We understand that choice, flexibility and affordability are top priorities. To help best meet your needs, we offer two provider networks for
our Oxford2 products — the Freedom Network and Liberty Network.
•Freedom Network: Premier tri-state network (CT, NY, NJ), which includes over 83,000 physicians and other health care professionals,
who practice at more than 124,000 office locations, and over 200 hospitals.3
•
Liberty Network: Subset of the Freedom Network, and is only available with Oxford products for New Jersey and New York
employers. The Liberty Network provides members accesss to 54,600 physicians and other health care professionals, who practice
at more than 78,000 office locations and 168 hospitals. All Freedom Network providers in Connecticut are considered in-network for
New Jersey and New York members enrolled in a Liberty Network product. That adds another 13,900 physicians and 32 hospitals.3

National Access
In addition to our robust local network, many of our plans give your employees seamless access to our UnitedHealthcare Choice Plus
national network4 — the largest single proprietary network available — when they are outside of the tri-state area of Connecticut, New
York and New Jersey. This is great news for “snow birds,” travelers and parents of students attending an out-of-state college. To search
for one of the 617,000 physicians and other health care professionals or 5,010 hospitals in our UnitedHealthcare Choice Plus national

Using participating providers helps your employees get the most from their health plan
Good news! 96%1 of Health Net physicians, hospitals and other health care professionals already participate in our tri-state
(Connecticut, New York and New Jersey) Freedom Network.
• Based on a comparison of PCPs, OB/GYNs and specialists1, our Freedom Network has 9,300 more physicians than the Health Net tri-state network.
• Our Freedom Network has 24 more hospitals than the Health Net tri-state network.1
In the unlikely chance that a Health Net physician does not participate in our network, your employees will receive information on
transitional care options. For example, an employee who is receiving ongoing treatment, or is in her second or third trimester of
pregnancy, will receive this information.

Finding a physician is easy
1) Members can simply use the provider search tool on our Web site, www.oxfordhealth.com, to find physicians and other health care
professionals in their area, or to search by the criteria that are important to them.
2) Members without Internet access can request a copy of the printed provider directory.
3) Members can call Customer Service at 1-800-444-6222 to confirm whether a provider participates in our network.
More information for you about physicians
The UnitedHealth Premium® designation program is an online tool that enables members to view information about physicians
identified for meeting or exceeding standards for quality care established by professional societies like the American College of
Cardiology. Members may access information indicating physician quality and efficiency, which can be of assistance with making
decisions about their care. If a physician has received a Premium designation, members will see one or two blue stars next to the
physician’s name: * = quality physician, ** = quality + efficiency of care physician.
Of course, evaluating physicians is only half the story. We believe physicians need to stay at the top of their respective fields and to
help, we sponsor continuing medical education. By putting these steps in motion, we believe our members have a better chance of
receiving the best possible care. (Note: Program applies to certain specialists and not to all physicians.)
Pharmacy network
All members enrolled in an Oxford product have access to our national pharmacy network of over 60,000 retail pharmacies5, including
both chain and independent stores located across the United States. We have compared our pharmacy network with your current Health
Net pharmacy network and we are glad to tell you that we have over a 99% network match.6 So you can feel comfortable knowing that
your employees will have virtually the same access as they have now.
Laboratory network
Members enrolled in an Oxford product have access to 425 free-standing lab patient service center locations in the tri-state area of
Connecticut, New York and New Jersey — which is 90 more locations than the Health Net tri-state lab network.7 We have a broad local
and national network, including specialty (e.g., Clinical Lab Partners, Enzo, Sunrise Labs, Shiel Medical Lab) and hospital labs, as well as
Laboratory Corporation of America. You and your employees can search for participating laboratory service center locations using our
provider search tool on www.oxfordhealth.com. (Note: Quest Diagnostics is an out-of-network provider.)
