It has been a very educational 3 years. I have enjoyed getting to know many of you better, learning about the complex workings of our organization, facing difficult challenges and growing as a person through the knowledge of my peers. We have had some rough times and tribulations, but we are healing and moving forward in many positive ways. We have a great meeting planned for you in our slightly abridged meeting in Chicago at the Embassy Suites. It may be shortened in length, but the content is still very good. I found an amazing group of volunteers to give courses and Jennifer Lambert has put together an outstanding program for you, starting on Sunday morning and going on through Monday. Also, do not forget to attend the Sunday Night Symposium, “Shake, Rattle and Roll: The Shimmy on Nystagmus”. Our Presidential Reception on Saturday night will be less elaborate, but still very nice. These changes are so important to keep down registration fees and avoid raising dues. This is necessary for the organization as a whole and sacrifices needed to be made in the most sensible areas. This is a great organization full of amazing and talented people, so be respectful of each other. Even if you disagree, listen, get the facts before reacting and remember that most people have good intentions. Your elected Board of Directors are good and wise people that want the AACO to thrive. Orthoptists have enough adversaries, so we need to be strong at our core. As Americans we have heard this all of our lives, “United we stand, divided we fall”, so remember that motto as an AACO member as well. JCAHPO Report Tribute Committee Over the last year, AACO meeting organizers have articulated an interest in incorporating poster sessions into the programs of their regional meetings, and have inquired about obtaining JCAHPO CE credit for these sessions. Working with JCAHPO, I was able to obtain approval for AACO meeting attendees to receive JCAHPO type A CE credit for attending a formal poster session review that would have to be given by the host organizers. For example, attendees could receive 1.75 CE hours for attending a 60 to 90 minute poster review discussion session of selected type A / Core poster abstracts. Thanks to the following members who have made donations to the Tribute Committee in the past year: The AOC made its annual $1000 contribution to the JCAHPO Education and Research Foundation on May 26, 2016. A new scholarship cycle begins in January 2017. JCAHPO Education and Research Foundation Scholarships were awarded to 5 orthoptic students totaling $1550.00. Two students from the University of Minnesota ($500 & $250), two from the University of Iowa ($300 each), and one from the Ross Eye Institute ($200) received scholarships. Miyazaki, CO AACO Vice President www.fresnel-prism.com 1-952-496-0432 PRISM, page 3 IOA Update: The Challenges and Rewards of Diversity The International Orthoptic Association Council of Management (COM) held its annual meeting just preceding the XIII IOA Congress (see article on page 7) in Rotterdam, Netherlands. One of the current projects of the COM is the development of universal documents that define and describe the worldwide practice of Orthoptics and its role in eye care. Such materials are necessary as our profession expands to other countries. This is an exceptionally difficult undertaking, however! Unlike pediatric or neuro-ophthalmology, the practice of Orthoptics varies extensively from country to country. For example, after undergoing appropriate education, our British colleagues can supply and administer prescription medications in a way that we cannot: by injection! In sharp contrast, in some states in the United States, orthoptists are prohibited by law from instilling cycloplegic eye drops, even at the direction of an ophthalmologist. Orthoptists in other member countries are also able to prescribe spectacles, while in the USA orthoptists are limited to refractometry. And in many others, orthoptists are able to work and bill for service independent of an ophthalmologist. In the United States, even when following the restrictive rules for coding, reimbursement for sensorimotor examinations are often denied. However, while the practice of Orthoptics varies, our external and internal threats to the profession are very similar. For example, several countries are experiencing escalating conflicts with organized optometry. These include South Africa, Switzerland, Austria, Portugal, Norway, the Asia Pacific Association, the Czech Republic, and China (Hong Kong). In these countries, optometrists are either competing successfully for orthoptic jobs, or are holding seats on the medical boards that control orthoptic practice. For example, in South Africa, the optometric-lead health ministry closed the orthoptist registry eight years ago with- PRISM, page 4 out informing the orthoptic association before or after the decision was made. In South Africa, one cannot practice without being registered with the Ministry, so closing the register meant that no