operate initiatives and provide
funding to more than
400 health organizations
in the region.
Dakota Medical Foundation improves the health and health care
of people in eastern North Dakota and western Minnesota by
closely working with and through hundreds of engaged partners
in the region: health-related nonprofits, healthcare facilities,
universities, public agencies, foundations, businesses, volunteers,
donors and more.
We cast a light on significant health issues, advance
better understanding around these complex challenges
and forge strong collaborative networks to address or
solve them. Obesity is the single largest and costliest threat
to America and our region, causing heart disease, diabetes
and many other chronic illnesses. Together with highly-engaged
partners, we’re creating childcare, school, work and community
settings with abundant healthy foods and physical activity in
order to reverse the obesity epidemic.
We invest our time, expertise and resources in helping nonprofit
leaders reach their highest personal potential so they, in turn,
can shape organizations that produce extraordinary health results
for the people of our region.

Discuss the foundation’s role in
building exceptional nonprofits.
Pat Traynor: A great nonprofit achieves its mission in a
superior way. Dakota Medical Foundation, together with
Alex Stern Family Foundation, created the Impact Institute
to prepare an army of charitable top-performers, highly
skilled in fundraising and exceptional at producing results.
With hands-on training and technical assistance, we help
nonprofits clarify their missions to be distinct, focused
and compelling.
Dr. Vetter: Clarity of mission leads to accountability
for results. It inspires passion and commitment by staff,
directors, volunteers and donors. These nonprofits are able
to attract the resources necessary to go above and beyond
anything done in the past – getting exceptionally better
rather than incrementally better. Charities that become adept
at fundraising have strong economic viability. They can go
beyond merely treating problems to preventing and solving
problems in our communities.
Pat Traynor: In a sample study we conducted of
22 nonprofits who participated in our training from 2007
to 2010, we discovered an increase in financial support
from the public of 48% — rising from $32 million to a
remarkable $47 million in the three-year period. That
was an average increase of 16% per year.
Many years ago, we began talking about the difference
between “feeding for a day” and “teaching how to fish”
to feed for a lifetime. We are extremely proud of the many
committed partner organizations that have adopted new
methods of fundraising, or learned how to fish. They testify
that this challenge to grow their own bench strength has left
them dramatically better off.

We have sharpened our own skills by testing ourselves
to understand and teach best practices. Dakota Medical
Foundation has shaped a unique, carefully-defined role
of leveraging talents of nonprofit leaders.
We create value when our foundation generates the greatest
possible societal return from our expertise, financial
investments and collaborative networks – multiplying our
impact through this leverage strategy.
Dr. Vetter: For the future, we need bold new health and
wellness ideas that break through the status quo and the
limits of current thinking. Our Breakthrough Idea Challenge
stimulates ideas that can have dramatic impact, and it’s
exciting to have Dakota Medical Foundation be a lightning
rod for health innovation in the region through this program.
We have a long history of pursuing breakthroughs,
establishing pilot projects, and then guiding them to
fruition – our Lend A Hand program and AED Initiative
are examples. It will be exciting to accomplish this in a
new way with the Idea Challenge.

Pat Traynor: The future is filled with hope and
immense opportunity for health organizations that
prepare themselves to successfully tap into the wealth
transfer phenomenon. It is estimated that $308.6 billion
of wealth will transfer to heirs, charities and others in
North Dakota, and $59 trillion will transfer nationally.
In North Dakota alone, $95 billion is estimated to transfer
to charities from 2007 to 2061. We feel this figure is quite
conservative given the explosive economic expansion in
the state that has been fueled by strong growth in the
agricultural and oil industries.

Our partners are developing smart fundraising systems and
acumen to tap this unprecedented $95 billion opportunity
for funding their good work. They are capturing the endless
possibilities created by the transfer of wealth in their own
back yards! These possibilities include solving problems
rather than just treating them, and achieving the greatest

