Dear Parents:
On Friday, May 5, 2017 Lakeside Middle School student body will be sponsoring a “Bungle in the Jungle” Dance for Lakeside students
ONLY. If your student purchased an Annual Dance Pass, no individual ticket purchase is necessary! Individual
tickets for this dance WILL ONLY BE SOLD AT THE DOOR. Please refer to the following information and behavior expectations:
LOCATION:
Lakeside Middle School in the Field House and commons area.
TIME:
The dance will take place from 6:00-8:00 PM. Parents or guardians are expected to pick up students within fifteen
(15) minutes (8:15 PM) after the end of the dance. Once inside the dance area, no one will be allowed to leave and
re-enter.
PRICE:
ID cards with dance passes that were pre-purchased MUST be presented at the door. Individual tickets are $8.00
each and WILL ONLY BE SOLD AT THE DOOR. NO REFUNDS.
BEHAVIOR EXPECTATIONS:
1. All students must present their ID cards at the door of the dance. Students will not be admitted after 6:30pm!
2. Once students arrive at the dance, they must remain until 8:00pm, unless they are picked up PERSONALLY by a parent.
3. Students may not use and/or purchase another student’s dance ticket.
4. Students who are absent the day of the dance will not be allowed to attend.
5. Refreshments are on sale in the patio area. All food/drink must stay outside in the patio area.
6. Lakeside’s STUDENT BEHAVIOR expectations are still in effect; this includes DRESS CODE & NO GUM.
7. Inappropriate behavior at the dance (before, during, or after) could result in the loss of any or all dance
privileges.
PURCHASING TICKETS:
Parents or guardian must sign the permission slip printed below. Please provide $8.00 (CASH
ONLY) and this slip, completely filled out. Students must submit this permission slip and
payment at the door, in order to enter this dance. If you have any
