His leadership and visions made our profession as strong and highly recognized as any other profession in Government. Indiana State Coroners Association looked back at the history of David’s dedication, love, leadership and affiliation with the Association. The board of the Indiana State Coroner’s Association reflected on the growth, positive changes and guidance that David brought to this fraternal organization. David was a very valuable part of making the Indiana State Coroners Association and the Coroners Training Board into what they are today by his active involvement and encouragement, and forethought of the future and his drive for education and professionalism. Based on all of this the board unanimously voted to rename our annual conference the Dr. David Avolt Memorial Training Conference. This is in recognition of a man who truly dedicated his life to the betterment of his fellow coroners. He was elected Tippecanoe County Coroner in 1984 after serving as a deputy under Dr. William Sholty. He served in the coroner’s office since then. As Coroner he was instrumental in raising the professionalism of the office, helping to write the legislation in 1993 that required all Coroners and Deputy Coroners to become trained, state certified and requiring continuing education. Dr. Avolt was the first Veterinarian to be elected as a Fellow of the American Academy of Forensics Sciences in 1979. He had served as President of the Coroners Association in 1992 and a longtime board member. He had also served as the Executive Director of the Coroners Training Board from 19941998. “Doc” is considered by many, as the Dean of Indiana Coroners. His willingness to counsel, assist or advise those seeking his wisdom and experience has been a tremendous asset over the years, not only to coroners but to the families being served as well. His proudest accomplishments are the students and colleagues he has mentored through the years that achieved the great heights “Dr. Dave” knew they were capable of achieving. This means that you should dress appropriately to represent your office in the most professional attire. We will not allow shorts, sweats of any kind, ripped or torn jeans (even if manufactured that way), printed t-shirts or the like. Appropriate dress would be slacks or khaki’s, collared shirts or blous-es (either polo type or button down) or other dress attire. Suits and ties, sport coats or dresses are not required but are considered appropriate. We are a professional organization and we will be having guests attending the conference who are also professionals in their field. Your cooperation will be greatly appreciated in adhering to this dress code and anyone who is not dressed appropriately will be asked to either change or leave the conference. General Session Attendance The conference is for in-service training for the elected coroners and deputy coroners, therefore the ISCA must enforce that only coroners and their deputies that hold a current association ID badge are present in the general sessions. Spouses are welcome to attend the banquet or any other event held outside of the actual general session. General Business Meeting The general business meeting will be held on Thursday, June 2nd from 4:15 pm – 4:45 pm. This meeting is open to coroners who have paid their membership for 2016. Each county coroner with a paid membership shall have one vote in the general business meeting. Panel Discussion A panel discussion will be held utilizing 4 coroners from around the state from various size agencies. The purpose of this panel discussion is to share information, discuss issues faced by coroners, to share problem solving techniques and to answer those questions of how does someone else handle this situation. Questions should be about real issues, such as handling problem with IDRS, problems with law enforcement agencies, problems with prosecutors, problems with funeral homes or hospitals, what if the deceased is from out of state or country and FARS. This list is not inclusive as to topics but is used as a general guideline to establish the type of questions sought by the panel. All questions should be submitted to Lisa Barker @ [email protected] by May 15th so the panel will have time to review all questions and time will be allowed at the end for open questions from the floor. Murtaugh serves as the Executive Director of the Indiana Criminal Justice Institute. David was appointed by In-diana’s 50th Governor, Michael R. Pence in July, 2015. David has been a lifelong public servant, serving many of his years in public safety. David served eight years as United States Marshal with the United States Marshals Services, after being nominated by President George W. Bush in December 2001 and confirmed by the U.S. Senate in