The quotes are due on or before 1:00 PM (local time) Wednesday August 05, 2015
in the office of the Village Clerk.
PROJECT AWARD
The Engineer shall review each quote and forward the results to the Owner. The
Owner will award the work in his best interest on or about August 11, 2015. The Owner
anticipates construction to shortly follow the award, with piping completion by
September 25, 2015 and restoration completed by October 9, 2015.
CONTRACTOR AGREES
The undersigned Contractor agrees to furnish all material and perform all work, as
shown in the Plans and Specifications, in a workmanlike manner, as required by the
Wisconsin Department of Natural Resources, in accordance with the “Standard
Specifications for Sewer and Water Construction in Wisconsin”, the Wisconsin DOT
Standard Specifications for Road and Bridge Construction, and as acceptable to the
Village of Kronenwetter.
CONTRACTOR REPRESENTS
The Contractor has examined copies of the Plans, Specifications and the quotation
documents.
The Contractor has familiarized himself with the nature and scope of the work, the site
and any local conditions that in any manner may affect the cost of furnishing the
material and completing the work.

The Owner has not performed any soil tests pits on the project site. The site is
believed to be sand and gravel formation, with groundwater below 20 feet deep.
The Contractor assumes the responsibility for obtaining and studying any
examinations, investigations, explorations or tests that pertain to the
subsurface or physical conditions at the site or otherwise that may affect the
cost, progress or performance of furnishing the material and completing the
work.
This Quote is genuine and not made in the interest of or on behalf of any undisclosed
party.
MATERIALS
1) Copper Service
Type K soft annealed seamless copper tubing, AWWA C-800 and ASTM B-88
2) Insulation
Extruded polystyrene foam insulation, minimum 2-inch thick, 4 ft. x 8 ft. sheets.
3) Restoration
Areas of turf disturbed by the Contractor, shall be spread with a minimum of four (4)
inches of salvaged topsoil, DOT seed mixture No. 2, fertilizer and mulch in
accordance with DOT Section 630.
Pavement areas disturbed by the Contractor, shall have a minimum 8 (eight) inches
of crushed native base course DOT 1-1/4 inch dense graded. Bituminous Pavement
shall be a minimum of 3 (three) inches of E-1 (MV) pavement, placed in two layers
with the binder a maximum of 1.75 inches. Tack coat between lifts if more than 4
days between lifts or unusual amounts of debris accumulates.
CONSTRUCTION METHODS
The Contractor shall utilize the best available construction methods, as recommended
by material manufacturers, as outlined in the Standard Specifications for Sewer and
Water Construction in Wisconsin, the Wisconsin DOT Standard Specifications for
Road and Bridge Construction and/or the Village of Kronenwetter Standard
Specifications or as described herein.
1) Intent of Project
The purpose of the project is to re-lay and/or insulate water services that have a
history of frequent freeze ups in the street portion of the line.

2) Scope of work
The Contractor shall expose the water service on the street side of the curb stop
and determine the depth of cover. If the cover is over 7.5 ft., the contractor shall
continue exposing the service to the water main to verify that the service remains
deep. If at any point the depth of cover is between 7.5 and 6.5 ft. the service shall
be insulated with extruded polystyrene foam four (4) inches thick, four (4) feet
wide and with one (1) ft. vertical sides – completing a three (3) sided box. If at any
point the depth of cover is less than 6.5 ft. the service line shall be lowered to 7.5
ft. of cover. The contractor shall complete the work per SPS 382.30(11)©2.d and
Tables 382.30-5, -6 and -7, or the above, whichever is more restrictive.
3) Performance Testing
The water service shall be visually checked for leakage while under system
pressure prior to the backfilling of the trench.
4) Surplus Material
Surplus excavated material, blacktop and excavated soil, shall become the
property of the Contractor. Unclassified excavated material shall be used first to
fill road sections to subgrade before removing material from the site. Salvaged
excavated CABC material, if any, shall be stockpiled on the site for re-use as
base course.
5) Bedding and Initial Backfill
On site materials shall be used.
6) Backfill
After confirming there are no leaks the trench shall be backfilled. Backfill material
shall be on-site or imported soils free of organics, frozen material and rocks
greater than eight inches in size. Materials shall be placed in uniform lifts and
compacted as specified in the Standard Specifications for Sewer & Water in
Wisconsin, except that flooding will not be permitted.
7) Restoration
The disturbed turf areas shall be restored immediately. At least one (1) but not
more than two (2) weeks after the trench has been backfilled the blacktop shall be
saw cut and the excavated area prepared for the blacktop patch.
SPECIAL CONDITIONS
1) Site Conditions
Two of the locations are in public streets. The last is located in an easement area off
of Old Hwy 51. Typical underground utilities should be expected at all three sites.
2) Insurance
The Contractor shall have insurance of the type and amount commensurate with the
scope of the project to protect the Owner, the public and himself. At a minimum there

