HOW TO FAX REQUIRED DOCUMENTS INTO YOUR APPLICATION
Go to http://www.mncred.org/, the Minnesota Credentialing Collaborative home page.
Click on the Individual Provider button.
This action takes you to the login page. Enter your Username and Password.
Click on Login

This action takes you to the Provider Home page.
Click on the application button at the top of the page.
This action will bring you to the individual provider credentialing application.

Go to the black navigation bar and click on Image List under Step 3.
This action brings you to the Image List Summary page.
Click on the Add New Image button.

This action takes you to the Add New Image page.
Click on the Fax Cover Sheet link.
This action will open and allow you to download and save your personal Fax Cover Sheet on your
desktop as a pdf. Print this Fax Cover Sheet. Place the Fax Cover Sheet in front of the document(s) that
you want to fax into your credentialing record, i.e. license, liability insurance, C.V., diploma etc. Fax this
information to the number printed on the fax cover sheet. After the document(s) are faxed, the document
is now referred to as an image and is located in your Image List as a line of numbers. Hints about faxing
in documents:
1. Make sure you do not put an additional Fax Cover Sheet over the MCC Fax Cover Sheet. This
fax cover sheet has a bar code that deposits your documents on the Image List in your application.
2. Do not write on the fax cover sheet as it may interfere with the reading of the bar code.
3. Make sure that the Fax Cover Sheet you printed does not have any lines or smudges from your
printer. This may interfere with the reading of the bar code.
4. If you receive the message that your document is to large you may need to check the settings on
your fax machine. Do not fax in color. You may want to experiment as all faxes are different.
5. During times when the system is busy it may take more than two hours for the faxes to appear on
your Image List. If these faxes do not appear after one day, do not refax. Call MCC Customer
Service at 612-360-9793 for assistance.
To see if you have successfully faxed your images, click on the Image List link.

This action takes you to the Image List Summary page.
Your image will enter the list in an alpha numeric format. You may change this name. Click on the View
button next to the first Image. Determine the type of document and then close. Double click on the box to
select the document, and type in a file name that will describe your document.
This will help you keep your images organized and easily identifiable. Once this is complete, you will
want to attach this document to the area of the application where it belongs. To do so, click on the Attach
button that is located to the right of the document.
This action brings up a dialog box.
Click on the arrow on the dropdown box. This brings up a list of the following options:
Insurance Coverage
Specialty/Boards
Licenses
Certifications
Education
ECFMG
Training
Teaching
Military

Select the most appropriate record type.
This action will bring up the name of the company or organization you have entered into your application.
Click on the Attach button that is located next to the company or organization that relates to the document
you are attaching.
This will then take you back to the Image List Summary page.
To check to see if the document has been attached to the appropriate area of your application, click on
Liability Insurance (for example) on the black navigation bar to check to see if the document has been
attached.

This action takes you to the Insurance Summary page.
Click on the underlined insurance carrier.
This action takes you to the Update Insurance page. This view is what the bottom of the page looks like.
The “Document Available” indicates that the document is now attached to the liability insurance.

Click on the VIEW button to make certain that your document is indeed the one you need in this area.
Repeat this process for all other documents that you have faxed into the Image List.
