractor shall also abide by other Terms and conditions as
stipulated in the relevant contract depending upon the circumstances, as the case may be.
9. The contractor shall not appoint any sub-contractor for the work assigned to him. If any subletting is noticed,
the contract will be short closed forfeiting PSD (Performance Security Deposit).
10. The contractor will have to take out the following insurance policies to cover all the risk and keep them valid
till the successful completion of the contract:
(i) Workmen’s Compensation (if applicable) (ii) Risk/Liability of the 3rd party
11. All the rates quoted by the firm should be in prescribed format attached with the tender. Quotation not in
prescribed format is liable to be treated as invalid.
12. Quantity Increase (Option) Clause: The factory reserves the right to increase quantity of the order at the
prevailing rates, terms and conditions during the validity of the contract. However, the option will not exceed
50% of the original ordered qty.
VII. PAYMENT TERMS:- Payment shall be made on submission of :a) 100% payment of labour charges will be made on the basis of following documents:
i. Job Completion Certificate issued by User section representative.
ii. Attendance Sheet (Signed by JWM/paying authority of user section)
iii. Wage disbursement certificate in presence of paying authority of respective section. The WIC &
EIC of the user section will certify bank statement that shows the payment of wages by the
contractor to the worker.
iv. The contractor should also submit the bank account details of all his workmen within one week of
commencement of contract. The payment of wages of the workmen should be made through bank
account only. The contractor should submit monthly bank account statement of workmen clearly
mentioning the payment of monthly wages on commencement of contract.
b) 100% payment toward government contribution of EPF and ESIC will be reimbursed on actual basis on
submission of EPF and ESIC Challan. Contractor is required to submit EPF and ESIC return and other
relevant documents periodically.
c) 100% payment for the consumables and equipment charges will be made on prorata basis every month
on submission of usage/completion certificate issued by the user section.
d) 100% Service Tax will be reimbursed on submission of service tax challan along with relevant
documents or as per latest Govt Order.
e) Form 6A should be submitted within one month of end of contract period. The contractor should satisfy
the signing of withdrawal forms and Form 3A of employees to the management within one month of
end of contract. These two points would have to be satisfied before claiming last bill and performance
security deposit.
VIII. GENERAL NOTE ON TENDER ENQUIRY:
1. Conditional offers/offers deviating from the TE specifications offers with unreasonable total cost of
consumables and equipments charge i.e. the offers with below 70% of reserve price fixed by the factory for
such items shall be treated unresponsive and shall be summarily rejected. The cost of consumables and
equipments charge in the offer taken together shall be taken for comparison with total value of reserve price
fixed by the factory.
2. The contractor shall quote minimum service charges (i.e profit) of amount 5.6% of labour and consumable
material/equipment charges as fixed lump sum amount. The service charges shall be fixed during the currency
of contract.
3. Prices shall be quoted by the contractor both in figures and words.
4. Earnest Money Deposit (EMD): Tenderers shall submit the Earnest Money Deposit in the form of a crossed
Demand Draft, fixed Deposits Receipts, Banker’s Cheque, Bank Guarantee (BG) from any scheduled
Commercial Bank in favour of General Manager of the factory, otherwise their offer may be ignored. The
firm’s already registered at the factory/Sister Ordnance Factories contract need not pay EMD.
5. Offer should have a minimum validity of 180 days from the date of opening of the Tender.
6. The Tenderer should offer rate confirming to the specification indicated in the Tender. However, if he is
offering deviation such deviations should be indicated clearly to evaluate the offer. In no case the request for
change in specification after placement of supply order will be agreed to.
7. Unsolicited letter/canvassing /post tender correction: - Canvassing by Tenderers in any form, including
unsolicited letters on tenders submitted or post tender corrections shall render their tenders liable for
summarily rejection.
8. Contractor must start the work within 07 days after acceptance of Tender/Issuance Supply Order or as
incorporated in Supply Order.
9. Risk Purchase: If the firm fails to complete the work delivery period, factory reserves the right to complete the
same at firm’s risk and cost.
10. Jurisdiction of Court: The Civil Court of Jabalpur shall only have jurisdiction to all Civil Suit / legal
proceeding arising out of in any way touching or concerning to the case.
11. Penalty For Use Of Undue Influence : The contractor has to undertake that he has not given, offered or
promised to give directly or indirectly any gift, consideration reward, commission, fees, brokerage to any
person in service of the Factory or otherwise in procuring the contract or for bearing to do or for having done
or for borne to do any action in relation to the obtaining or execution of the contract or any other contract with

