*Note to supervisors – You must read the following statements prior to signing
the driving experience logsheet above. By signing as a supervising driver in this logbook
(above in the driving experience table) you acknowledge and agree that you are also
making the following declarations:
1. You declare that you are the supervisor of the learner driver on the relevant dates and
that as the supervising driver you were at all relevant times, the holder of at least a
valid Class C or CA open driver licence or a licence granted outside of Queensland that
corresponds to a class C open driver licence, and that you held that licence for at least
1 year;
2.You declare that the information which you have provided in this logbook is complete, true and correct in every detail.
3.You understand that Queensland Transport will consider the information which you have provided in this logbook to determine if it is accurate, complete and otherwise satisfies the requirement of the Transport Operations (Road Use Management Driver Licensing) Regulation 1999). To enable Queensland Transport to do so, you give Queensland Transport permission to access your driver licence record to confirm the information provided.

Privacy Statement
Queensland Transport provides this form under the Transport Operations (Road Use Management-Driver
Licensing) Regulation 1999 to determine if the learner driver has completed driving experience under the
supervision of an appropriately licensed supervisor.
The information collected in this form is accessible by authorised departmental officers and some of this
information may be disclosed to the Queensland Police Service and interstate driver licensing authorities.
Queensland Transport will not disclose your personal details to any other third parties without your
consent unless required by law.
