GAC #16-07 – Meningitis Immunization Policy

Policy
Purpose:
College students, especially first-year college students living in residence halls, are at an slightly
increased risk for meningococcal disease compared with other persons of the same age. This
policy is in compliance with Senate Bill No.umber 716748, 2014, Section 174.335, RSMo, effective
July 1, 2015 and revised 2016. and the Centers for Disease Control
& Prevention recommendations and will take effect beginning fall 2015.
Definitions:

Policy:
1. Upon matriculation at Missouri Western State University, aAll students who plan to
reside in Missouri Western State University on-campus housing are required to must
have received the meningococcal conjugate vaccine unless a signed statement of medical
or religious exemption is on file with the Esry Student Health Center.
a. A student shall be exempted from the immunization requirement upon signed
certification by a doctor licensed under chapter 334, indicating that either the
immunization would seriously endanger the student’s health or life or the
student has physician’s documentation of disease or laboratory evidence of
immunity to the disease.
b. A student shall be exempted from the immunization requirement if the student
s/he objects in writing to the institution’s administration that immunization
violates theirhis or her religious beliefs.
2.
The Esry Student Health Center shall maintain records on the meningococcal
vaccination status of every student residing in on-campus housing at the university.
3.
The meningococcal vaccine may be provided by a student’s personal physician, or it may
be administered in the Esry Student Health Center. The cost of the vaccination is the student’s
responsibility.
b. Missouri Western considers a vaccine received within 5 years before
matriculation as valid.
i. If this vaccine was administered before the student’s 16th birthday, a
booster dose is recommended before residing on campus.
c. Students must be revaccinated every five years with the meningococcal vaccine
for it to be effective. It is each individual student’s responsibility to ensure their
vaccinations are up-to-date.
a. For students who receive the first dose at age 11- 12 years, a one-time booster
dose must be administered, generally at age 16. Students who receive their first
dose of meningococcal vaccine at or after age 16 years do not need a booster
dose.

3. All immunization information will be collected by oInformation related to a student’s
health record can be delivered in person tor may be sent to Residence Life, & / or the Esry
Student Health Center, faxed to the Health Center (816) 271-4498 or sent via mail to the
Esry Student Health Center, Missouri Western State University, 4525 Downs, Drive, St.
Joseph, MO 64507.. staff, who will submit them to the Esry Student Health Center
personnel for review.

4. No student shall be allowed to move intoStudents are expected to provide
either proof of the immunization or have filed a waiver prior to moving in to on-campus
housing until proof of the immunization has been received or an exemption is on file..
SFailure of students who fail to complete the meningitis immunization or
exemption will result in a have a notification sent to the student via official MWSU email
accounts, and a hold placed on their enrollment for the subsequent semester until the
immunization has been completed or a waiver is on file. The student will not be permitted
to maintain enrollment in the subsequent semester until this is completed.student will not

residential student, all on-campus housing charges will apply, even if the student is unable
to move in due to his or her non-compliance.
5. In the case of a confirmed meningitis case, the University will follow the contagious /
communicable disease policy.

The Esry Student Health Center is located on the second floor of Blum Union in Room 203. A
nurse practitioner and nursing staff are available to provide assistance with health care needs
through diagnosis and treatment, immunizations, health counseling, and specialist referral
services. The Health Center also offers a variety of educational pamphlets and information.
Health records are maintained with STRICT CONFIDENTIALITY.
All new first-time students are required to complete the Tuberculosis Risk Assessment and
submit it to Esry Student Health Center.
Students desiring campus residential housing are required to provide a completed
Health History Form, a Tuberculosis Risk Assessment and the Immunization
Documentation Form signed by their health care provider before being allowed to
move into the residence halls. The policy and forms may be found at
www.missouriwestern.edu/HealthServ/. International students that do not plan to live
on campus should complete the Tuberculosis Risk Assessment and the Health History
Form and submit them to the Esry Student Health Center, 4525 Downs Drive – Blum
203, St. Joseph, MO 64507.

MWSU students residing in the residence halls are required to provide a completed Health
History Form, a Tuberculosis Screening Questionnaire and the Immunization Documentation
Form or other official immunization records such as Health Department, physician office,
hospital or school. These documents must be turned into the Esry Student Health Center.
These forms are to be completed before being allowed to move into the residence halls.
Required Recommended Immunizations:


Measles/Mumps, Rubella (MMR) – 2 doses, the first given at age one year or later;
the second at least 4 weeks after the first for students born in 1957 or later
Meningococcal disease (Menactra within last 5 years)

Tetanus-Diphtheria-acelluar pertussis (Tdap) within the last 10 years

Tuberculosis Screening Questionnaire (PPD) – All new students are to
complete the Tuberculosis Screening Questionnaire and return to the Esry
Student Health Center. International students will receive tuberculosis testing
upon arrival at MWSU. Frequently Asked Questions (FAQ) regarding
Tuberculosis Screening Questionnaire may be found under the Frequently
Asked Questions tab.




Students who have ever had a positive tuberculin skin test, must submit a physician signed chest
x-ay report with their health and immunizations records, prior to move-in to confirm the
absence of active disease.
Students who object to immunizations because of cultural, religious, or medical reasons must
provide a completed exemption waiver request. Please visit our website
at www.missouriwestern.edu/HealthServ/ for a copy of the MWSU waiver.
All documentation must should be submitted to Esry Student Health Center, 4525 Downs
Drive-Blum 203, St. Joseph, MO 64507 prior to moving in; or fax to (816) 271-4498. Failure to
provide this information may delay your check-in.
