DOCUMENT CHANGES
Thank you for using The Phoenix Trans Resource Guide.
The Resource guide is created by the community for the use of the
community, and is only as good as the information we get from you –
the people this guide reaches out to.
For this January 2013 Edition of the Phoenix Trans Resource Guide, we
have a new layout, new categories, info on hormones, new categories,
an Index, and useful information. Every three to four months, we
attempt to update he guide, and we can only do that with your help,
as this guide is created by the community.
Each of the listings herein was suggested by Trans people who have
used them and had a good experience. This does not, however, mean
that all Trans people will have equally good experiences with the
specific listing, it is only indicative that there is a good chance of a
positive experience. Your individual experience will vary.
Whenever possible, we have provided websites, email addresses, and
telephone numbers for the people, businesses, and organizations in
here.

TO CHANGE GENDER MARKER ON ID OR DRIVERS LICENSE:
A signed statement from a licensed physician is required. The signed
statement must:
State that the customer is irrevocably committed to the
gender-change process (that exact word)
Include the physician's license number
Cannot be dated more than three (3) months prior to the date
of it's submission
Note: It is not necessary for the customer to have completed the
surgical gender-change procedure.

BIRTH CERTIFICATE LAWS IN ARIZONA:
Relevant Arizona Revised Statute
A.R.S. 36-337. Amending birth certificates
A. The state registrar shall amend the birth certificate for a person
born in this state when the state registrar receives any of the
following:
1. Except as provided in subsection D of this section, an adoption
certificate or a court order for adoption required pursuant to section
36-336.
2. A voluntary acknowledgment of paternity pursuant to section 25812.
3. For a person who has undergone a sex change operation or has a
chromosomal count that establishes the sex of the person as
different than in the registered birth certificate, both of the
following:
(a) A written request for an amended birth certificate from the person
or, if the person is a child, from the child's parent or legal guardian.
(b) A written statement by a physician that verifies the sex change
operation or chromosomal count.
4. A court order ordering an amendment to a birth certificate.

2-60112-601. Application; venue; judgment
A. When a person desires to change his name and to adopt another
name, he may file an application in the superior court in the county of
his residence, setting forth reasons for the change of name and the
name he wishes to adopt. The court may enter judgment that the
adopted name of the party be substituted for the original name.
12-60212-602. Notice of application; effect of change on rights and
obligations
A. If upon the filing of the application for change of name the court
deems it proper that notice be given, it may order that notice of the
application be given by publication or by service upon any party
interested.
B. The change of name shall not operate to release the person from
any obligations which he has incurred or is under by the original name,
or defeat or destroy any rights of property or action which he had in
his original name.

Cardiovascular and Other Risks
Estrogens with or without progestins not indicated for
cardiovascular disease or dementia prevention;
Increased risk of stroke and Deep Vein Thrombosis and heart
attack, stroke, and invasive breast CA in postmenopausal
women;
Use lowest effective estrogen dose, shortest duration based on
individual goals and risks
Venous thromboembolic disease
• Estrogen use increases the risk of venous thromboembolic
events (VTE), particularly in patients who are over age 40,
smokers, highly sedentary, obese, and who have underlying
thrombophilic disorders.
• This risk is increased with the additional use of third generation
progestins. (progesterone)
• This risk is decreased with use of the transdermal route of
estradiol administration, which is recommended for patients at
higher risk of VTE.
Lipids
• Oral estrogen use may markedly increase triglycerides in
patients, increasing the risk of pancreatitis and cardiovascular
events.
• Different routes of administration will have different metabolic
effects on levels of HDL cholesterol, LDL cholesterol and
lipoprotein(a).

• In general, clinical evidence suggests that MtF patients with
pre-existing lipid disorders may benefit from the use of
transdermal rather than oral estrogen.
Liver/gallbladder
• Estrogen and cyproterone acetate use may be associated with
transient liver enzyme elevations and, rarely, clinical
hepatotoxicity.
• Estrogen use increases the risk of cholelithiasis (gall stones)
and subsequent cholecystectomy.
POSSIBLE INCREASED RISK:
Prolactinoma
• Estrogen use increases the risk of hyperprolactinemia among
MtF patients in the first year of treatment, but this risk unlikely
thereafter.
• High-dose estrogen use may promote the clinical appearance of
preexisting but clinically unapparent prolactinoma.
Inconclusive or no increased risk: Items in this category include
those that may present risk, but for which the evidence is so
minimal that no clear conclusion can be reached.
Breast cancer
Type 2 diabetes mellitus
• MtF persons who have taken feminizing hormones do
experience breast cancer, but it is unknown how their degree of
risk compares to that of persons born with female genitalia.
• Feminizing hormone therapy, particularly estrogen, may
increase the risk of type 2 diabetes, particularly among patients
with a family history of diabetes or other risk factors for this
disease.
• Longer duration of feminizing hormone exposure (i.e., number
of years taking estrogen preparations), family history of breast
cancer, obesity (BMI >35), and the use of progestins likely
influence the level of risk.
Hypertension
Things Estrogen Will Not Do
• Estrogen use may increase blood pressure, but the effect on
incidence of overt hypertension is unknown.
• Spironolactone reduces blood pressure and is recommended for
at-risk or hypertensive patients desiring feminization.

