MISSION STATEMENT
MINNESOTA VISITING NURSE AGENCY (MVNA)
The mission of the MVNA is to provide comprehensive and culturally competent community
health and related services in collaboration with public health and health service providers
to ensure a healthy future for Minnesota residents.
MVNA is a nonprofit public health care provider in the Twin Cities metro area. Since 1902,
MVNA provides nurse home visiting services to pregnant women, infants, children, adults
and older adults. MVNA is licensed by the State of Minnesota, certified by Medicare and
accredited nationally by CHAP (Community Health Accreditation Program).
HOSPICE OF THE TWIN CITIES (HOTC)
HOTC’s mission is to enhance the quality of the lives of our patients and their families
by providing respectful care based on maintaining dignity, alleviating physical,
psychosocial and spiritual suffering, advocating for the fundamental rights and affirming
the sacred value of life.
HOTC is a nonprofit, Medicare certified, state-licensed hospice program, Community Health
Accreditation Program (CHAP) and a wholly owned subsidiary of MVNA. HOTC provides
care that focuses on the person, not the disease, empowering individuals to live with dignity,
while remaining alert and pain free. HOTC care is about living life to the fullest in secure
and familiar surroundings with those who matter the most.
FROM THE CEO
Elisa* had her first child at 14. A family health nurse became involved during the
pregnancy. Elisa experienced depression, poor relationships, and lived in substandard
housing. She shared with her nurse that she wanted to graduate from high school, but
didn’t think she could. The nurse worked with her on parenting and basic life skills. She
helped her to address barriers, focus on the needs of her child and herself and to set
goals. Elisa benefited from the nurse’s mentored support, graduated from high school
this year, and plans to attend college this fall.
Collaborative Leadership
MVNA/HOTC senior leaders visited U of M beehives,
led by Dr. Marla Spivak, MacArthur Fellow and
Distinguished McKnight Professor in Entomology, to
observe the impact of collaborative leadership as
represented by the insect community. This visit was
broadcasted on NPR, The Promised Land, hosted by
Majorca Carter in September 2011.
At MVNA and HOTC, improvement in life situations is expected. In 2011, nearly 16,000
individuals were served through home visiting, all with the goal of making their lives
better. Individuals and families received nursing
care, rehab and interpretation services, education
to improve their parenting and life situations, and endof-life care. Our efforts were acknowledged locally and
nationally through the National Public Radio,
Minnesota Public Radio, Prairie Home Companion,
Star Tribune, KARE 11 and KSTP-TV.
Unfortunately, reimbursement for our programs does
not cover the costs. Community support and generosity
fills the gap. As I completed my first year in Minnesota,
I continue to be humbled by the work we all do; donors,
volunteers, staff and board members as together our
efforts makes a difference in this community. On behalf
of MVNA and HOTC, we are grateful to have your

Rob Grunewald MVNA Board Chair
As an economist at the Federal Reserve Bank of Minneapolis I have conducted research on
the economic benefits of investing in young children. Studies show that programs that support
maternal and child health, such as the nurse-based home visiting models delivered by MVNA,
have a strong return on investment to society. Mothers are more likely to graduate from high
school and find employment, while children are more likely to arrive at kindergarten prepared
to succeed in school.
I became interested in the work of MVNA and went on a couple home visits, which further
illuminated the value of nurse-based home visiting. After joining the board I have learned more
about MVNA’s work in home health, corporate wellness and HOTC’s hospice services, all
providing innovative and essential services to the community.
I thoroughly support the mission of MVNA/HOTC and staff and volunteers who make it
all happen.

MVNA and HOTC address issues that are very important to me–end of life care and health
care for people experiencing homelessness. Care of dying people has been a heartfelt
commitment since I became a nurse.
I have witnessed excellent care, satisfied patients, families, and professionals, and an
organization that remains on the forefront of creativity and innovation. I remain on the board
because I am proud to be a part of the best hospice organization I have known.
In my time at the U of M Center for Bioethics I had an opportunity to come to know people
who are homeless; being a part of the HOTC/MVNA boards makes me more confident that
we can assist homeless and disenfranchised people with health care issues throughout their
life spans.

HOSPICE
Hospice of the Twin Cities (HOTC) is committed to enhancing the quality of life of each
individual served. Through a team of health care professionals, hospice provides care
and services to meet the unique needs of those living with life-limiting illness including
expert medical care, symptom management, emotional and spiritual support. Through
an interdisciplinary approach to care, the HOTC team works to meet the specific needs
of the patient. Hospice also provides support for the patient’s loved ones.

Services Provided Wherever the Patient Calls Home
Hospice services are delivered to the patient wherever they call home. This may be their
own private residence, the home of a family member or loved one, a nursing home, board
and care center, assisted living facility, skilled nursing facility or residential hospice. In 2011,
HOTC patients resided throughout the metro area including Anoka, Carver, Chisago,
Dakota, Hennepin, Isanti, Le Sueur, Ramsey, Scott, Sherburne, Sibley, Washington and
Wright counties.
More Hospice Patients Over 85 Now Being Served
HOTC served a high percentage of patients over age 85 totaling 42.6% which is higher than
the national average of 38.9%.

