Every applicant to The Hair Nail & Skin Institute must meet the following requirements for admission into our programs:

A completed student application form.

Minimum of 16 years of age.

A copy of high school diploma, GED, transcripts with a minimum of 10 credits ( two credits in English), or proof of 18 years of age or
older.

Valid Picture ID (Drivers license, State ID, Military ID, or Passport).

Social Security Card.

Arizona. Drivers License, Birth certificate or passport for proof of citizenship.

Attendance
Cosmetology Students are scheduled to attend Tuesday thru Saturday from 8:30 A.M. to 4:00 P.M. (35.0 hours per week) with 30 minutes for
lunch daily. Aesthetics students are scheduled Tuesday thru Saturday from 8:30 A.M. to 4:00 P.M. (35.0 hours per week) with 30 minutes for lunch
daily, not included in clock hours. All students are expected to comply with the schedule and attend theory class as required. Absent or tardy students
must notify the Director at 928-910-1002 between the hours of 8:00-8:30am. If the phone isn’t answered promptly you will need to send a text or
leave a voicemail for a returned call. A No CALL/NO SHOW will not be tolerated by the Institute and may result in a 1-3 day suspension. All absences
and tardiness for scheduled instruction are recorded by the time clock. If students are late for theory, they will not be able to clock in until 10:00 A.M.
If absences or tardiness are excessive and affect satisfactory progress, the student will be counseled. A record of attendance is maintained by time
clock records for each student and is available for review by authorized persons.
Start Dates
Classes will be scheduled to start at 5 week intervals for Cosmetology and Aesthetics, and by special arrangement for Cosmetology Student
Instructor. Modified Schedules may be approved on a case by case basis. The Institute reserves the right to reschedule, postpone, or cancel classes.
Holidays and School Closings
The

In the event of severe weather or snowfall, students should call the School attendance number regarding school closure or a delayed schedule. The
Institute reserves the right to close additional days with posted notice. School closures will result in an extension of the contract’s end date.
Make-up Work
Absent or tardy students are required to make-up all tests and required operations. If a student is tardy or absent the day a test is scheduled,
the student must make-up the test the day they return. Students may not take tests if required homework is not handed in prior to test.
ACADEMIC PROGRESS EVALUATIONS
Students must maintain satisfactory progress in attendance and academic work. The qualitative element to determine academic progress is a
reasonable system of grades as determined by assigned academic learning. Students are assigned academic learning and practical procedures.
Academic learning is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion only
when rated as satisfactory or better. If the performance does not meet satisfactory requirements, it is not counted and the performance must be
repeated. At least two comprehensive practical skills evaluations will be conducted during the course of study. Practical skills are evaluated according
to text procedures and set forth in practical skills evaluation criteria adopted by the School. Students must maintain a written grade average of 75%
and pass a Final Written and Final Practical exam prior to graduation. Students must make up failed or missed tests and incomplete assignments.
Numerical grades are considered according to the following scale:

Satisfactory Academic Progress
Failure to maintain satisfactory progress can cause delay in graduation, and additional tuition charges. The Satisfactory Academic Progress
Policy is consistently applied to all students enrolled at the School. It is printed in the catalog to ensure that all students receive a copy prior to the first
day of class. The policy complies with the guidelines established by the National Accrediting Commission of Career Arts and Sciences (NACCAS) and the
federal regulations established by the United States Department of Education.
EVALUATION PERIODS
Students are evaluated for Satisfactory Academic Progress as follows:
Cosmetology: 450, 900, 1350 clocked (actual) hours
Aesthetics: 300, 600 clocked (actual) hours
Evaluations will determine if the student has met the minimum requirements for satisfactory academic progress. The frequency of evaluations
ensures that students have ample opportunity to meet both the attendance and academic progress requirements of at least one evaluation by midpoint
in the course.