the Government for showing or for bearing to show favour or disfavour to any person in relation to the
contract or any other contract with Government. Any breach of the aforesaid undertaking by the contractor or
any one employed by him or acting on his behalf , as defined in the Indian Penal Code, 1980 or the Prevention
of Corruption Act, 1974 or any other Act enacted for the prevention of corruption shall entitle the Factory to
cancel the contract and all or any
other
contracts with Contractor and recover from the Contractor the amount of any loss arising from
such cancellation also shall render the Contractor to such liability, penalty as the Factory may deem proper,
including but not limited to termination of contract, imposition of penal damages, forfeiture of the Security
Deposit and refund of the amounts paid by the Factory.
A decision of the Factory or his nominee to the effect that a breach of the undertaking has been committed
shall be final and binding on the Contractor. Giving or offering of any gift, bribe or inducement or any attempt
to any such act on the behalf of contractor towards any officer/Employee of the factory or to any other person
in a position to influence the decision of the Factory directly or indirectly or any attempt to influence any
Officer/Employee of the factory for showing any favour in relation to this or any other contract, shall render
the contractor to such liability, penalty as the factory may deem proper, including but not limited to
termination of contract, imposition of penal damages, forfeiture of the Security Deposit and refund of the
amounts paid to the Factory.
The contractor shall get open the bank account of all the contract labourers engaged for the work. The
payment to the contract labourers shall be made to their bank account.
The contractor’s performance in respect of similar nature of work undertaken by them at the factory or in any
of the units of the O.F. Board Organization or Central Government/ State Government/PSUs should be
entirely satisfactory otherwise his offer will not be considered.
In case two or more quotations are equally placed in terms of cost and other conditions, the contract will be
awarded to the contractor with higher vendor rating, in the absence of which, the same will be equally divided.
The order shall be placed on the firms in alphabetical order of the name of the firm in case of division of
contract on more than one firms.
PSD – The successful tenderer will have to pay performance security deposit of 10% of the contract value in
the form bank guarantee/DD/FDR.

Annexure-B
Contract for Cleaning, Ward House-keeping, Material Handling & Maintenance of
Gardens and Lawns in Hospital
SCOPE OF WORK
1. General:
1.1 Bidders are advised to study carefully the various clauses contained in the Terms and
Conditions before submitting their tender bids.
1.2 Contractors who are interested to quote for the above services are advised to visit Gun
Carriage Factory Hospital, Jabalpur to physically inspect the site and acquaint themselves
with the facilities available, and nature and quantum of work involved before submitting
the tender bid. The areas wherever mentioned are indicative only and contractor has to
complete the work on “Whole job” basis.
1.3 The contract shall be awarded on two bid system. Price bids of only those firms shall be
considered who shall qualify in the technical bid.
2. Period of contract:
2.1 The contract shall be for a period of 12 months from the date specified in the Supply Order.
3. Scope of work:
3.1 This contract is for provisioning of cleaning, ward house-keeping, miscellaneous manual
material handling, washing of hospital linens, and up-keeping of the hospital gardens and lawns
in Gun Carriage Factory Hospital, Jabalpur. The job involves provision of a clean environment
for patients, visitors, and staff, ensuring high standards of environmental hygiene aiming at
reduction in incidence of hospital associated infection. The cleaning services are to be provided
round the clock on all days including holidays whereas other services are to be provided on all
factory working days (including OT working days) in an effective and efficient manner as per
the specifications given using well-trained, experienced and well mannered personnel and
quality materials.
4. Termination and Extension of contract:

If the contractor fails to complete satisfactorily the contracted work or any portion there of or
refuses to comply with any direction given in this regard to him, the contractor shall forfeit
the Security Deposit and the contract shall be terminated at his risk and cost.
The contract can be terminated by factory management by giving one month notice to the
contractor. The contractor is permitted to terminate the contract by giving minimum 3 months
notice in writing to factory management. During the period of notice the party shall continue
to discharge all their duties and obligations.
General Manager, Gun Carriage Factory, Jabalpur reserves the right to extend the tenure of
the contract by a maximum of 50% of the total annual value at the existing rates and terms
and conditions.

6.2 STANDARDS OF CLEANLINESS: The contract is expected to achieve visible and
discernable outcomes achieved as a result of the cleaning process. The contractor is therefore
expected to provide the services of cleaning of the hospital to attain, constantly maintain and
demonstrate the required standard of cleanliness.

6.3 CLEANING FREQUENCIES AND METHODS: The cleaning frequencies and methods of
cleaning given below are indicative of minimum requirements only. The service provider is
required to provide spot cleaning by employing suitable means and methods, as deemed
necessary to meet the required cleanliness standards. Indore cleaning should be both
convertional & mechanized cleaning as per area and requirement and that is superior quality.
6.4 AREA TO BE COVERED: Area to be covered, number of units in an element etc mentioned
are approximate. Minor variations (± 10%) may occur during contract period due to various reasons.
The total number of these articles available in the hospital at various locations is given in
Annexure 1.