new orthoptic positions - or new orthoptists - would be allowed. Here is a chart illustrating the number of ophthalmologists and optometrists per orthoptist for three different countries. The data is based on IOA Country Reports and World Ophthalmology Council data. It is interesting that although in all countries, optometrists outnumber ophthalmologists and orthoptists combined, the ratio of optometrist to ophthalmologist to orthoptist is most equal in the UK (10:2:1), where Orthoptics has been able to expand its scope, and most distorted in South Africa (136:30:1) where Orthoptics faces extinction. Of course, there are many different reasons for the similarities and the differences amongst the IOA countries. Some of these are differences in orthoptic education, qualification (licensure, certification, etc.), and national health systems. The predominance of optometrists compared to other eye care providers is only one factor that impacts orthoptists (and ophthalmologists) in many – but not all IOA countries. If you are interested in helping our few remaining South American colleagues (only eleven orthoptists), ask the AACO Board of Directors to submit a letter of protest to the South African Ministry of Health and Registry Board. The diversity that is an intrinsic part of organizations such as the IOA helps us to surmount challenges. As I close out my term as your IOA Representative, I want to encourage continued active participation in international Orthoptics at all levels, by the AACO and its individual members. Congratulations to Sarah Whitecross (former TCOS president), and current Co-presidents Jeff Locke and Liz Armstrong for organizing an outstanding meeting! Friday started off with an afternoon of “dynamic” workshops from orthoptists and ophthalmologists, providing many expert tips on dynamic retinoscopy (Teresa Warren), photographic documentation of strabismus (Jocelyn Zurevinsky, Nichole Pereira, Dr Raymond Ko), visual fields (Dr Michael Richards) and the examination and treatment of near-distance disparity esotropia (Dr Ian Clark). Saturday and Sunday, orthoptists joined COS pediatric and neurology sessions. One of the great points in the format of this meeting is that the TCOS, CSOMP (technical), and CSORN (nursing) sessions are held in the same venue as the COS meeting, allowing attendees to find a host of interesting and pertinent presentations to attend, and the opportunity to obtain maximum CE credits. This year, Michael O’Connor, MD entertained and educated as he presented the John Pratt-Johnson Lecture: “Mechanical” strabismus in adults: concepts, diagnosis, and management at the scientific session on Monday. Other highlights from the session included Dr. Stephen Christiansen’s presentation bringing our attention to auricular associations with strabismus, and a panel discussion with orthoptists and ophthalmologists exchanging ideas on difficult cases in strabismus and pediatric ophthalmology. The panel consisted of Drs. Stephen P. Kraft, Michael O’Connor, Stephen P. Christiansen, Linda Colpa, and Darren Oystreck. Nichole Pereira wrapped up the day, presenting a case of osteogenesis imperfecta with recurrences of trauma and retinal hemorrhage, helping us all to gain new knowledge about a rare condition. This was an excellent showcase of the depth and knowledge of our orthoptic colleagues, eight of the thirteen presentations were made by orthoptists and one by an orthoptic student. This is a great accomplishment for our small society! The TCOS cocktail party Sunday night at the Side Door Bar in the Byward Market gave everyone a chance to meet and mingle. The food was excellent and the company was sparkling. And for those who watch the Canadian news, we did not lose any attendees in the very large sink hole right in front of the Shaw Centre. 2017 marks the 50th Anniversary of TCOS and a big celebration is being planned. Vancouver Hosted AAPOS Meeting April 6-10 Vancouver, BC was the host city for the 2016 AAPOS conference. Beautiful landscape, great weather, and brilliant speakers set the stage for yet another successful AAPOS meeting. The meeting kicked off with an opening reception at the host hotel, the Pan Pacific Hotel. The night was filled with an irresistible array of food, the drinks were savory and there was jam-packed mingling of friends, colleagues, mentors and advisors. Alas, the fun was over and the learning began. Edward L. Raab, MD, JD was named the Costenbader Lecturer. He delivered a superb talk on the innovative history and treatment of exotropia. The remainder of the day was filled with talks on all forms of strabismus, and the ever changing treatment in ROP. An interesting talk on time requirements for EHR presented by Michael Chiang, MD made everyone in the room aware of how much time is spent documenting EHR charts. He showed the time requirements to be quite significant with much of this time spent after hours or on weekends. It was hard to choose which workshop to attend, as all workshops intrigued the eye nerd in me. The