February 2002. He also served as Acting Deputy Director for State, Local, and Tribal Affairs in the White House Office of National Drug Control Policy during his tenure as U.S. Marshal. Murtaugh began his career at the Tippecanoe County Sheriff’s Office, where he moved up the ranks, serving as Patrolman, Sergeant, Lieu-tenant, Captain-Commander of the Enforcement Division, and eventually as sheriff for two terms. Prior to his current role, David served the Office of United States Senator Dan Coats – first as Regional Director for North-west Indiana, then as Senior Advisor. David earned his undergraduate degree from Indiana University – Northwest, and graduated from the FBI National Acad-emy, 165th Session. David currently resides in Lafayette with his wife Jenny of 34 years. They have two married adult chil-dren, and two grandchildren. John Trestrail, III graduated with honors, obtaining a B.S. degree in Pharmacy, from Ferris State University, Big Rapids, Michigan, in 1967, where he was initiated into Rho Chi (Pharmaceutical Honor Society). In 2010, he received the “Distin-guished Alumnus Award” from his alma mater. From 1967-1968, he attended graduate school, at the College of Pharmacy, The Ohio State University, Columbus, Ohio, ma-joring in natural product chemistry. Mr. Trestrail’s public service experience was as a Volunteer with the United States Peace Corps, from 1968-1970, where he taught chemistry at the University of the Philippines College of Agriculture, in the Republic of the Philippines, as well as special projects providing medical aid to members of the cultural minority peoples of the area. He is a practicing boarded toxicologist, and for many years, was a visiting instructor at the FBI National Academy in Quantico, Virginia, on the subject of criminal poisoning. He is recognized as the foremost authority on the subjects of criminal poisoning and murder by poison. For 33 years (1976-2009), he served as the Managing Director of one of the nation’s certified regional poison centers. He now serves as the Director of the Center for the Study of Criminal Poisoning. Since 1990, Mr. Trestrail’s seminars on “Murder by Poison!” and “Poisoners Throughout History”, have been received with wide acclaim by audiences throughout the United States, Cana-da, Europe, and Asia. Mr. Trestrail has served as an expert consultant in many criminal poisoning investigations, to law en-forcement and attorneys. Having presented over 300 seminars, he is a popular speaker. He has been honored as a Fellow by the American Academy of Clinical Toxicology, and is a Diplomate, by examination, of the American Board of Applied Toxicology. Mr. Trestrail founded the Center for the Study of Criminal Poisoning, as well as the Toxicological History Society, and has been featured in multiple episodes on The Discovery Channel, The History Chan-nel, The Learning Channel, and PBS. He was the project leader for the forensic research project that was able to resolve the key question in the British, 1910 Crippen murder case, using DNA comparisons with living exemplars, and took part in London, England, as a member of the research team, in the making of the PBS documentary “Secrets of the Dead: Executed in Error”, on this very famous poisoning murder case. He has been an active participant in the International Program on Clinical Safety, of the World Health Organization (WHO), working for the establishment of poison control services in the world’s developing countries. He is the author of the pioneering book Criminal Poisoning: An Investigational Guide for Law Enforcement, Toxicologists, Forensic Scientists, and Attorneys, published by Humana Press, in 2000 (2nd edition in 2007). He is also one of the co-editors of the popular book Toxicology Secrets, published by Hanley and Belfus Publishers, in 2001. His third book The Poi-son Quiz Book 2nd Ed., was published by McGraw-Hill, in 2006. Indiana State Coroners | Dr. David Avolt In-Service Training Conference � Mr. Trestrail is a member of the following professional organizations: the “American Academy of Clinical Toxicology” (AACT), the “American Association of Poison Control Centers” (AAPCC), the “North American Mycological Association” (NAMA) - a “Life Member”, the “Toxicological History Society” (ThiS), and the “Toxicology History Association” (THA). John and Mary Trestrail have been married since 1971, they have two children, and four grandchildren. In his spare time, John sings barbershop harmony with his barbershop quartet “Jubilation!”