7. Standards of cleanliness:
7.1 Overall Appearance:a) The area should be tidy, ordered and uncluttered

7.2 Odour control:a) The environment and equipment should smell fresh and pleasant.

7.3 General standards of cleanliness:a) Surfaces of the building, fitments, furniture, equipment, etc, including underneath and
inaccessible areas (edges, corners, folds, crevices, narrow spaces) should be visibly clean.

e) Floors, wall tiles, wall fixtures (including soap dispensers and towel holders) and bath
mats are free of dust, grit, smudges/streaks, mould, soap build-up and mineral deposits.
f)Plumbing fixtures are free of smudges, dust, soap build-up and mineral deposits.

10.1. Biomedical waste handling, treatment and disposal - Collection of biomedical waste from

12. WORK SPECIFICATIONS (Ward House keeping):

13. WORK SPECIFICATIONS(Garden):
13.1 The areas where the gardens are to be developed and the areas where there are established
gardens and lawns which are required to be maintained are detailed in Annexure 8.
13.2 Proper growth and maintenance of the existing garden, lawns and plants in the above areas
and potted plants in cement and earthen pots.
13.3 Watering on daily basis including drip or sprinkle.
13.4 Mowing of lawn as required.
13.5 Removal of weeds, undesirable plants, wild grass, dry leaves, litter etc to keep the area
clean.
13.6 Beautification of garden by dressing/shaping of plants and trimming of hedges.
13.7 Applying of manure, fertilizers and change of soil as and when required.
13.8 Spraying insecticides, pesticides, as and when required.
13.9 Painting of tree guards, pots etc.
13.10 Maintenance of existing fountains.
13.11 Proper care will be taken to ensure that there is no damage to the lawn and flowering
shrubs in the premises.
13.12 Planting, growing and maintaining seasonal flower plants in summer and winter.
13.13 Placing and rotating flowerpots at various places in hospital.
13.14 Flower arrangement in specified rooms/places on specified occasions.
14. WORK SPECIFICATIONS(Dhobi):
14.1 The person engaged in washing hospital linen should be well versed in handling soiled clothes
and laundering work.
14.2 He should follow the instructions outlined at 16.7 above.
14.3 In the absence of the hospital washer man he should keep a record of clothes received and
hand them back after laundering to the Ward Master in good condition.
14.4 Soap and detergents shall be supplied by the hospital.
14.5 After wash the clothes should look clean and should not be torn.
15. Equipment & Materials:
15.1 The contractor shall arrange all equipments, tools, materials and transport required for the
cleaning and garden work. The cost of these items should be included in the rate for the
work and no separate payment shall be made.
15.2 The list of bare minimum equipment, tools and consumables required is given in
Annexures 3 thru 7. The Cleaning Tools and Garden Equipments should be new. The list
is indicative and additional equipments, tools, materials as required shall be used by the
contractor to achieve the required standard of work.
15.3 Before the contractor shall be allowed to start work the successful bidder has to submit
representative samples of ALL consumable items that shall be required to as mentioned in
Annexure 3 thru 7 with a documented list of their costs in the form of Cash Memos etc.
The quantity of such representative samples that shall have to submitted is given in
Annexure 11.
15.4 In case the Contractor fails to provide documented rate list of consumable items as
specified in Annexure-11 and fails to supply the items in the requisite quantity / of
required quality a Board comprising of representative of Hospital, Finance and PV shall fix
the rates by suitable market rate and deduction of an amount owing to short/non supply of
the items as per clause 21.5 vide infra shall be made rates so fixed thereon.
15.5 Penalty for Non/Short Supply : In case of non-supply/ short supply of these consumables
THREE TIMES the cost of these items as declared by the bidder, of such short supplied
items, shall be recovered from the bill of the Contractor.
15.6 All consumable materials should be brought once in a month in the beginning for
inspection by the MO-in-Charge, Contract Services and proper record to be maintained of
their receipt and issues.
16. Disposal of waste materials:
16.1 All rubbish/mud/silt/rubbish collected from dustbins should be collected daily at a place
earmarked by hospital authority. The same shall be disposed off to the site earmarked by
the factory authority and to be treated as directed once a week.
16.2 Biomedical waste must be disposed as per hospital protocol.
17. Staffing:
17.1 The contractor has to provide the service in three shifts on ALL days for Cleaning and
Ward House-keeping Services. Other services shall be required on factory working days
only.
17.2 The manpower requirements are given as Annexure 2. This is a bare minimum
requirement required to be present during various shifts. Daily detailment shall be as per
Matron’s order and as per the actual requirement of the time.
17.3 The above manpower requirement does not include ‘weekly off, leave, absence’
requirements.
17.4 To ensure deploying required manpower, the contractor shall maintain sufficient additional
staff to meet ‘weekly off, leave, absence’ requirements.
17.5 The departments shown indicate the reporting area. The contract workers will be required
to carry out work in any department of the hospital, FHC etc as required. Though
requirements are indicated for certain departments on holidays, whenever required the
manpower shall be provided by the contract workers posted to wards.
17.6 The contractor shall not engage any person below the age of 18 years for the work.
17.7 None of the personnel engaged by the contractor against contract shall have any right/claim
whatsoever for permanent employment in the factory.
18. Skills and qualifications
a. The supervisor and workers should have knowledge, skills and experience in carrying out
cleaning and housekeeping activities especially in the health care institutions.
b. They should also possess knowledge, skills and experience ini. Personal hygiene measures including hand washing and skin care
ii. Safe disposal of sharps
iii. Safe handling of Bio medical waste
iv. Reporting accidental exposure to blood and bodily substances
v. Correct handling spillages of blood or bodily substances
vi. Handling of soiled linen
vii. Use of personal protective equipment
viii. Awareness of safety precautions to be observed in the work
ix. Safe manual handling practices like Lifting, Pushing, Pulling, and Carrying,
using ladder.
c. The personnel posted should possess good health, good manners, and good character. They
should be responsible, reliable, punctual and amenable to discipline. They should be polite
and exhibit good manners with hospital authorities, staff, patients and visitors.
d. The staff should not be in the habit to taking liquor or other intoxicating substances and
must not be under the influence of alcohol or other intoxicating substances while on duty.
Upon report of any misconduct by the labour employed by the contractor, he (the
contractor) should immediately replace them.
e. At the beginning of the contract, Contractor shall provide a panel of staff proposed to be
employed by him. The contractor shall at his own expense get the character/anecdotes of
his employees verified through the local police. The contractor shall provide photo identity
cards to the labourers employed by him duly countersigned by the HOS of Hosp.

