Mission of the Hopkins Community Coalition: ONE VOICE for Reducing Youth Chemical Use
We will live in a community where more of our youth will choose not to use alcohol, tobacco, marijuana,
and other drugs; and where the community speaks out with one voice regarding illegal chemical use.
Approved funding of this grant request will be determined by how well it fits into the Hopkins Community
Coalition mission and goal. Range for grant awards: Up to $1,000.00

Request for Funds Application Form 2008-2009
The goal of Hopkins Community Coalition: ONE VOICE for Reducing Youth Chemical Use is
to encourage a change in adult behavior that guides and supports the decisions of youth to
resist using alcohol, tobacco, and other drugs. Strategies include:
√ Parents become less accepting of alcohol, tobacco, and other drug use amongst their
children.
√ Parents gain skills around setting firm boundaries and consequences for illegal alcohol,
tobacco, and other drug use.
√ Adults (School staff, coaches, youth workers, employers, etc.) in the community become less
accepting of alcohol, tobacco, and other drug use amongst youth.
√ Adults (School staff, coaches, youth workers, employers, etc.) in the community gain skills
around setting firm boundaries and consequences for illegal alcohol, tobacco, and other
drug use.
√ Businesses support measures that prevent illegal alcohol, tobacco, and other drug use
amongst youth.
√ Youth feel empowered by adults and gain knowledge and/or skills to resist use of alcohol,
tobacco, and other drugs.

Include an overall budget for the project including all sources of revenues and
expenses. Funding requests for indirect services such as food and beverages will be reviewed
for compliance with Federal grant administration guidelines.

Evaluation: As part of your award you will be required to complete a project evaluation form
and give a brief presentation explaining your project at a meeting of the Hopkins Community
Coalition: ONE VOICE for Reducing Youth Chemical Use, including how grant funds were used.