shall be a $1,000,000 General Liability policy. The Contractor shall provide a Certificate
of Insurance to the Owner, with the owner listed as co-insured.
3) Safety
During the installation of the underground facilities the Contractor shall have control of
the work areas and shall be responsible for safety on the site. The Contractor shall
place and maintain warning signs and barricades at the entrance to the site.
4) Noise
This project construction is limited to the hours of 7:00 AM to 8:00 PM.
5) Equipment and material storage
The Owner shall designate a space on site for the Contractor’s material and
equipment. The Contractor shall review with the Owner the areas of the site that are
not to be disturbed.
METHOD OF MEASUREMENT AND PAYMENT
1) Method of measurement
a) Excavation – length of pipe from curb stop to water main.
b) Insulation per square foot of 4-inch insulation on top and 2-inch vertical sidewalls.
c) CABC – Cubic yard by neat calculation of 8-inches deep by width of street
(shoulder to shoulder) by length of patch.
d) Pavement restoration by square yard of surface.
e) Saw cut by lineal foot.
f) Disturbed turf surface are by square yard restored with 4-inches of topsoil,
fertilizer, seed and mulch.
2) Method of payment
The Contractor may submit monthly pay requests to the Engineer. The Engineer shall
review, correct as necessary and approve it for the Owner’s payment. The retainage
shall be on the following basis;
a)
10% retainage of the first 50% of work completed
b)
5% retainage until 100% completion of work.
3) Warranty

The Contractor shall warranty all materials and workmanship for one year following the
acceptance of the work. When notified of a defect in materials or workmanship, the
Contractor shall complete corrective work within 30 days.

The Contractor accepts all of the terms and conditions of this Invitation to quote. The quote
will remain subject to acceptance for 10 days following the quote date.
It is the intent of the Owner to have the services installed and backfilled by September
25, 2015 with complete restoration by October 9, 2015.

VILLAGE OF KRONENWETTER
MATERIAL SUPPLY AND INSULATION CONTRACT AGREEMENT
Water Service Insulation 2015
This agreement, dated below by and between the Village of Kronenwetter and
___________________________, an independent contractor, agree as
follows:
WHEREAS, the Village operates and maintains a water distribution and
wastewater collection system, and
WHEREAS, the Village is required to install bypass lines around all meters 1.5inch and larger, and
WHEREAS, the undersigned independent contractor has agreed to provide
materials, install said materials and restore the project site as per the project
quotation documents, attached to this agreement and made part of it.
NOW THEREFORE THE PROVISIONS OF THIS AGREEMENT ARE:

The Village agrees to pay the independent contractor the sum of _____________, with final
total amount based on the units provided and the quoted unit prices for the complete
installation of the services and restoration of the project site.
The independent contractor agrees to provide all labor and materials to complete all work
satisfactory.
The independent contractor agrees to indemnify and hold harmless the Village of
Kronenwetter from any or all causes of action caused by the negligence of the independent
contractor or his employees during the time this Contract Agreement is in force.
The independent contractor shall provide the Village with a Certificate of Insurance, with the
Village listed as an insured party.
The Village of Kronenwetter may terminate this Contract Agreement with the independent
contractors at any time by serving a two-week notice.
The independent contractor agrees to comply with all federal and state laws and policies.
