Root Operation: Extraction
In an effort to aid Health Information Management Coding and Medical Billing Professionals with
ICD-10, the following training tip is provided with an educational intent
The third character in PCS defines the root operation, or the objective of the procedure. The root operation of
‘Extraction’ represents the third character value ‘D’ for codes in both the Medical Surgical and Obstetrics Section.
Definition of Extraction: Pulling or stripping out or off all or a portion of a body part by the use of force.
Explanation: Extraction is coded when the method employed to take out the body part is done so by pulling or stripping.
Minor cutting, such as, that used in vein stripping procedures, is included in Extraction if the objective of the procedure is
accomplished by pulling or stripping.
**Note: The seventh character qualifier ‘Diagnostic’ is used to identify extraction procedures that are biopsies.
Examples
• Needle biopsy of bone marrow of the iliac crest: 07DR3ZX
• Bone marrow biopsy is coded to the root operation Extraction because the objective of the procedures is to pull
out a portion of the bone marrow
• Nonexcisional (Versajet) debridement of left foot due to diabetic skin ulcer: 0HDNXZZ
• The indexed entry for “Debridement, Non-excisional” in the PCS codebook refers the coder to Extraction
• Suction dilation & curettage using hysteroscopic approach for retained products of conception: 10D18ZZ
• Extraction would be the root operation because the uterine wall is scraped during the procedure
**Note: A code from the Obstetrics section “1” is only assigned if the procedure is performed after delivery or abortion.