. Lou Ann Baker’s career in communications and public affairs has included service in state and local government, global environmental firms and her own consulting agency working with clients in business, motorsports, and not for profit indus-tries. A native of Corydon, she has worked with media at all levels, coached business and government executives, instruct-ed university classes in crisis communications and spoken around the world on media relations, government services, and health care/environmental topics. Rachel Kenny, INVDRS Epidemiologist, graduated from Syracuse University with a Bachelor of Arts degree in Anthropology and Forensic Science. She is currently working towards a Master’s degree in Epidemiology at the IU Richard M. Fairbanks School of Public Health. She worked at the University of Indianapolis prior to joining ISDH. Murray Lawry, INVDRS Coroner Records Coordinator, Murray graduated from Ball State University with a Bachelor of Science degree in Political Science and a Masters in Public Administration. He is also a certified EMT. Murray has been with the Indiana State Department of Health for a number of years. He transferred to the Trauma Program from the ISDH Hos-pital Preparedness Program, where he was responsible for the hospitals in Preparedness Planning Districts 1,2,3,4 and 6. Tony Ciriello, MDI retired after a 34 year career in law enforcement most recently serving as Chief of Police for the Syra-cuse Police Department. Ciriello started his law enforcement career as an arson investigator for the State of Indiana before joining the Kosciusko County Sheriff’s Department where he served in numerous capacities during his tenure there. Those capacities were patrol officer, supervisor, detective, narcotics officer, D.A.R.E. officer and field training officer. In addition to his law enforcement career, Ciriello holds a certification as a Medicolegal Death Investigator, is also a Center for Disease Control Certified Infant Death Investigator/Instructor. For the last 17 years Ciriello has served as the Director of Training and Education for the Indiana State Coroners Training Board and for 23 years has been a Deputy Coroner and now serves as the elected Coroner in Kosciusko County. Ciriello is currently the business manager and pre-arrangement specialist for McHatton-Sadler Funeral Chapels in Warsaw and Pierceton, Indiana. He holds licenses in Life and Health In-surance. In addition to those activities he is the Past-President of the Indiana Association of Chiefs of Police. Ciriello is a graduate of the Kosciusko Leadership Academy and the Indiana Police Executive Leadership Academy. He serves on numerous local boards and civic organizations. Ciriello is the recipient of the Excellence in Government Award from the Syracuse/Wawasee Chamber of Commerce, two times Leaders Leader Award from the Indiana Police Executive Leadership Academy and was awarded the William Sudbury Memorial Leadership Award from the Indiana Association of Chiefs of Police. He was honored in 2015 with a Sagamore of the Wabash by Governor Pence. � Indiana State Coroners | Dr. David Avolt In-Service Training Conference Doug Banks, MDI, currently is serving his 2nd term has Decatur County Coroner. Doug has been an EMT for the past 20 years for the Decatur County Hospital. A member of Clarksburg Volunteer Fire Department for 25 years. Doug and Charity are owners of a lawn care business for the past 25 years. They have a son Cody and daughter Christin. Mike Burris, MDI is the Chief Investigator for the Allen County Coroner’s Office. He was hired into the position in October of 2010. He retired from the Fort Wayne Police Department in 2010 where he served in a supervisory position in the Crime Scene Management Section since 2001 when he was promoted to Sergeant. Prior to his promotion he served as a patrol officer in the Crime Scene Section. Prior to 1991 Michael served as a Patrol Officer for the Fort Wayne Police Department. In 1995 Dr. O’Shaugnessy accepted Michael into the Allen County Coroner’s Office as a Deputy Coroner. In 2001 Dr. Bran-denberger hired Michael as a part-time deputy coroner, until his retirement and was then hired as the chief investigator. Michael has given instructional training over Photography, at the Sexual Assault Nurses Training held in Fort Wayne on numerous occasions. He has instructed a Photography Workshop for Coroners at the Annual Coroner’s Conference. He has taught Crime Scene Management for the Fort Wayne Police Academy for recruit classes since 1995, and has lectured dur-ing the annual in-service trainings provided by the police department. Michael has been a guest lecturer for various classes at IPFW, Ivy Tech, and Indiana Institute of Technology. Annette Morgan, MDI, is currently employed as a Registered Nurse for the MSD of Martinsville and a 911 Dispatcher for Morgan County. Annette started her career in law enforcement as a dispatcher and reserve police officer before getting hired full time as a patrol officer in Mooresville Indiana, graduating