Wallet-size health care proxy card available from E.J. Noble Hospital
A statewide effort has been
launched to help New Yorkers gain
more control over their health care
decisions. EJ. Noble Hospital has
joined the statewide initiative to provide wallet-size "health care proxy"
cards to all New Yorkers. The cards
were developed through the joint
efforts of the Healthcare Association
of New Yoric State and the New York
Chapter of Society of Patient Representatives.
"Few New Yorkers realize that
if they become unconscious or oth-
erwise unable to make health care
decisions for themselves, family
members or loved ones have no legal authority to step in and make
those decisions unless the person has
a health care proxy," said Charles
Conole, Administrator of E.J. Noble
Hospital. "We hope this will encourage patients and community members to exercise theirrightto self determination by taking the time to fill
out a health care proxy. New Yorkers can ensure that their wishes about
their health care treatment are re-
spected by carrying this wallet-size
card with them all times," Mr,
Conole said.
By State law, a health care
proxy enable competent adults to
appoint someone they trust to decide
about treatment on their behalf if
ihey are unable to decide for themselves. The appointed agent can consent orrefuseany treatment, service,
or procedure to diagnose or treat the
patient's physical or mental condition. Health care providers such as
hospital and doctors must comply
with the agent's decisions.
The wallet-sized proxy cards
are available at no cost to all New
Yorkers. To obtain copies, please

After school
activities are fun!
The After School Recreation
Program in Gouverneur is accepting
registrations for the third session. The
program is designed for children in
grades K-6 and takes place from 35p.m. at the Community Center on
Tuesday and Thursday afternoons.
Program participants have the
opportunity to become actively involved in many alternative games and
an/craft projects. Recently, children
attending the program have designed
canvas handbags, decorated jewelry/
treasure boxes, painted pumpkins and
created gliuer art. Playing cage-ball,
fly-swatter badminton, dancing contests and roller-blading have been
popular activities as well.
The recreation staff provides
positive interaction and encourages
children to attend the after school program. Please do not hesitate to contact Recreation Director Casey

her greenhouse in Martha's Vineyard
is sold to restaurants, resulting in a

Several interested parties in the
area want to build a similar greenhouse at the Canton College farm.
Other parties wishing to participate
are Clarkson, Canton, BOCES, Associated Colleges and schools.
She will be at the Southwest
Technical School on October 25, at
9a.m. Anyone interested can go to the
BOCES school on that day or call the
school for more information.