Therapy Services
Sharon Alery has been a music therapist for 28 years and is currently employed by Hospice
of the Twin Cities and working toward a master's in music therapy. She does her job for
HOTC with voice, guitar, and piano—and two therapy dogs named Franky and Bella. Sharon
performed on the Prairie Home Companion Show in fall of 2011.

Bereavement support is provided for at least 13 months following the death of a loved one
for all hospice families and in 2011, 616 individuals received this service. This support
includes: regularly scheduled mailings related to grief and support services; newsletters;
phone calls to survivors; personal visits; educational opportunities related to grief and loss;
and Services of Remembrance.

HOME HEALTH
Home Health provides home visits to adults with acute and chronic
medical conditions including heart disease, diabetes, HIV/AIDS,
cancer, chronic obstructed pulmonary disease (COPD), dementia,
individuals requiring IV therapy and care for wounds, as well as
people struggling with chemical dependency and mental health issues.

MVNA nurses stress a holistic approach to
health in caring for people who are ill or
disabled, with the goal of helping people
establish or maintain their self-sufficiency
and continue to live independently for as long
as possible. About 90 percent of clients
served have multiple chronic diseases and
often lack a caregiver. MVNA nurses and
home health aides form a care team and if
needed connect clients to additional services
such as food or housing assistance.

HOME HEALTH
Reducing Emergency Room Visits and Repeat Hospitalizations
Home Health care is a critical element of the health care system, as it helps to control the
overall cost of health care, keeping patients out of expensive acute care hospitals, and
reducing the need for patients to reside in long-term care facilities. To keep uninsured and
underinsured patients healthy and costs low, MVNA endeavors to be a model of costeffective health care and public health information delivery.
In a 2011 pilot project which included Hennepin County Medical Center and MVNA Home
Health staff, care coordination was focused on reduction of hospital readmissions and
emergency room utilization for low income, medically complex patients. This resulted in a
40% decrease in hospital readmission and a 30% decrease in emergency room visits. This
reduction in readmissions and emergency usage was due to the collaborative partners,
improved disease management and implementation of a coordinated system of care that
integrates hospital, clinic and home health care services.
Challenges Met by MVNA Home Health Staff
In the past year patients discharged by hospitals to MVNA trended towards more complex
medical and socio economic situations:
• Prevalence of mental health behavior in addition to multiple co-morbidities
• Live in homeless shelters
• Lack of primary care physician
• Treatment for alcohol, drug incidents and frostbite
MVNA home health staff continues to strive to assure safe and effective care to those
experiencing destabilizing challenges.

FAMILY HEALTH
Family Health provides family centered and community-based public health nursing
services to clients from diverse racial, ethnic and socio-economic backgrounds. Families
receive services through a number of programs that have specific goals and expected
outcomes for selected client populations.
MVNA served 5,264 clients
Nurses have access to clients’ lives through home visits, community outreach, clinics and
school based visits. Clients welcome the support and care from the nurses because over time
a trusting relationship is established. Nurses provide services within the client’s environment–
typically the home setting. The public health nursing focus is on prevention to promote healthy
birth outcomes, positive parent-child interaction, healthy child growth and development,
family self-sufficiency and prevent complications of illness and chronic disease conditions.

Pregnant & Parenting Women & Families
MVNA provides nurse home visits to first time pregnant women and teens living in poverty. The
public health nurse has unique access to provide education and support to the teen in her home
environment. This is a long-standing, one-to-one relationship, spanning from pregnancy until
the child is two years old, and until the young mother graduates from high school. The
relationship between the teen and the nurse coincides with research, which has shown that if
children and their families are supported in early, healthy development, they are more likely to
succeed later in school and in the community. MVNA works with parenting families in many
stages, focusing on identification of developmental delays and care for children with chronic
conditions.

FAMILY HEALTH
A Nursing Visit to a Teen Mother
MVNA is one organization among many around the state trying to help families through
distressing times. One of its public health nurses (PHNs), Jeanne Kumlin, paid a visit to 19-yearold Garmai, who shares a crowded Brooklyn Center apartment with her six-week old son, along
with her sister’s family. Kumlin was accompanied on her visit by MPR Reporter, Laura Yuen.
The reporter noted that Kumlin is vigilant for any signs of slow development, because babies
born into poverty and chaos can have a toughter time adapting to school later in life.
“Even though they’re living in poverty, I know Garmai is going to raise Joshua with a lot of love
and affection,” Kumlin said. “So he’ll be fine. It’s the babies who don’t have this tender loving
care that we all need, that will suffer down the road.”