f. Only persons from the panel shall be allowed to work. Any additions and deletions to the
panel should have the prior permission of the MO/Quality, Gun Carriage Factory Hospital,
Jabalpur. The authority of Gun Carriage Factory, Jabalpur reserves the right to reject any
name in the panel so submitted by the contractor.
g. All persons employed by the contractor shall possess valid photo identity pass while on
duty.
h. Penalty : Failure to submit copies of valid PVR of the employees engaged by the
contractor within one month of the contractor to the Hospital Office shall attract a penalty
of ` 500/- per such month or part thereof, of non submission excepting the first month.
19. Uniform and Personal Protective Equipments:
a. The contractor workers shall report in uniform dress. The uniform shall bear name of the
contractor/firm. The uniform shall be got approved by the contractor from Medical Officer-incharge hospital. Description and scale of issue of the uniform is given in Annexure-10.
b. The contractor has to arrange for issue of sufficient numbers of uniforms for its workers along
with provisions for its washing and ironing.
c. Protective clothing and equipment should be readily available in appropriate size ranges and
accessible one set in clean and good condition to all employees. They include:
i) Footwear, goggles, gloves, mask, apron etc.
ii) Personal cleaning materials
d. Workers shall not be allowed to work without personal protective equipment or specified
uniforms.
e. Penalty : Failure to provide recommended uniform to the employees engaged by the Contractor
within one month of the Contractor shall attract a penalty of ` 500/- per such month or part
thereof, of non submission excepting the first month.
20. Management of the service:

Contractor and his supervisor are responsible for ensuring that the work is carried out as per
the Terms and Conditions of the contract. For supervision and control of workers
MO/Quality, who is the Works-in-charge, shall deal with them through the supervisor only.
The supervisor is responsible for ensuring that-
i. Manpower is deployed as per the schedule and work needs.
ii. The workers are provided with and properly use specified personal
protective equipment.
iii. The workers are provided with and properly use required tools, equipments
and materials
iv. Records, check lists etc regarding the daily attendance of the workers,
satisfactory completion of the work etc as required by Works-in-charge are
maintained up to date and signed by the staff concerned on day-to-day basis.
v. All the statutory requirements are fulfilled; all the statutory registers and
documents are maintained correctly and up-to-date, and the same are
produced to Gun Carriage Factory, Jabalpur / statutory authorities when
called for.
The contractor shall bear the complete responsibility of conduct and punctuality of the
personnel employed by him. The contractor shall remain fully responsible for any undesirable incidence/situation caused/created by their conduct/activities.
The contractor will not permit his employees to participate in any trade union activities or
agitation in the premises. All demands/disputes of personnel engaged by the contractor or
their Union will have to be settled by the contractor himself. Gun Carriage Factory, Jabalpur
administration will in no way be involved and shall not be responsible for any settlement of
such disputes.
21. Safety and Security:
The contractor shall be responsible for his employees observing all the security and safety
regulations/instructions as may be issued by the Gun Carriage Factory, Jabalpur and the
hospital from time to time.
22. Facilities to be provided by Gun Carriage Factory Hospital, Jabalpur:
A room will be provided to the contractor for keeping his equipment, tools and materials at his
own risk. The contractor shall make his own arrangement for locking. Gun Carriage Factory
Hospital, Jabalpur shall not be responsible for the safety of the contractor’s items.
23. Performance evaluation:
Contractor or his authorized representative shall meet the Works-in-charge, or his
representative once a week for review of the work done, review the cleaning processes with a
view to continuous improvement and plan the work for the future.
23.1. Work carried out as per schedule:
1) The cleaning work has to be carried out as per the time schedule prepared in
consultation with the Matron, nurse/staff in charge wards/departments taking into
consideration the needs of the particular area and the needs of the patients.
2) Suitable work diary/check-lists shall be maintained by the supervisor for this
purpose and the signature of the authorized hospital staff will be obtained as proof.
23.2. Provision of adequate materials of required quality:
1) Supervisor shall maintain documents of
i. inspection of monthly stores signed by Works-in-charge
ii. supply/use of periodic materials such as deodorants, naphthalene balls etc
confirmed by staff responsible for the area of work shift wise and
countersigned by Works-in-charge.
23.3. Quality of Output:
1) In order to ensure quality of output an Objective Assessment System shall be in place in
the form of Cleaning Inspection Sheet. In order to achieve the desired standard of cleanliness
Acceptable Quality Level for individual functional areas and for overall hospital MUST be 85%.
2) The periodicity of such Inspection shall be once in a month by two members from the
hospital nominated by the MO I/c for this purpose.
3) A sample sheet of such inspection Sheet is reproduced at Annexure –9 for information
and guidance of the contractor.

Eligibility and qualification Criteria:
a) Firm should submit details like proprietary affidavit/ partnership deed/ certificate of
incorporation/Memorandum and Articles of association
b) The contractor must submit labour license issued by Labour Commissioner for the executed supply
order/completed work to prove that they have executed at least one contract with valid license.
c) The contractor must submit document of valid registration with EPF
d) The contractor must submit document of valid registration with ESIC.
e) The contractor must submit document of valid registration with Service tax
f) The contractor should have executed at lest one work order of 80% of estimated value or two work
orders of 50% of estimated value or three work orders of 40% estimated value. The relevant
documents for work experience should be submitted.
g) Average turnover of last three years should be minimum 30% of the estimated value. The relevant
documents for confirming the turnover of last three financial years duly certified by Chartered
Account should be submitted in place of balance sheets for last three years.
Declaration required:
a) The contractor shall agree to the terms and conditions of TE including scope of work.
b) The contractor / Firm shall submit an undertaking to the effect that in case of any outstanding
amount which becomes payable to the Government on account of any liabilities due to non
observance of provisions of laws/rules in force, the same shall be borne by them in its entire.
c) Furnish details of any other contract already awarded by the factory to him.
d) Bank Account No. and Name of Bankers of the Contractor/ Firm.

a) * Quote below than the minimum wages prescribed in the latest circular No J-92(1)/2014ES-III Dt.
08/04/2015 applicable from 01/04/2015 (Central Govt.) will be ignored. The rates are subject to
revision as per circular issued by labour commissioner (Central Govt.) Minimum Wages Act, 1948

b) ** The contractor shall quote these (B) charges as per prevailing rate and overall life of equipment
in the market. If the above charges are below 70% of estimated price, the offer will be ignored.
c) *** The contractor shall quote minimum service charge of amount 5.6% of total value of C above
otherwise offer will be ignored. The service charge shall be fixed lump sum amount during the
currency of the contract. Any bid where service charges is quoted in % (percentage) will be
rejected. This minimum (%) percentage of services charges will be calculated on the basic
minimum wages as applicable from 01/10/2014, which are: (i) Un-Skilled labour Rs.290/- per day
(ii) Semi-Skilled labour Rs. 328/- per day (iii) Skilled labour Rs. 385/- per day.