Cass Clay
Healthy People
Initiative
The Healthy People Initiative leads community
changes to prevent chronic disease, reduce
childhood obesity and help people live longer,
healthier lives – making Cass and Clay Counties
the healthiest place to live. The initiative is a
partnership of Dakota Medical Foundation and
committed partners that are guiding strategies
at childcares, schools and elsewhere in the
community so these settings support healthy
behaviors – making the healthy choice the easy
choice. We’re seeking to reduce childhood
overweight and obesity in Cass and Clay
Counties 20% by 2020.
Already 200 of the 600 Cass-Clay childcares
have taken part in a program to teach and reward
changes to early childhood settings, ingraining
healthy habits at the youngest ages.
We’ve teamed with all local schools as they
develop successful wellness policies, find
innovative ways to improve school lunch menus,
and add 60 minutes of physical activity to every
school day. Continually improving, these changes
affect the lives of 28,000 school children in
Fargo-Moorhead annually.
The Healthy People Initiative helped bring several
national experts to Fargo-Moorhead in 2012,
including a Cornell University professor on
research-based lunch line design to nudge
students to make healthier food choices.
Specialized knowledge and models for ideal food
placement in the school lunchroom were shared
with schools during the day, and a large public
crowd eagerly gained knowledge as he interpreted
research findings for practical use at home.
We presented our own pioneering work and
approaches in regional and national webinars.

To tap into the power of committed individuals,
we taught more than 50 volunteers of diverse
professions the compelling facts around the obesity
crisis in Cass and Clay Counties. We shared
resources and insights that they’re now using
individually to create a groundswell of active
living and healthy eating.
In the past 3 years, more than 20,000 people have attended open street events organized by Cass Clay
Healthy People Initiative and Active in Moorhead. The goal: igniting movement in our communities and
inspiring people to adopt more physically active lifestyles – and make active living the status quo. In
summer 2012, StreetsAlive! events such as pole vaulting drew crowds of 11,500 attendees.
We expect momentum for the Healthy People
Initiative to grow through the new Dakota
Medical Foundation building to be complete in
2013. For example, its teaching kitchen will
provide people essential life skills and coaching
for professionals on preparation of healthy
foods. Audiences can witness and engage with
instructors for a new depth of learning. Webinar
capabilities from our kitchen will enable rural or
distant audiences to learn healthy cooking!
Two out of every three adults are overweight or obese, and the annual cost of
illnesses related to this epidemic is $190.2 billion in America.
Healthy lifestyles can do more to improve health than any other single medical
breakthrough of the past 100 years. Worksites are rising to the challenge of
helping their workers reduce preventable diseases.
Beginning in 2009, Dakota Medical Foundation partnered with Healthy
North Dakota and Blue Cross Blue Shield of North Dakota to help employers
begin and sustain effective, cost-efficient wellness programs across the state.
More than 200 North Dakota worksites have implemented measurable wellness
programs to improve the health of 48,000 employees and 24,000 family members.
The Worksite Wellness Initiative transitioned at
the end of 2012 to the capable hands of Blue Cross
Blue Shield of North Dakota. We invested nearly
$650,000 in this program over four years to fuel
its development.

SPOTLIGHT ON
I N W E S T FA R G O
ROCKS!
On rare occasions, you find unexpected
champions. Dakota Medical Foundation found
three in the West Fargo school system.
In 2010, as our Healthy People Initiative was
gaining momentum, Westside Elementary
Principal Beth Slette took a closer look at the
tradition to celebrate student birthdays with high
calorie sugary snacks. These snacks created a
predictable sugar high, followed by a sugar low
and a full tummy when lunch time rolled around.
The snacks interfered with academic learning and
student health. Beth was determined to change
the culture by engaging staff and parents in
shaping a healthier future.
Today, student birthdays are announced over
the intercom and each child receives a special
birthday trinket. Once a month, birthday students
go to the gym with the principal to play his/her
favorite games until they are “red in the face.”
What Beth Slette began, Westside’s new principal,
Tabatha Joyce, continues. Tabatha has added to
the movement by welcoming a team of North
Dakota State University physical education majors
to help Westside identify and implement more
physical activity opportunities in the school day.
Tens of thousands of junk food calories eliminated
annually caught the attention of West Fargo
Assistant Superintendent Louise Dardis. She
charged ahead to revise and strengthen the
district’s school wellness policy and adopt
Westside’s classroom celebration standards. They
didn’t stop there. In 2012, West Fargo Schools
took a big leap and served as the Healthy People
Initiative’s pilot site for Active Recess training.
Typically, recess supervisors observe children and
mediate behaviors. That was about to change!
All 120 West Fargo Schools recess supervisors
attended a pilot full-day training to facilitate
20 minutes of daily moderate to vigorous physical
activity into the students’ recess time – preferably
outside and before lunch. Some who attended
said it was the best training they’ve ever received.
Assistant Superintendent Dardis also arranged
for active recess to be the topic of P.E. teachers’
professional development days and elementary
principal meetings. She is helping weave physical
activity into the fabric of the district and shares that
“SchoolsAlive! Active Recess training is practical,
engaging and highly effective!”
The future holds unprecedented joint trainings for
physical education staff and recess supervisors,
the start of Active Recess Committees, an Active
Recess Week and classroom teacher training.
Beth, Louise and Tabatha – bold champions of
change. We applaud their courage and commend
them for positively impacting the health of their
students and families.

