GROUP BENEFITS OUTLINE
ELIGIBILITY
Health
Insurance
Employees who are regular (not temporary) employees and regularly scheduled to work 20 hours per week or
more are considered by Catholic Charities to be “benefits eligible” employees. Eligible employees may enroll in
the benefit plans within the first thirty (30) days of employment, during the open enrollment period, or if an
employee or dependent experiences a “Change of Family Status”.
HealthPartners is our health insurance carrier.
There are two deductible plans offered and four employee coverage tiers. Search www.healthpartners.com
under “Find care in your plan network/Group Medical Networks” under “2016 Distinctions II” to search for
clinics and doctors in the network when you seek medical service.

Delta Dental is our dental insurance carrier. All employees pay the same cost. For Delta Dental contracted
providers, search www.deltadentalmn.org .PPO providers offer a higher benefit within our plan. The
Orthodontic discount plan provides a discount on new services at specific network orthodontic providers.

EyeMed is our vision insurance carrier. Eye Med’s SELECT network has a wide range of major optical service
retailers that provide vision products at a reduced rate. Per paycheck premiums:

Catholic Charities has a flexible spending program for reimbursement of Medical, Dependent care or
Transportation expenses. Re-enrollment is required each calendar year. Up to $500 of unspent Medical
account expenses may roll over into the next year.
Alliance Benefits Group is our Flexible Spending Account Administrator.
Catholic Charities’ Well-Being program, is run through HealthPartners’ wellness website and offers many
trainings, health assessments, and health programs. Employees on the health plan can earn a significant
premium discount on their Health insurance for completing wellbeing programs each year.

Benefits
In the event of an employee’s death, this employer paid plan provides a settlement to your beneficiary equal
to two (2) times your annual salary up to a maximum of $250,000. An equal amount of accidental death and
dismemberment insurance is provided as well.
Benefits eligible employees may apply for additional basic term life insurance at their own cost. Guaranteed
issue amounts for new hires are $150,000 coverage for employees, $30,000 for spouses, $10,000 for children.
Employee coverage is sold in units of $10,000, spouse coverage in units of $5000, child coverage is sold in units
of $2500. At least one unit of employee coverage must be purchased if buying spouse or child insurance.
Catholic Charities covers all benefits eligible employees in Short and Long Term Disability insurance policies.
Short Term Disability compensates 60% of wages to the disabled employee for a term of 75 days after a 14 day
waiting period.
Long Term Disability compensates 60% of wages to the disabled employee following the 90 day short term
disability period.
401(k) Retirement Plan (employee & employer contributions)
Regular benefits-eligible employees over the age of 21 may enroll in Catholic Charities 401(k) savings plan and
defer a percentage of their own salary immediately after hire or a benefit status change. Once an employee is
eligible, Catholic Charities will match a half of a percent per employee deferral to a maximum match
percentage of 3%. To get the maximum match, an employee must defer at least 6%.
For eligible employees, an automatic enrollment at a 2% deferral may be triggered if an employee has not
already previously elected or declined enrollment.
403(b) Retirement Plan (employee contributions only)
Catholic Charities 403(b) plan can be entered any time. The payroll deductions are taken pre-tax reducing an
employee’s taxable income and invested with one of three vendors available through the plan. There is no
employer match in the 403(b) plan.
Catholic Charities’ provides an Employee Assistance Program (EAP) for its employees.
HealthPartners EAP offers many resources and referrals. This service is 24/7, confidential, and can help with:
relationship issues, legal issues, financial issues, mental health/substance abuse issues, and grief issues. They
also have online learning and training seminars for professional development and resources/referrals for
daycare, elder care, and pet care. To access the EAP call 1-800-626-7944 or go online at www.hpeap.com

Paid Time Off (PTO) is a combination of vacation, personal and sick time. PTO is accrued each paycheck at a
rate per hour worked. It is available to use immediately once it is added to your balance. A full-time employee
earns up to 20 days of PTO by the end of his/her first year. PTO Accrual is prorated for part-time employees.
The amounts earned increase throughout an employee’s career at Catholic Charities until, employees who
have worked 15 or more years earn up to 30 days per year. Carryover of PTO is allowed and is equivalent to
the amount earned by an employee within a calendar year. Please refer to the Catholic Charities Benefits Guide
for the full scale of accrual rates per year worked.
Banked hours of sick leave are available for employees who experience an extended illness. Employees who
are eligible for PTO also accrue sick leave bank hours at the rate of 1.231 hours of sick leave per pay period for
full-time employees, and prorated for part-time employees. Employees are allowed to accrue a maximum of
480 hours of sick leave. Details available in the employee handbook
Eleven (11) paid holidays per year: New Year’s Day, Martin Luther King Jr. Day, President’s Day, Good Friday,
Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving, Christmas Eve, and
Christmas Day.