newest my- opia treatment workshop packed the house- standing room only for myopia - who would’ve thought it? The AACO workshop was my favorite. Kyle Arnoldi, Laurie Hahn-Parrott, and Vaishali Mehta shared some pearls of knowledge you can only learn with experience. Orthoptists were represented in the poster section - Sarah Whitecross, Helia Garcia, Evelyn Cohen, Illana Karshae, Lindsay Klaehn, and Marlo Galli shared their knowledge of strabismus, binocularity, visual fields, nerve palsies, and neuro-imaging. Saturday greeted us with a beautiful lecture: “Pursuit of Stereopsis” presented by the 2016 Knapp Lecturer, Pradeep I. Sharma, MD, FAMS. Not only was this topic near and dear to every orthoptist, Dr. Sharma sang a song using “AAPOS” as the refrain! The talk ended with a standing ovation in his honor. The last hurrah was Sunday morning, adding just a bit more education before everyone pulled out their passports and headed towards the airport. I look forward to what the future holds, and will keep up my duty as an orthoptist - keeping the world straight. The International Orthoptic Association hosted its XIIIth International Orthoptic Congress from June 27–30 in Rotterdam, Netherlands. Seven hundred members from 42 countries met to share knowledge, present research, and engage in social functions, while celebrating the work orthoptists from all over the globe take part in every day. The objective of the congress was to build bridges between different worlds, and IOA Congress Scientific Program Committee chair Kyle Arnoldi and her committee, which included AAOC member Carole Panton worked tirelessly to plan, execute, and encourage an environment of learning. Conducive to the learning environment was the downloadable mobile app offered for free to all attendees of the IOC. This app provided important information about session start times, paper abstracts, poster topics, and after-hours event times and locations. There was even the option for attendees to include their own notes and designate their favorite papers, posters, and presentations. There were two workshops on Monday. The first, titled “Bridging Worlds: Changes in Personal and Professional Identities of Orthoptists,” discussed the ever-changing role of orthoptists. Attendees learned how orthoptists’ skills are utilized worldwide, including in PRISM, page 6 non-traditional roles, the importance of orthoptists evolving their clinical skills to meet emerging healthcare needs, and developing ways to educate others regarding their roles in clinic. The second workshop, “Bridging Worlds Between Assessment and Intervention in CVI in Children,” aimed to provide a framework for understanding and working with CVI children, including discussing the causes, designing a framework for assessing children with CVI, describing the visual and behavioral characteristics of these children,and the interventions for children with CVI. There were 12 theme blocks over Tuesday, Wednesday, and Thursday. These were scientific sessions with concurrent poster displays. Two sessions were in the morning, followed by a lunch break, and two additional blocks ended each day. Presenters had eight minutes to speak about their research related to each theme block. Additionally, there were two “rapid fire” poster presentations after each block, where the first author of the two highest-rated posters presented a synopsis of their work after the formal presentations. After each block, there was an opportunity for attendees to visit posters that were displayed throughout the venue. The two Tuesday morning theme blocks were “Binocular Vision — Fusion” and “Binocular Vision — Stereopsis.” The Wednesday’s morning theme blocks were “Neuro-Ophthalmology” and “Genetics and Syndromes.” The afternoon blocks were “Technology in Eye Disease” and “Expanding Orthoptic Practice.” In the neuro-ophthalmology block, Gill Roper-Hall presented a paper, “Ocular Motility Dysfunction in Patients with Meningiomas,” and Sarah Whitecross presented “Visual Field Outcomes in Children with Neurofibromatosis Type 1 Associated Optic Pathway Gliomas.” Additionally, Angela Dillon presented a rapid-fire poster titled “Oscillopsia Due to Malabsorption Secondary to Wernicke’s Encephalopathy.” In the genetics and syndromes block, Jennifer Lambert presented a paper titled “A Prospective Observational Study of Ocular Comorbidities in Children with NAS: Study Design and One-Year Outcomes.” On Thursday morning, the theme blocks were “Screening, Vision, and Refractive Error” and “Amblyopia.” The afternoon blocks were “Strabismus” and “Strabismus Management.” During the strabismus block, Kathleen Curtin presented a paper titled “Large-Angle Congenital Exotropia Secondary to Absent Bilateral Medial Rectus Muscles,” and Sarah MacKinnon presented “Exaggerated V-Pattern Strabismus and Displacement of the Superior and Lateral Rectus Muscles Near Their Origins: Impact of Altered Boney Morphology of the Posterior Orbit in Systemic Craniosynostosis.” Two orthoptists were honored for their many years