from ILEA basic class 03-155. In 2008 Annette returned to college and obtained a degree in nursing which she uses in her employment today. Annette was a deputy coroner in Morgan County for 12 years before being elected as the Morgan County coroner in 2013. Annette is a certified Medicolegal Death Investigator and Certified Infant Death Investigator. Annette serves on many boards in Martinsville. Annette also teaches new recruits at the Indiana Law Enforcement Academy about scene preservation and Coroner law. Annette is active with Morgan County Substance Abuse Council and was instrumental in getting narcan for the county. John Sullivan, MDI - has been an employee of LaPorte County Coroner’s Office for 23 years, the last eight as Coroner. He also serves as Captain on LaPorte City Fire Department for 28 years. John has been a firefighter for Cass-Clinton VFD in Wanatah for 36 years. He is married with two sons Michael and Ryan. John is a member of Sacred Heart Church in Wana-tah. He farms a 300 acre grain farm in Wanatah. John received the 2nd Annual Crowner Award for Outstanding Coroner in 2013 Dr. Tracey Corey is a native of Kentucky, Tracey S. Corey MD received her undergraduate degree from Centre College in Danville KY in 1983, with a major in Biochemistry and Molecular Biology, With Distinction. She received her MD degree from University of Louisville School of Medicine in 1987. She completed her postgraduate training in Anatomic Pathology and Forensic Pathology in Louisville, KY, and became a Kentucky Medical Examiner in 1992. She was appointed Kentucky Chief Medical Examiner by Governor Paul Patton in 1997. She served as Kentucky Chief Medical Examiner from 1997 until her retirement from Kentucky state government in 2016. During her time as she also served as Professor of Forensic Pathol-ogy, Fellowship Program Director, and Division Director of the Division of Forensic Pathology at University of Louisville School of Medicine. She has been featured on national news shows including Dateline NBC, 48 Hours, and Good Morning America. Since retirement from Kentucky, she performs private pathology work, serves as a contract consultant to the FBI Behavioral Analysis Unit, and continues to teach death investigators around the nation. Forensic Entomology 1989, Ph.D. F.E. 1993 (all from Purdue) After farming for over 15 years, Dr. Neal Haskell entered a new graduate program at Purdue University to study how insects could be used to answer questions at murder scenes. His initial interest in “bugs” began with a 4-H project, age 11. He completed a Master’s of Science Degree with the focus in Forensic Entomology in 1989 (the first such degree in the country) and a Doctor of Philosophy Degree with Forensic Entomology (also the first in U.S.) as the specific area of study in 1993. He has given hundreds of scientific lectures, seminars and training classes worldwide to provide coroners, medical examiners, death scene investigators, and others with valuable information regarding entomological evidence from death scenes. Also several books and book chapters have been authored by Neal. Neal has been featured in a number of books and in over 150 news articles, books, and other news media. In a book by Dr. Michael Baden and Marion Roach, Dead Reckoning, an entire chapter is devoted to stories of Dr. Haskell, the pig research and training in Rensselaer, and interest-ing case studies. Dr. Haskell is a Professor of Forensic Science and Biology at Saint Joseph’s College in Rensselaer, IN and also continues as a private international forensic entomology consultant to hundreds of law enforcement agencies across North America and in Europe. He holds adjunct appointments at Michigan State University and the University of Nebraska. He has board certi-fication as a Board Certified Entomologist and is one of twelve members as a Diplomate of the American Board of Forensic Entomology. Some of the agencies that he has worked with over the years include the Indiana State Police, FBI, Office of the Chief Coroner, Ontario Canada, Florida Department of Law Enforcement, New York State Police, and Seattle Washing-ton Medical Examiner’s Office, Las Vegas Metro Crime Lab, Montana Medical Examiner’s Office. During this work over the past 31 years he has assisted in over 800 cases which have had questions regarding insects. He has been accepted and recognized as an expert witness in both state and federal courts and has qualified as an expert in 31 states in the U.S. and regional, provincial courts in Canada and also the Ontario Court of Appeals. He has given testimony in over 120 trials and hearings, many of which were capital murder cases. Some of the most notable cases include the Bernardo/Homolca trial, the most highly publicized trial in