What?
This form is used for students seeking to defer a final examination. It must be submitted within five working days after the scheduled
examination date. Approval for a deferred examination is always at the discretion of the faculty. A student requesting a deferred final
examination for medical reasons must submit both this form and the UOIT Medical Statement form together to the faculty office. The
Medical Statement form is available on the UOIT website and from the Registrar’s office. Faculties will only grant deferred
examinations where sufficient documentation is submitted by the student.
Who?
The form(s) must be completed, dated and signed by the student. The rationale for the deferral must be provided and supporting
documents must be attached to this form. It is the responsibility of the student to determine from the faculty whether a
deferred final examination has been approved.
When?
The processing time for deferral decisions usually takes up to a week. If approved, students will initially be notified of the deferred
exam time period. Once the schedule has been set, students will receive confirmation details about their deferred exam.

Fee: Please note that if this request is approved, there is a $25 non-refundable administration fee for each deferred
exam. This fee will be charged to your student account.
Place of writing: It is the responsibility of the student to check their UOITnet email for messages regarding the
room and time of the deferred examination.