of work and the contributions they have made to the field. Kudos to Samantha Garner Pape for chairing this very well received, well organized, fun and informative meeting. Approximately 75 orthoptists came from near and far, excited to catch up with old friends and make new ones. The meeting had it all, excellent academic programs, social events including dancing courtesy of Bobby Pape (pays to have connections) and even a session on Mindfulness lead by Nina Palomba. Thursday afternoon session got off to an excellent start with talks from our orthoptic students. Thank you to Kylie Smith, VinhVy Nguyen, Rupa Nayee, Tori Rausch and Tony Klauer. Congratulations to their program directors. You have much to be proud of. The future of orthoptics is secure with the caliber of students we heard. In the neuro-ophthalmology imaging symposium, a panel of three neuroophthalmologists from Boston’s major academic centers was presented with 7 interesting and unusual cases by several Boston orthoptists. Their review of the imaging accompanying the diagnoses really helped us understand better how to navigate MRIs and CT scans. The lectures covered a wide range of topics throughout the meeting. Controversies in Diagnostic Testing” made us all think. There were several case presentations on the more unusual patients we might see in our practice, such as Spinocerebellar Ataxia Type 3 presented by Jennifer Lambert and Neuromyelitis Optica presented by Nina Palomba. The program included updates on Thyroid Eye Disease and Traumatic Brain Injury. The meeting ended on Saturday morning with a workshop on Retinoscopy for the Orthoptist with emphasis on dynamic retinoscopy. I was pleased and proud to see that most of the speakers were orthoptists. The Providence Eastern Regional AACO meeting was enjoyed by all. It was a perfect blend of learning and camaraderie with our fellow orthoptists. Shelley Klein, CO, COMT AAP News I am very pleased to report that the American Association of Pediatrics Board of Directors has approved our Section on Ophthalmology’s request to endorse the AAPOS statement on “Orthoptists as Physician Extenders”. The endorsement and document will be published in the Pediatric Clinical Practice Guidelines and Policies Manual and for the AAP News to announce to their membership. This is a huge accomplishment and has been in the works since March 2015. The AAP is a large and powerful professional medical society made up of over 64,000 pediatricians and pediatric medical and surgical subspecialists. To have the recognition and support of the AAP for our long term goal of securing our professional position in the medical field is paramount especially during these very ophthalmologic politically charged times. Other goals of the AAP Section on Ophthalmology include: • Increasing accuracy and percentage of eye examinations in pediatricians’ offices by increasing reimbursement for vision screening. We have provided pediatricians with quality, evidence-based education on vision screening and guidelines for vision screening, along with establishing quality metrics. • Increasing parental awareness of children’s vision health issues It is important that we support the AAP just as they are supporting us. Cindy excelled in all three. She is a researcher of the highest integrity who keenly identifies and investigates patterns and clinical questions using sound scientific methods. She has been an invited lecturer around the world, and has also delivered the Richard G. Scobee Memorial Lecture. She has served as a member of the AOJ Editorial Board for decades and is trusted by the Editors for her perspicacious and unbiased reviews. She is a skilled communicator, both as an author and a speaker, and has published her research in several peer-reviewed journals. Cindy is a frequent speaker at regional and national meetings and has served as AACO Program Chair. Her scientific and educational accomplishments were acknowledged by the IOA in 1999 when she was chosen to be Chair of the IOA Congress Scientific Program Committee for the congress in Stockholm, Sweden. She was responsible for broadening the reach of the IOA Congress to ophthalmologists by creating the first invited symposia at that congress. Cindy was a member of the American Orthoptic Council for almost a decade, and in that time served as Examination Co-chair for the AOC National Board Exams. During her tenure, she completely renovated the oral/practical exam, standardizing the scoring and streamlining the exam process. Her system formed the foundation upon which the AOC oral/practical exam has been built over the past 15 years. Cindy has held elected and appointed positions within the AACO including Treasurer, Director of Finance, Program Chair, and AACO Representative to the IOA. It is in this last role that Cindy has truly distinguished herself. As a member of the IOA Council of Management, Cindy was elected as Chair of what is now the International Cooperation Program. And in that position, she created and developed the IOA