activity for at least 20 minutes in the great outdoors! Recess supervisor Penny Eckelberg keeps
kids hopping and playing, using skills she learned at Active Recess training.

Lend A Hand
Initiative
In fiscal 2012, $1.2 million was raised to help 44 families.
Dakota Medical Foundation’s Lend A Hand
Initiative teams with volunteers who are helping
families experiencing financial hardship due
to medical crisis. Our program director offers
experienced fundraising guidance, a detailed
event planning kit and help with community
fundraiser promotions. People are inspired to
open their wallets with match funding provided
by Dakota Medical Foundation. For those
who can’t attend dinners or events, online
giving is easy and immediate at our website
dakmed.org/lendahand.
In 2012, Lend A Hand celebrated its 5th
Anniversary and a total $5.2 million raised
for 194 families in Cass and Clay Counties.
Lend A Hand has teamed with over 5,000 caring
community members who have led fundraisers
in local communities. More than 75,000 people
attended or supported these events.
Since 2007, Dakota Medical Foundation has invested
$1.6 million in Lend A Hand, including $1 million in
match dollars to families. Ninety percent of the
families assisted have insurance coverage.

Children’s
Mental Health
Initiative
Children’s Mental Health Initiative provides free
social-emotional mental health screenings of
children up to age 8 in Cass and Clay Counties
at pediatric clinics, childcares and other FargoMoorhead sites. More than 4,200 children have
been screened since 2009.
This early intervention has led to 353 referrals for
mental health services to build positive behavioral
skills preventing future difficulties in school, work
and relationships.
Prescription
Assistance
Initiative
developing emotionally healthy children
organized by the initiative in November 2012 and
led by Dr. Terrie Rose, a child psychologist and
nationally-recognized innovator and speaker on
early childhood development and mental health.
This initiative managed by our partner, Region V
Children’s Services Coordinating Committee, also
trains child care providers in the Nurtured Heart
approach to positive behavior reinforcement, with
180 providers prepared in healthy ways to support
developing kids.
Hundreds of parents and health professionals
attended a community education event on
Prescription Assistance Initiative
helps area people with low
incomes secure free and
low-cost medicine from
pharmaceutical companies.
Over 25,000 applications
for medications have been
processed for about 5,000 people, resulting
in procurement of more than $14.3 million of
essential prescription drugs since this initiative
began in 2003.
In fiscal 2012, over 600 clients accessed 5,500
healing and life-sustaining prescriptions that
would otherwise have been out of financial reach.
With a value of $4.5 million, this program
generated a $42 return for every dollar invested.
Our partner, South Central Adult Services of
Valley City, N.D., manages this initiative.
Healthy Steps
Initiative
Dakota Medical Foundation’s Healthy Steps
Initiative provides education and outreach to
families of over 13,000 uninsured children in
North Dakota, seeking to connect them to one
of three free or low-cost assistance programs:
Medicaid, Healthy Steps Children’s Health
Insurance Plan and Caring for Children. There
are 43,470 children enrolled in all three insurance
assistance programs.
Studies show that uninsured children are ten
times more likely to have unmet medical needs
and five times more likely to go more than two
years without seeing a doctor.
The initiative is funded by a $650,000 grant from
the North Dakota Department of Human Services
and $80,000 from Dakota Medical Foundation.
Over the past three years, the Healthy Steps Children’s Health Insurance Plan
has provided coverage to 24% more children who qualify.

