Disponible en français. www.healthcareathome.ca/ northwest Our School Health Support Services How to Access Our Services The CCAC’s School Health Support Services provide health care services to children who otherwise would be unable to attend school, or whose education would be disrupted by having to leave the school setting for needed therapy. We can arrange the following services for children attending public, private or home schools: In order to obtain the CCAC’s school services, a referral must be made by your child’s school. If you or your child’s teacher identifies a need for in-school services, the school can initiate a referral to the CCAC. You will then be contacted by a CCAC Community Care Coordinator. If your child is home schooled, you can contact the CCAC directly. • Speech-language therapy • Occupational therapy • Physiotherapy • Nursing • Nutritional counselling • Personal support (home and private school settings) In addition, our Community Care Coordinators can link you to other support services available in the community. Our goals are to support your child’s independence and maximize his or her ability to participate in school activities and instruction. Who Can Access School Health Support Services? The CCAC’s School Health Support Services are funded by the Ontario Ministry of Health and Long-Term Care. Services are available to those children who: • have a valid Ontario Health Card; • attend a public or private school or are home schooled; The CCAC’s Community Care Coordinator will: • determine what health services your child requires; • assess your child’s eligibility for CCAC services; • develop a service plan with you for your child; • arrange for health care services in the school setting; • connect you and your child to other resources and services in your community; • have a health condition that can be treated • work closely with health professionals involved • require one of the CCAC services in order to • communicate with you and the school about effectively in the school setting; attend school, participate in school routines or receive instruction at home; and • are willing and able to participate in the required care, along with his/her family.