Caring for Children with Illness or Chronic Disease
Nurses provide home visits for children with illnesses or chronic diseases such as asthma and
lead poisoning. The focus is care for the child, however, the nurse also makes sure other family
needs are well supported, such as access to food, transportation and housing.

The MVNA public school nurse program–one of the first in the country–was established in
1908, with funds raised by the Woman’s Club of Minneapolis. A century ago, it was
recognized that a healthy school environment allowed successful learning. Today MVNA
continues to support children’s learning in child care and school settings. The nurse
consultants work with 228 child care centers and 39 schools to ensure healthy, safe
conditions which supports academic success.

When first seen by MVNA Nurse Louise*, Graciela*
was very depressed but able to express her goal of
completing high school. Louise worked with Graciela
on getting treatment by helping her to recognize that
depression was preventing attainment of her goal.
After Graciela enrolled in school, the MVNA nurse
continued to support her by providing resources and
parenting support, as she had no family. During her
senior year, Graciela went to school from 8am-4pm
and then worked from 5pm-11pm to support her
child. She graduated, received a scholarship, and

WELLNESS
Worksite and Community Flu Clinics
Every year over 1,000 employers host a worksite flu vaccine clinic staffed by MVNA nurses. Additionally, MVNA hosts
nurse-staffed public flu clinics with community partners such as Health Fair 11 at the State Fair, Kohl’s, Lunds Byerly’s,
Children’s Hospital and civic organizations. Specialized community clinics hosted by MVNA are tailored to vaccinate
children ages six months and up. Because children are two to three times more likely to get the flu, in-school vaccination
clinics by MVNA support a healthier school environment. Since MVNA is able to bill many insurance providers, this is an
accessible service to the community.
Health Screenings
92,094 flu vaccines administered
MVNA Worksite Health Screenings Services are nurse led, responding to the goals of employers to guide and inform employees about their health. MVNA nurses meet one-on-one with participants to provide baseline screenings such as cholesterol, Body Mass Index (BMI), or other health indicators. Nurses discuss key lab results, necessary dietary and health
related changes in their lives.

wellness clinics have been well received by area
employers and their employees. 97% of MVNA
Wellness Services clients rated themselves “very
satisfied” with services (MVNA Wellness
Services 2011 Customer Satisfaction Survey).

COMMUNITY OF CARING
Donors Make A Difference
Because the majority of MVNA clients live below the poverty level, there is a need to provide
non-medical items, with donated funds by the community, to help overcome temporary barriers.
MVNA nurses initiate the request for modest, pragmatic items. In turn, the program’s donors
have the unique opportunity for direct philanthropy, as donor contributions–whether the
item is a handmade blanket, crib, diapers or financial support–have an immediate transformative
impact on the clients we serve. Needed material goods are identified and brought to the family
by the nurse to enhance health, safety and self-sufficiency. This process bridges the gap in the
lives of our clients as they move toward health and independence.
6,100 requests from nurses for items to
assist clients, a 41% increase
The donor community includes congregations, community
groups, individuals and 162 employee groups from 25
corporations. Many of the in-kind donations are hand-made
quilts, breastfeeding pillows, and baby sweaters. Group
donors collected items and assembled over 3,000 kits to
address non-medical needs. These are supportive teaching
tools for our nurses. Kits include “Rub-A-Dub-Dub” baby’s
first bath kit which includes all the items a nurse needs to
teach a mother safe bathing, such as a towel, bath water
temperature indicator, wash cloth, and baby wash.
Funds were raised to support the purchase and distribution
of 300 portable cribs to parenting clients. MVNA nurses
focus on a safe sleep environment and the provision of
a crib ensures sweet dreams and safe sleep. With the
community’s financial support, MVNA has been providing
about 300 cribs a year for the past 10 years.

COMMUNITY OF CARING
Reading Begins At Home™
MVNA’s literacy program, Reading Begins At Home ™ began in November 2010 and has served
a total of 1,442 individuals with board books–increasing access to books in client homes. The
nurses recognized a critical need to bring books into the lives of our clients, as almost all the
households we serve do not have any books. Through this program the nurses provide about
10 new donated books and inform parents about accessing library resources. The Hennepin
County Library system partnered with MVNA to promote the development of early literacy skills for
young clients by providing books and music along with educational support for nurses. In addition
new books or financial support were received by The Woman’s Club of Minneapolis, Minneapolis
Rotary 9, Altursa Women-St. Paul, Kenwood School parents, and Birchbark Books.
SEASON OF SPARKLE: LIGHTS AND LIMOS AND GIVING TREE
Lights and Limos
For the last four years, Hospice of the Twin Cities volunteers have been organizing limousine
rides for hospice patients and their families to view holiday lights as a way to celebrate their life
together. In 2011, 27 families were served with a total of 221 individuals participating creating
lasting memories for family members and patients alike.

Holiday Giving Tree was generously supported by over 900 individuals from 50 groups. The
community generosity brightened the holiday for 470 children and older adults.
