Adoption Fee: Thank you for adopting from Lucky You! Animal Rescue. Please READ and INITIAL all statements. Please ask a Lucky You! representative if you do not understand a statement. In adopting this pet I certify that I understand and agree to ALL of the following: 1. _____ I understand that the animal I am adopting is a rescue animal and may have been previously exposed to, and/or is susceptible in the future, to illness or disease. These issues may not be readily apparent to foster parent or adoption counselor. 2. _____I understand that the animal I am adopting may have a medical or behavioral condition that is not readily apparent and that Lucky You! Animal Rescue cannot guarantee the medical or behavioral health of this animal in your home. 3. _____ I understand that upon taking possession of this animal, the ownership of this animal transfers to me. From that point forward, Lucky You! Animal Rescue is released from any monetary obligation or liability to the health of this animal. This includes any pre­existing conditions (all known conditions listed in item #13). 4. ______ If I am currently a pet owner, I understand that I am assuming all risk and expense related to any exposure to illness or disease that may arise as a result of this new family member and agree to have taken proper precautions with my previously owned pets. 5. ______ I understand that I have 72 hours (3 days) from the time of adoption to contact a representative of Lucky You! Animal Rescue by phone or email, if there are medical or behavioral concerns that need to be addressed. 6. ______I agree to provide all basic and emergent needs of this animal. Including, but not limited to, food, water, shelter, grooming, and routine and emergency vet care and I am financially capable of doing so. 7. ______ I consent to maintain a level of privacy between adopter and rescue and will contact Lucky You! Animal Rescue directly if issues arise. 8. ______ I agree to comply with all applicable ordinances and laws pertaining to owning this animal, including maintaining a dog license when required. 9. _______ I agree to register the microchip for my new pet within two weeks of adoption. 10. ______ I understand that if Lucky You! Animal Rescue has reason to believe the animal is being abused, neglected, or mistreated we will contact you to conduct a home visit. Upon refusal of a home visit, we will notify the PSPCA cruelty department (or other proper authority). 11. ______ In the event that I can no longer keep this animal, I will contact a representative of the rescue and return the animal to Lucky You! Animal Rescue, or inform the rescue if a new home is found. 12. _______Upon adopting my new best friend, I agree to hold harmless Lucky You!
