$35 (+$25 for each additional sibling)
(this includes poms for girls, megaphones for boys, and a
SNACK)
*Learn cheers, band dance, and jumps.
*Craft time*Perform at SLHS during the Girls Varsity ½ time.
(Game starts at 4:00- Your family will be admitted at no cost.)
Please fill out a registration form on the back of this form. Mail it with your
payment or drop it off at the school between 8-4 Mon.-Fri. Registration received
after Friday January 29th will be accepted, but participant will not be guaranteed
cheer accessories.
Wear athletic shoes and a white, navy or maroon shirt.
Hair must be pulled back and for safety purpose no jewelry or gum is permitted.

Mini Cheer Camp
SLHS Cheerleading Mini Camp Medical and Liability Release:
________________________________________elects to take part in the SLHS Cheerleading event, which is sponsored by the SLHS Cheerleading squad. I/We understand that cheerleading is an activity in
which there are risks of injury. I accept and assume any and all risks associated with his/her attendance and participation in the clinic and its activities. I understand that my child must abide by clinic rules and the
instructions of the clinic staff. I/We agree to, and by the signing of the agreement, release the coaches, volunteers, staff of Sierra Lutheran High School from any claim of negligence by ourselves, our
son/daughter, our heirs, executors and assigns, from any liability arising from claims for damages for injury to our son/daughter and any claims for loss or damage to his/her property which may arise our of
his/her participation in the Sierra Lutheran High School Cheerleading Mini Camp on February 14, 2015.
I/We further acknowledge that the above individual is covered by health insurance the particulars of which are described below. I hereby agree that we are responsible for any required medical treatment, and give
permission for my child to receive medical treatment in the event that I am unable to be contacted. In order that participant may receive necessary treatments, I hereby hold Sierra Lutheran High School and their
Cheerleaders, and coaches harmless in the exercise of this authority.
