Directions:
One application form needs to be submitted for each act. Select a contact person for
your group and have them fill out the application form. One permission slip for each
member of the act must be completed and attached to this application form before
you turn it in.
A COPY OF YOUR SONG LYRICS MUST ALSO BE ATTACHED TO THIS APPLICATION
Incomplete forms will not be accepted.
