Tuk-A-Way Paintball Permission Slip

RISK AWARENESS DISCLOSURE
1. Paintball is physically and mentally intense and may require extreme
exertion to play and the possibility of injury to myself and/or others does
exist. I understand that paintballs can and do leave welts, bruises and
occasionally can break the skin.
2. Paintball can be dangerous if not played in accordance with the stated
rules and the wearing of proper safety gear. Rules will be provided to the
participant prior to entering the play field and the minimum safety equipment
provided by Tuk-A-Way must be worn at all times on the play field. Personal
safety equipment is allowed but must at a minimum, cover the full face
including the ears. Failure to abide by the rules and the proper wearing of
the safety equipment will result in immediate expulsion from the playing
field with no refund of any fees paid.
3. All Tuk-A-Way Paintball Markers are adjusted to fire at no more than 280
fps before the beginning of each period of play. Personal Markers that are
used must be adjusted to this requirement to be eligible for use. No
adjustment tools are allowed on the field.
4. All participants must use Paintballs provided by Camp Tuk-A-Way.
CONFIRMATION AND AGREEMENT
I confirm and agree that I am fully aware of the risk I and that I am
physically and mentally able to be fully involved in the activities and will
comply with all rules, regulations, and verbal orders of the field referees.
RELEASE:
I (or my parent or designated guardian if under age 18) hereby release,
remise and forever discharge from any claim and liabilities whatsoever
without limitation that I might I might have against Tuk-A-Way Paintball and
their staff, indemnifying them against any and all claims, actions, suits,
procedures, costs, expenses, (including attorney fees and expenses), damages
and liabilities arising out of, connected with, or resulting from my playing
paintball. I understand that this release shall be binding upon my estate,
my heirs, my representatives, and assigns. I hereby certify that I am
physically and mentally capable of participating in this activity.
ASSUMPTION OF RISK:
I confirm that I have specific insurance to cover any injury that I may
sustain or cause to others and I have read and fully understand the terms of
this agreement. This is fully intended to be a legally binding contract for
one year. If I have any doubts concerning any aspect of its contents, I will
consult an attorney before signing it.