AED Initiative Completed
Our AED Initiative placed life-saving heartstarting automated external defibrillators in
rural ambulances, police cars, fire trucks
and community locations in our service area.
A documented 25 lives were saved as a result!
A total 548 AEDs were placed through
Dakota Medical Foundation’s investment
of $1.3 million since 2000. With this oncepioneering equipment now widely-accepted
and placed, others in the community are
prepared to teach and champion AED use.
We completed our program funding in 2012.

Organizational
Effectiveness
Initiative
We’re building exceptional fundraising and
leadership skills among nonprofit leaders –
together with powerful tools for charity success.
Dakota Medical Foundation led the formation
of Impact Foundation and its Impact Institute
to build the capacity of nonprofits to produce
superior results in health. Expanding their fundraising capabilities creates an ability to develop
funding sources from individual donors, far
exceeding what DMF could otherwise provide.
The Institute provides a ladder upon which
aspiring organizations can rise.
With abilities to build self-sustaining capital,
charity leaders and boards create stronger
organizations better prepared to serve people
in their communities. Some 4,000 nonprofit
executives, staff and directors from more than
350 nonprofits have attended Institute professional
development courses.
In 2012, we offered training on fundraising,
personal productivity, hiring practices and other
topics to 804 attendees. Many continued their
professional development in the fundraising
curriculum developed by FundingLogic with
its founders coaching use of this well-proven

‘intensives’ were held for small groups to build
mastery of fundraising and leadership areas.
The new Dakota Medical Foundation building
will have a classroom with seating for 150 and
vastly expand the offerings of the Impact Institute.
More than 80 nonprofit board members attended
our High Impact board training to learn about their
critical role in nonprofit performance in producing
dramatically better results for the people they serve.
DMF continues to support the powerful online
resource used by more than 450 regional
charities: the impactgiveback.org website.
Since 2007, $5.5 million has been raised
online at this site.
Cyber giving is rising at a pace equal to online
shopping in America, so online donations are a
must today. We fund the continual development
and coding of new functions for the website so
that charities have an affordable, feature-rich site
for profiling their organizations, taking donations,
posting volunteer needs and taking online gala
and event registrations.
In our study of a sample 22 nonprofits who participated in training
from 2007 to 2010, fundraising increased 16% per year, surpassing
our goal of 10% annual growth.

Breakthrough
Idea Challenge
fosters innovations for health
Dakota Medical Foundation is investing in the
brightest new health ideas to accelerate breakthrough change.
The nonprofit sector seldom has dollars
available for risk-taking and experimentation
with entrepreneurial ideas. Yet innovation is the
most likely path to produce breakthroughs that
conquer persistent health problems. We engage
and challenge people and organizations to think
bigger and bolder, and then supply the grant
capital and guidance to explore new ideas and
launch new ventures to tackle tough issues.
In early 2012, as we celebrated our 50th
Anniversary, we launched our Breakthrough Idea
Challenge with the same spirit of our visionary
founders. We set aside $500,000 to invest in
ambitious, creative new ideas to remarkably
improve the health of people in our region – or
remove difficult hurdles that impact health
and well-being. We received submissions from
nonprofits, health organizations, medical experts
and people of all walks of life.

Dakota Medical Foundation will continue to seek
fresh thinking and concepts on the frontiers of
these focus areas: ideas to see that all people are
able to obtain needed healthcare or medicine,
particularly kids; recruiting and keeping welltrained physicians, nurses and other healthcare
workers in rural areas; ways to vastly improve the
impact of organizations producing healthy results
for people of our region; and fitness, nutrition and
healthy lifestyle innovations.

Center for
collaborative teams
to shape a healthier future
In March 2013, Dakota Medical Foundation will
open the doors of its new conference and meeting center at 4141 S 28th Avenue in south Fargo.
“Great ideas come from convening great minds.
Visionary thinking and powerful motivation will
be inspired at retreats, conferences and health
leader gatherings at our new building,” comments
Dakota Medical Foundation President Pat Traynor.
“Courageous thinking will help solve persistent,
complex health problems we face – and help our
region attain the greatest quality of life.”
The building will be a high-tech conference and
meeting center for engaging health leaders and
other stakeholders. It will become a center for
the Impact Institute to sharpen nonprofit leadership skills so that they can have exceptional
impact. The Impact Institute helps nonprofits
forge new strategies, develop peer networks and
create organizations that better understand, meet
and exceed the needs of their communities.
An enthusiastic ground breaking celebration was
held April 30, 2012, with directors, DMF founders,
city officials, Chamber representatives, nonprofit
directors, staff and other DMF friends. Our close
partner Alex Stern Family Foundation committed
to funding our boardroom, and Essentia Health
donated the teaching kitchen in this facility.

