Instructions
To apply for a permit, call or visit Visitor Services at (630) 933-7248 or 3S580 Naperville Road in
Wheaton Monday – Friday 8 a.m. – 4 p.m. To get started, you can submit this completed application
in advance via fax at (630) 933-7217 or by mail to Forest Preserve District, P.O. Box 5000, Wheaton, IL
60189. (Add five to seven business days for processing.)
You must submit a certificate of insurance that has your name as the insured; $1,000,000 in liability
coverage for each occurrence; and the Forest Preserve District of DuPage County, 3S580 Naperville
Road, Wheaton, IL 60189 as the certificate holder and as an additional insured with the location and
purpose listed under the description of operations.

Fees Fees are nonrefundable, and permits are nontransferable. Permits are valid January – December only.

Still Portrait Photography Rules and Regulations
For everyone’s safety and enjoyment, you must follow these rules and regulations.
• To take “for profit” photographs in a DuPage County forest preserve, you must have a valid still portrait photography permit in y our
possession.
• Notify Visitor Services of any photo shoot at least three business days in advance. The Forest Preserve District may deny a request if the shoot has the potential to conflict with previously scheduled activities, damage property, affect other forest preserve visitors or use restricted areas. The Forest Preserve District may also restrict the number of individuals in your shoot or your use of models, sets or props.
• Notify Visitor Services before your photo shoot if you’ve changed your original application.
• Provide any additional information or documentation regarding your photo shoot that the Forest Preserve District requests. The Forest Preserve District may deny your permit if you fail to do so.
• Only schedule photo shoots during regular forest preserve hours: one hour after sunrise until one hour after sunset. If you wish to schedule one at another time, you must get permission in advance through Visitor Services. Additionally fees apply.
• Do not block any roads or parking lots or prevent other forest preserve visitors from accessing any areas. Your permit does not grant you exclusive use of any area.
• Do not disrupt any Forest Preserve District employees or ask them to deviate from their normal duties to accommodate your photo shoot.
• Submit requests to shoot at Danada Equestrian Center, Fullersburg Woods Nature Education Center, Kline Creek Farm, Mayslake Peabody Estate, St. James Farm and Willowbrook Wildlife Center through Visitor Services in advance. Additional rental and permit fees apply.
• Only photograph contracted participants; do not photograph other forest preserve visitors or Forest Preserve District employees or
vehicles.
• Do not solicit, advertise or collect fees on Forest Preserve District property.
• Follow all Forest Preserve District of DuPage County ordinances, state statutes and administrative orders. The Forest Preserve District may modify its still portrait photography rules and regulations at any time.
By signing this application, I agree to present my permits for inspection to a Forest Preserve District of DuPage County agent if asked.
I also agree to abide by the above rules and regulations. The information I’ve provided on this application is true. I understand that if I
supply false information or do not comply with the above rules and regulations the Forest Preserve District of DuPage County may fine
me or revoke my permit without refunding my permit fee.

Waiver and Assumption of Risk
I hereby fully waive and release the “Releasee” Forest Preserve District of DuPage County, from any and all claims for personal injury,
monetary loss, property damage, or death that may result from my participation. I hereby voluntarily, at my own risk, agree to the Waiver
and Assumption of Risk in sole consideration of being permitted to use the Forest Preserve District of DuPage County facilities/property
or services offered. Agreement to this Waiver and Assumption of Risk, I fully assume the dangers and risks, and agree to use my best
judgment while engaging in those activities. I further agree to indemnify and hold harmless the Releasee, its employees, agents, officers,
from and against any and all liability incurred as a result of or in any manner related to my participation in the activities or services offered.
