complex patients, this symposium is designed to be
even more interactive than usual.

psychologists or pharmacists, to comprise your team.

practitioner students/residents to be
part of your team.
You and your team will:
 Take advantage of breakout sessions designed
to stimulate team conversation leading to more
effective collaboration;
 Share ideas and strategies during lunch,
facilitated by symposium planners;
 Enjoy time to informally network in a comfortable,
creative atmosphere.
The registration fee is $95.00 which includes:
 you and up to three additional team members
 all meals and refreshments
 7.75 Category 1 CMEs/CEUs per team member
Please register today. It is not necessary to list your
team members at this time. We will confirm your
registration and send a request for names and contact
information mid-August. In the meantime, copy and
share this brochure with those you invite, or direct them