• Best practices, proven strategies and new
ideas can come to the table for solving major
issues affecting our communities
• Space for donor meetings with nonprofits and
Dakota Medical Foundation experts in strategic
giving so that donors can better navigate area
nonprofits, social causes and breaking new
ideas to adopt high-impact giving.
This new gathering site creates space for idea-sharing to transcend old ways of thinking and achieve
results through innovation, risk-taking and revolutionary problem-solving. It will foster more partnerships, more dialog and improved capacity to deliver results.

Family HealthCare
match grant success
Family HealthCare in downtown Fargo began in
1990 with a health care site for the homeless at
the Salvation Army. Now with more than 40,000
patient visits, it serves more than 12,000 people
each year, primarily a low-income, culturally
diverse population.
In late 2012, Family HealthCare opened the
doors to a new $15 million clinic in a renovated
historic building at 301 NP Avenue. Its new
state-of-the-art facility provides excellent primary
care and dental services for adults and children,
lifestyle medicine and homeless healthcare
without barriers imposed by language or ability
to pay.
Family HealthCare features 30 medical exam
rooms, 7 dental chairs, a demonstration kitchen,
wellness center and pharmacy that dispenses
nearly 80,000 prescriptions each year, including
over $3.5 million in free and discounted
medicines. Dakota Medical Foundation is proud
to have provided technical assistance and
invested $250,000 in match funding to kick
off the building campaign that made this facility
possible. Since 1996, DMF has invested over
$1.6 million in building Family HealthCare’s growing
capacity to serve.
Family HealthCare is the only community
health center within 100 miles and the only
federally-funded homeless health program in
North Dakota and western Minnesota.
We congratulate the leaders and staff of Family
HealthCare for their bold vision and determined
spirit! We thank the many donors who have
joined in making this tremendous community
asset possible.
Family HealthCare secured over $13 million in federal, city and local foundation grants, tax credits and private contributions.
Our $250,000 in match funding kicked off local fundraising for the $1.6 million needed from community donors to build this
valuable asset for the health of area people.

Together, Dakota Medical Foundation and
Impact Foundation organize and promote a
powerful Giving Hearts Day to invigorate
community charitable giving and create an
influx of donations for regional charitable
organizations through a one-day give-a-thon.
We hosted the first Giving Hearts Day in
2008, and charities raised $325,000 online
at impactgiveback.org. Donations topped
the million-dollar mark in 2011 and we
were ecstatic when they soared a staggering
40 percent in 2012: on February 14, gifts
totaling $1.6 million in over 10,850 individual

partner with DMF in financially supporting important health-related programs in our region. All funds awarded become

giving, while DMF manages donations to
qualified charitable organizations, tax and
legal responsibilities and other oversight.
A charitable fund can be established with
a tax-deductible, irrevocable contribution
of $20,000 or more. At this time, it is the
Foundation’s practice not to charge
expenses to administer these types of
funds other than the direct costs of external
investment management services. We

For the 12-month period ended 9/30/2012
The Foundation’s market value of its portfolio of cash and investments at September 30, 2012,
increased to $91,032,016 from the September 30, 2011, balance of $85,051,373. The net change
includes the effect of expenditures for grants, initiatives and other expenditures totaling $4,166,790.
The Foundation’s composite return for the twelve months ended September 30, 2012, was a positive
12.1% compared with the prior year’s positive return of 3.4%.
The improved return for 2012 is the result of improved market conditions and changes implemented
to the portfolio structure in the first quarter of 2011 allowing for a more dynamic management of
portfolio investments by the investment manager, permitting a quicker response to volatility in rapidlychanging market conditions.
The Foundation’s board approved the construction of a new office building in 2011. The Foundation
obtained a 10-year $4 million tax-exempt bond issue and committed approximately $1 million as
an equity contribution to finance the building construction and equipment. Construction began in
April 2012 and is expected to be complete by March 2013.
