TERMS AND CONDITIONS
OF THE SPECIAL ST. VALENTINE’S DAY OFFER AT JATOMI FITNESS
(“T&C”)
The Special Offer is provided by: Pure Health and Fitness Sp. z o.o., a company having its
registered office in Warsaw (00-838); ul. Prosta 69, NIP [tax identification no.]: 7010098429;
REGON [company statistical no.]: 141130420; entered in the Register of Entrepreneurs at the
District Court for the Capital City of Warsaw, 12th Commercial Department of the National
Court Register, under No. KRS: 0000289302 (“Organizer”), owner of the Jatomi Fitness fitness
center chain (“Fitness Centers”).
I. VALIDITY PERIOD
The Special Offer, on the terms referred to herein ( “Special Offer”), shall become effective on
Sunday, 15 February 2017 at 00:01 and shall end on Wednesday, 28 February 2017, at the end
of the day, i.e. at 23:59.
II. SUBJECT MATTER
1. PARTICIPATION TERMS – FOR EXISTING FITNESS CENTER MEMBERS

Members who have signed an effective agreement with the Organizer for the
use of the Fitness Centers (“Membership Agreement”), where it is still active as at
the start date of the Special Offer (“Existing Members”), can take advantage of
Special Offer as described in clause 1.6 below, if, during its validity period, they visit
the Fitness Center accompanied by a person who is not an Existing Member,
provided that the accompanying person signs a Membership Agreement on the
terms set out in clause 1.2 below (“New Member”).
1.2 In order for the Existing Member to be able to use Special Offer the Membership
Agreement signed by the New Member must be complete, signed during the validity
period of the Special Offer and not later than on the day when the Existing Member
uses the Special Offer. It must also contain a written acknowledgement of the rules
and regulations effective throughout the Fitness Center chain, as well as an
acknowledgment of these T&C. Moreover, the New Member’s Membership
Agreement must include the information necessary to enable the membership fees
to be effectively charged to the New Member’s payment card of the existing
member, according to the point 1.3.
1.3 The condition of using this promotion is the expression of agreement of an Existing
Member and providing in the membership agreement of New Member the
information necessary to enable the membership fees of the New Member to be
effectively charged from the payment card of Existing Member, which was provided
in the membership agreement of the Existing Member. The information about the
payment card in the membership agreement of Existing Member and New Member
must be the same. The membership fees of Existing Member and membership fees
of the New Member will be charged from the payment card of the Existing Member
for the whole time of the Membership Agreement of the Existing Member and the
whole time of the Membership Agreement of the Ne Member. Agreement reffered
in first sentence of this point Existing Member express by signing this terms &
conditions.
1.4 The New Member shall become a member on the same terms as regards the fee and
time arrangements as the Existing Member, reduced by 20%.
1.5 The pricing terms of the membership of the Existing Member shall not change.
1.6 The use of Special Offer by the Existing Member consists in an extension of the
duration of their membership by the next minimum membership period.
III. GENERAL PROVISIONS
1. Special Offer cannot be combined with any other promotional offers concerning Fitness
Center membership, including, in particular the “Recommend a Healthy Lifestyle”
(“Polecaj zdrowy styl życia”) program.
2. Acceptance of these T&C is a necessary precondition for taking advantage of the Special
Offer.
3. The Membership Agreement and the Fitness Center rules and regulations shall apply to
any cases not addressed herein.
4. The offer shall not apply to any assignment of membership as described in the “Terms
and Conditions of Assignment of Membership” available on the Organizer’s website:
www.jatomifitness.pl.
5. A full list of the Fitness Centers supporting the Special Offer can be found on the
Organizer’s website.

7. These Terms and Conditions shall remain effective from 15 February 2017 to 28
February 2017.