• Testosterone therapy decreases HDL, but variably affects LDL
and triglycerides.

Acne, androgenic alopecia
Acne and varying degrees of male pattern hair loss (androgenic
alopecia) are common side effects of masculinizing hormone
therapy.
LIKELY INCREASED RISK:
• Supraphysiologic (beyond normal male range) serum levels of
testosterone, often found with extended intramuscular dosing,
may worsen lipid profiles, whereas transdermal administration
appears to be more lipid neutral.
• Patients with underlying polycystic ovarian syndrome or
dyslipidemia may be at increased risk of worsening dyslipidemia
with testosterone therapy.
Liver
Polycythemia
• Transient elevations in liver enzymes may occur with
testosterone therapy.
• Masculinizing hormone therapy involving testosterone or other
androgenic steroids increases the risk of polycythemia
(hematocrit > 50%), particularly in patients with other risk
factors.
• Hepatic dysfunction and malignancies have been noted with
oral methyltestosterone. However, methyltestosterone is no
longer available in most countries and should no longer be used.
• Transdermal administration and adaptation of dosage may
reduce this risk
Weight gain/visceral fat
• Masculinizing hormone therapy can result in modest weight
gain, with an increase in visceral fat.
Psychiatric
Masculinizing therapy involving testosterone or other
androgenic steroids may increase the risk of hypomanic, manic,
or psychotic symptoms in patients with underlying psychiatric
disorders that include such symptoms. This adverse event
appears to be associated with higher doses or supraphysiologic
blood levels of testosterone

INCONCLUSIVE OR NO INCREASED RISK:
Items in this category include those that may present risk, but for
which the evidence is so minimal that no clear conclusion can be
reached.
Osteoporosis
• Testosterone therapy maintains or increases bone mineral
density among FtM patients prior to oophorectomy, at least in
the first three years of treatment.
Type 2 diabetes mellitus
• Testosterone therapy does not appear to increase the risk of
type 2 diabetes among FtM patients overall.
• Testosterone therapy may further increase the risk of type 2
diabetes in patients with other risk factors, such as significant
weight gain, family history, and polycystic ovarian syndrome.
There are no data that suggest or show an increase in risk in
those with risk factors for dyslipidemia.
Breast cancer
• There is an increased risk of bone density loss after
oophorectomy, particularly if testosterone therapy is interrupted
or insufficient. This includes patients utilizing solely oral
testosterone.
• Testosterone therapy in FtM patients does not increase the risk
of breast cancer.
Cardiovascular
• Testosterone therapy in FtM patients does not increase the risk
of cervical cancer, although it may increase the risk of minimally
abnormal Pap smears due to atrophic changes.
• Masculinizing hormone therapy at normal physiologic doses
does not appear to increase the risk of cardiovascular events
among healthy patients.
• Masculinizing hormone therapy may increase the risk of
cardiovascular disease in patients with underlying risks factors.
Hypertension
• Masculinizing hormone therapy at normal physiologic doses
may increase blood pressure but does not appear to increase the
risk of hypertension.
• Patients with risk factors for hypertension, such as weight gain,
family history, or polycystic ovarian syndrome, may be at
increased risk.
Cervical cancer
Ovarian cancer
• Analogous to persons born with female genitalia with elevated
androgen levels, testosterone therapy in FtM patients may
increase the risk of ovarian cancer, although evidence is limited.
Endometrial (uterine) cancer
• Testosterone therapy in FtM patients may increase the risk of
endometrial cancer, although evidence is limited.

CRITERIA FOR FEMINIZING/MASCULINIZING
HORMONE THERAPY
(ONE REFERRAL OR CHART DOCUMENTATION OF
PSYCHOSOCIAL ASSESSMENT)
1) Persistent, well-documented gender dysphoria;
2) Capacity to make a fully informed decision and to consent
for treatment;
3) Age of majority in a given country (if younger, follow the
SOC for children and adolescents);
4) If significant medical or mental concerns are present,
they must be reasonably well-controlled.
CRITERIA FOR BREAST/CHEST SURGERY
(ONE REFERRAL)
Mastectomy and creation of a male chest in FtM
patients:
1) Persistent, well-documented gender dysphoria;
2) Capacity to make a fully informed decision and to consent
for treatment;
3) Age of majority in a given country (if younger, follow the
SOC for children and adolescents);
4) If significant medical or mental health concerns are
present, they must be reasonably well controlled.
Hormone therapy is not a pre-requisite.
Breast augmentation (implants/lipofilling) in MtF
patients:
1) Persistent, well-documented gender dysphoria;
2) Capacity to make a fully informed decision and to consent
for treatment;
3) Age of majority in a given country (if younger, follow the
SOC for children and adolescents);
4) If significant medical or mental health concerns are
present, they must be reasonably well controlled.
Although not an explicit criterion, it is recommended that MtF
patients undergo feminizing hormone therapy (minimum 12
months) prior to breast augmentation surgery. The purpose is to
maximize breast growth in order to obtain better surgical
(aesthetic) results.
CRITERIA FOR GENITAL SURGERY (TWO
REFERRALS)
Hysterectomy and ovariectomy in FtM patients and
orchiectomy in MtF patients:
1) Persistent, well documented gender dysphoria;
2) Capacity to make a fully informed decision and to consent
for treatment;
3) Age of majority in a given country;
4) If significant medical or mental health concerns are
present, they must be well controlled;
5) 12 continuous months of hormone therapy as appropriate
to the patient’s gender goals (unless the patient has a
medical contraindication or is otherwise unable or
unwilling to take hormones).

