A community–based initiative seeking to
make positive changes in the health status
of Calumet County. The community chose
two health behaviors to focus on:
a. UCAN (Calumet Activity and Nutrition
Coalition) a community group focusing
on Physical Activity and Nutrition
programs within the county.
b. REACH (Reducing Excessive Alcohol
Consumption for Health) a community
group focusing on decreasing excessive
alcohol consumption.

1. Immunization Clinics
a. Immunizations are available for both
children and adults by appointment.
b. Calumet County Health Division offers
childhood vaccines at no charge to children
up to 19 years of age who are in one of the
following categories:
1. Have no health insurance
2. Are covered by Medical Assistance
(Forward Health Care/Badger Care)
3. Whose health Insurance does not cover
the needed vaccines
4. Who are American Indian or Alaska
Native
2. Parenting Education
a. Offered to the parent on an individual
basis.
b. Call for information.
3. WIC Program (Women - Infants - Children)
a. Food supplement program for pregnant
women, infants and children through age

Health and financial needs determine
eligibility.
4. Car Seat Safety
a. Certified technicians provide safety
education.

Free car seat safety checks are
available by appointment, call
