Mrs. Hale`s Disclosure Document

Mrs. Hale’s Disclosure Document Welcome to another school year! These are the basic policies: Curriculum: W
e use programs such as Balanced Math and Balanced Literacy to teach the Utah State Core Curriculum. Grading Policy: G
rading will be determined by participation, completed assignments, behavior, tests, quizzes, and projects. Homework: I n fifth grade, math and reading homework is online. Each week students m
ust read at least 90 minutes a
t w
ww.myon.com , plus an additional 60 minutes r eading other books/materials. Students will need to also complete t wo fifthgrade math skills a
t 90 or higher on www.ixl.com. If online access is not available to students, we host the Homework Club before and after school to provide access to students who sign up. Special Activities & Donations: S
pecial activities in fifth grade are privileges meant to enrich learning. We as teachers will notify you in writing when special activities are planned. A
ny voluntary donation you can make in time, funds, or materials is greatly appreciated. T
wo activities we request donations for are: Hope of America ($10 needed between Oct 1018) and CLAS ropes Course ($18, needed between Jan. 30 Feb. 10). If we find that funding for these activities is insufficient after the donation windows have ended, we will return your donations and plan alternate activities for our students. Students who demonstrate patterns of misbehavior will need to remain at Geneva instead of attending field trips. Suspensions, skipping class, or bullying are a few behaviors that disqualify students from field trips. Deadlines: S
tudents who turn in work on time receive full credit, while late assignments receive partial credit. The only exceptions to this policy are illness or other districtapproved absences. Students must complete the class assignments, even if they are absent. Following an absence, students must make up work within a reasonable time frame. Attendance: S
tudents are expected to be in their seats and prepared when the bell rings at their respective start time. Please call the school if your child will be absent. I f you need to check your student out early, you must stop in the office and sign him or her out. This policy is meant to protect students’ safety. We need confirmation from the office before we may dismiss children early to parents or family, even if we know and recognize them. Discipline: W
e seek to create a safe learning environment for all students so we use the Geneva Handbook so every student knows his or her responsibilities. Computer Use: S
tudents and teachers may benefit from using computers and the Internet. For students to use the internet, parents must complete an online written application and students need to follow the district’s Acceptable Use Policy. Log onto h