The aim of hormone therapy prior to gonadectomy is primarily
to introduce a period of reversible estrogen or testosterone
suppression, before a patient undergoes irreversible surgical
intervention.
These criteria do not apply to patients who are having these
surgical procedures for medical indications other than gender
dysphoria.
Metoidioplasty or phalloplasty in FtM patients and
vaginoplasty in MtF patients:
1) Persistent, well documented gender dysphoria;
2) Capacity to make a fully informed decision and to consent
for treatment;
3) Age of majority in a given country;
4) If significant medical or mental health concerns are
present, they must be well controlled;
5) 12 continuous months of hormone therapy as appropriate
to the patient’s gender goals (unless the patient has a
medical contraindication or is otherwise unable or
unwilling to take hormones);
6) 12 continuous months of living in a gender role that is
congruent with their gender identity.
Although not an explicit criterion, it is recommended that these
patients also have regular visits with a mental health or other
medical professional.
The criterion noted above for some types of genital surgeries –
i.e., that patients engage in 12 continuous months of living in a
gender role that is congruent with their gender identity – is
based on expert clinical consensus that this experience provides
ample opportunity for patients to experience and socially adjust
in their desired gender role, before undergoing irreversible
surgery.
TRANS RELATED FILMS
Different for Girls
Karl Foyle (Steven Mackintosh) and Paul Prentice (Rupert
Graves) were boyhood friends back in the 1970s, but when they
run into each other in present-day London, they learn that a lot
has changed. For starters, Karl has become Kim and has no
desire to go back to her past. As for Paul, he's just an aging punk

Southern Comfort
This moving documentary chronicles the last year in the life of
Robert Eads, a female-to-male transsexual dying of ovarian
cancer. We're introduced to several prominent figures in Robert's
life -- most importantly, his life partner, Lola Cola. Lola is a
transsexual who's become Robert's life partner and caretaker.
The two prepare to lead a panel at the annual Southern Comfort
conference, a yearly event created for transgendered individuals.
Trans: The Movie
"TRANS" is an up-close and very personal journey into the
transgender world through the memorable stories and the
unusual lives of a remarkable cast of characters.

Trained in the Ways of Men
Documentary filmmaker Shelly Prevost examines the brutal
2002 murder of Gwen Araujo, a transgender teen who was killed
by four men enraged to discover that she was actually a biological
male, in this nuanced look at gender identity and aggression. The
case became a rallying point for the transgender community
when the men, who had all been intimate with Araujo, claimed
that her deception justified their actions.
Soldier’s Girl
This powerful drama tells the true story of a young soldier, Pfc.
Barry Winchell (Troy Garity), who meets and falls for a beautiful
transgendered nightclub performer (Lee Pace) while on a
drunken outing with fellow soldiers. Winchell's relationship is
frowned upon by the other soldiers -- and eventually leads to
tragedy at the hands of his homophobic roommate.
Two Spirits
At age 16, transgender American Indian Fred Martinez became
one of the youngest hate crime victims in modern history when
he was beaten to death by a man who later bragged about the
brutal crime. This documentary tells Martinez's story. In the
process, it illuminates the Navajo two-spirit tradition -- which
honors individuals who embody both the masculine and the
feminine -- and demonstrates how ancient values can inform
(and improve) modern life.
Still Black: A portrait of a Black Trans Man
Frequently marginalized within a community that already
struggles against marginalization -- that is, the gay, lesbian and
bisexual community -- African American transgender males
make their voices heard in this film from Kortney Ryan Ziegler.
Six articulate black transmen -- who are also artists, students,
husbands, fathers, lawyers and teachers -- speak candidly about
race, gender, body image and status.
Gun Hill Road
After serving a three-year stint in prison, tough-guy Enrique
(Esai Morales) returns to his family in the Bronx to find that his
wife (Judy Reyes) is a virtual stranger to him and, worse, his son
(Harmony Santana) is in the middle of a sexual identity crisis.
Now Enrique must find a way to accept his altered life or risk
losing control and ending up back in jail.
A Girl Like Me: the Gwen Araujo Story
Based on a true story. As a young boy, Eddie Araujo always felt
different somehow. She started putting on her mom's makeup
and wearing her mom's clothes, which her mom found odd. By
the time she entered her teen years, Eddie could no longer hide
the fact that she was a lot different, that she was meant to be
female and not male. When she finally accepted it and with her
mom's eventual support, Eddie changed her name to Gwen and
started to live life as female until a tragic night changed
everything.