Volunteer Program, the Exchange Program, and the Partner Country Program. With the Volunteer Program, she coordinated clinical and teaching volunteers to bring their skills and expertise to many sites around the world, improving the quality of eye care worldwide. Her efforts have brought our profession to countries such as Malaysia, Pakistan, and China through program development and certification development. Other countries that continue to benefit from the project she started include Albania, Peru, Hong Kong, Bulgaria, India, Poland, Kenya, Dominican Republic, Slovenia, Estonia, Mexico, Taiwan, Ecuador, Nepal, Thailand, Egypt, Nigeria, Venezuela, Greece, and Pakistan. She has served as a volunteer herself, and has helped with the Tianjin Program in Pediatric Ophthalmology developed by AAPOS. Cindy’s special gift is her ability to connect people, and she continues to use this gift as Chair of the Volunteer Program, 16 years after its inception. The mission of the IOA is “to promote the science of Orthoptics worldwide, to provide information and support to national bodies and individual orthoptists, and to help maintain and improve standards of education, training and orthoptic practice.” Cindy Pritchard has distinguished herself in her typical quiet and humble manner in each of these categories, nationally and internationally, since she entered the field of Orthoptics in the late 1970s. Other than Gail Stephenson herself, there may be no better ambassador of the ideals and goals of the IOA than Cindy Pritchard. Kyle Arnoldi, CO COMT AACO Representative to the IOA PRISM, page 9 American Orthoptic Council Report The twentieth American Orthoptic Council annual business meeting and certifying oral/practical board exams were held September 16-18, 2016 in Nashville, Tenn. AOC President David Morrison, MD hosted the meeting with more than twenty orthoptists and ophthalmologists attending as officers and representatives. Steve Archer, MD and Tara Bragg were introduced as the two newest AOC members. A dozen guest examiners and helpers were present to help, along with Executive Director Leslie France. Miyazaki who are rotating off the Council this year. Dr. Raab was honored with a commemorative bowl for his 34 years of service to the AOC. He will be greatly missed. Emily Miyazaki was presented with a gift for her services on the board during her term as AACO Vice President. A record number of 29 candidates took the oral/practical examination and all 29 passed. Lindsay Horan, CO PRISM, page 10 Mission Trip to Belize City June 4-9, 2016 This was the second straight year that I had the incredible opportunity to join a pediatric ophthalmology mission in Belize City. Sponsored by the World Pediatric Project and coordinated in partnership with the Belizean nonprofit Belize Council for the Visually Impaired (BCVI), our team was led by Jenna Fedyschyn, Khandice Tillett and Camille Ferguson. I traveled to Belize City to join Courtney Kraus, MD, pediatric ophthalmologist from Johns Hopkins Hospital in Baltimore and Tom Bohannon, MD, anesthesiologist from Cinch Valley Medical Center in Virginia. Our team worked closely with BCVI’s dedicated support staff and volunteers to screen a total of 91 children and provide 28 surgical procedures for conditions including cataracts, strabismus, blocked tear ducts, and ptosis. This mission took place at the BCVI’s National Eye Clinic in Belize City, which was established in 2000 to centralize eye care for the country of Belize. It serves as the referral facility for all patients from clinics around the country needing further medical and surgical treatment for eye conditions. We treated three year-old patient Brenae who had sustained a traumatic sixth nerve palsy after head trauma involving an iron gate several months prior to our arrival. She had undergone surgery to place a plate in her skull and remained in an induced coma for days. Dr. Kraus performed surgery to improve her overall eye range of motion. We hope to evaluate Brenae on future missions and follow her path to recovery. We look forward to returning to the BCVI clinic in January 2017 to do more screenings and surgeries for the Belizean children. Marlo Galli, CO TCOS - Call for Speakers – Montreal, June 2017 The 2017 TCOS Annual Meeting will be held June 15-18, in Montreal, Quebec in conjunction with the Canadian Ophthalmological Society Annual Meeting. Orthoptists registering for the meeting may participate in the workshops on Thursday and may also attend any COS presentations. We are now accepting abstracts for the TCOS Scientific Session on Sunday June 18, 2017 from 8:00 a.m. to 5:30 p.m. If you are interested in presenting, please contact one of the TCOS co-presidents and request an application form called “Offer to Present”. She made improvements and updates to the process, putting in a great deal of time and effort during of period of much change. As the new Membership Chair I am thankful for her efforts. Student membership is important in creating dedicated, loyal, lifelong members.