Canadian history and the Westerfield trial in San Diego (Danielle Van Dam murder). He also assisted in insect collections at the Branch Davidian Compound in Waco Texas and the criminal investigation of the Sci-entology Group in Clearwater, Florida, also the Anthony case from Florida. Research was conducted by Dr. Haskell and others at the University of Tennessee, “Body Farm” where actual human corpses were compared to pig corpses for estab-lishing a replacement for humans in decomposition and forensic entomology studies needed across the U.S. This was the first major funding provided to forensic entomology by the US Justice Department in the U.S.. She is currently the commander of the Purdue University Detective Divi-sion. In addition, she is the Chief Deputy Coroner for Tippecanoe County and is a certified Medicolegal Death Investiga-tor. She graduated from Purdue University with a Bachelor degree. She is a Fellow with the American Academy of Foren-sic Sciences and has presented homicide case studies and research related to causes and trends of deaths among college / university students across the nation. She was certified as a polygraph examiner in 2000. Prior to joining the Purdue Uni-versity Police Department she was the Director for the YWCA’s Domestic Violence Intervention and Prevention Program. She has testified as an expert in Domestic Violence and Sexual Assault in both Federal and State courts. Dr. Jonathan Hayes is a British forensic pathologist. After forensic training in Miami, he became a New York City medical examiner in 1990. In the mid-1990’s, he was recruited by trendy Paper magazine; for several years, he wrote a monthly column on the music and culture of the nightclub and rave scenes, before shifting his focus to food writing, eventually be-coming a contributing editor for Martha Stewart Living magazine. His writing – on food, travel, video games and culture - has appeared in New York magazine, the New York Times, the New York Observer, Gourmet, Food & Wine, Premiere and various other magazines and journals. His chief forensic interests are asphyxial and drug-related deaths. His two novels, the forensic thrillers PRECIOUS BLOOD and A HARD DEATH, were published both in English and in numerous foreign lan-guage editions. In 2016, Hayes traded the hectic pace of life in New York for a quieter, hopefully more writing-friendly life as a medical examiner in beautiful Naples, Florida. Michael B. Brady, Director of INSPECT, Indiana Professional Licensing Agency Dr. Henry Lee - “Born in China and grew up in Taiwan, Dr. Henry C. Lee first worked for the Taipei Police Department, attaining the rank of Captain. With his wife, Margaret, Dr. Lee came to the United States in 1965, earned his B.S. in Foren-sic Science from John Jay College in 1972, and continued his studies in biochemistry at NYU where he earned his Master’s Degree in 1974 and Ph.D. in 1975. He has also received special training from the FBI Academy, ATF, RCMP, and other organizations. Dr. Lee is a recipient of 20 honorary degrees: Doctorate Degrees of Science from University of New Haven, University of Connecticut; Honorary Doctorate of Law from Roger Williams Law School, Mitchell College, American International Universi-ty and Taiwan Scientific Technology University; Honorary Doctorate Degree in Humane Letters from University of Bridge-port, St. Joseph College, Armstrong University; in recognition of his contributions to Law and Science. Dr. and Mrs. Lee have been married for 50 years and have two grown children, a daughter and a son, and three grandchildren. Dr. Lee is one of the world’s foremost forensic scientists. His work has made him a landmark in modern-day forensic sci-ences. He has been a prominent player in many of the most challenging cases of the last 50 years. Dr. Lee has worked with law enforcement agencies from 46 countries in helping solve more than 8,000 cases. In recent years, his travels have taken him to England, Bosnia, China, Germany, Singapore, Croatia, Brunei, Thailand, Middle East and other locations around the world. Dr. Lee’s testimony figured prominently in the O. J. Simpson, Jason Williams, Peterson, and Kennedy Smith trials, and in convictions of the “Wood Chipper” murderer as well as thousands of other murder cases. Dr. Lee has assisted local and state police in their investigations of other famous crimes, such as the murder of Jon Benet Ramsey in Boulder, Colorado, the 1993 suicide of White House Counsel Vincent Foster, kidnapping of Elizabeth Smart, death of Chan-dra Levy, the reinvestigation of the Kennedy assassination and the Casey Anthony case. Dr. Lee is currently the director of Forensic Research and Training Center and Distinguished Chair Professor in Forensic Science of University of New Haven.