ATTENDANCE PROGRESS EVALUATIONS
Students are required to attend a minimum of 75% of the hours possible based on the applicable attendance schedule in order to be
considered maintaining satisfactory attendance progress. Evaluations are conducted at the end of each evaluation period to determine if the student
has met the minimum requirements. The attendance percentage is determined by dividing the total hours accrued by the total number of hours
scheduled. At the end of each evaluation period, the school will determine if the student has maintained at least 75% cumulative attendance since the
beginning of the course which indicates that, given the same attendance rate, the student will graduate within the maximum time frame allowed.
MAXIMUM TIME FRAME
The maximum time (which does not exceed 150% of course length) allowed for each course is:
Cosmetology: (Full time, 35.0 hours/week--1600 hours total) 2000 hours maximum time allowed
Aesthetics: (Full time, 35.0 hours/week--800 hours total) 1000 hours maximum time allowed
The maximum time allowed for transfer students who need less that the full course requirements will be determined based on 75% of the
scheduled hours for the course.
DETERMINATION OF PROGRESS STATUS
Students meeting the minimum requirements for academics and attendance at the evaluation point are considered to be making Satisfactory
Academic Progress until the next scheduled evaluation. Students will receive a hard copy of their Satisfactory Academic Progress Determination at the
time of each of the evaluations. Students who fail to meet minimum requirements for attendance or academic progress are placed on WARNING and
considered to be making satisfactory academic progress while during the warning period. The student will be advised in writing on the actions required
to attain Satisfactory Academic Progress at the next evaluation. If at the end of the warning period, the student has still not met both the attendance
and academic requirements they may be placed on probation and, if applicable, students may be deemed ineligible to receive Title IV funds.
PROBATION
Students who fail to meet minimum requirements for attendance or academic progress after the warning period will be placed on probation
and considered to be making satisfactory academic progress when in the first probationary period, if the student appeals the decision, and prevails upon
appeal. Additionally, only students who have the ability to meet the Satisfactory Academic Progress Policy standards by the end of the evaluation period
may be placed on probation. Students placed on an academic plan must be able to meet requirements set forth in the academic plan by the end of the
next evaluation period. Students who are progressing according to their specific academic plan will be considered making Satisfactory Academic
Progress. The student will be advised in writing of the actions required to attain satisfactory academic progress by the next evaluation. If at the end of
the probationary period, the student has still not met both the attendance and academic requirements required for satisfactory academic progress or by
the academic plan, they will be determined AS NOT making satisfactory academic progress and, if applicable, students will not be deemed eligible to
receive Title IV funds, if applicable.
RE-ESTABLISHMENT OF SATISFACTORY ACADEMIC PROGRESS
Students may re-establish Satisfactory Academic Progress and Title IV aid, if applicable, by meeting minimum attendance and academic
requirements by the end of the probationary period.
INTERRUPTIONS, COURSE INCOMPLETES, WITHDRAWALS
If enrollment is temporarily interrupted for a Leave of Absence, the student will return to school in the same progress status as prior to the
Leave of Absence. Hours elapsed during a Leave of Absence will extend the student's contract period by the same number of days taken in the Leave
of Absence and will not be included in the student's cumulative attendance percentage calculation. Students who withdraw prior to completion of the
course and wish to re-enroll will return in the same satisfactory academic progress status as at the time of withdrawal.
APPEAL PROCEDURE
If a student is determined to not be making Satisfactory Academic Progress, the student may appeal the determination within ten calendar
days. Reasons for which students may appeal a negative progress determination include death of a relative, an injury, or illness of the student, or any
other allowable special or mitigating circumstances. The student must submit a written appeal to the School on the designated form with supporting
documentation of the reasons why the determination should be reversed. This information should include what has changed about the student's
situation that will allow them to achieve Satisfactory Academic Progress by the next evaluation point. Appeal documents will be reviewed and a decision
will be made and reported to the student within 30 calendar days. The appeal and decision documents will be retained in the student's file. If the
student prevails upon appeal, the satisfactory academic progress determination will be reversed and Title IV funds will be reinstated, if applicable.
NON-CREDIT AND REMEDIAL COURSES
Non-credit and remedial courses do not apply to this School. Therefore, these items have no effect upon the School's SAP standards.
TRANSFER HOURS
With regard to Satisfactory Academic Progress, a student's transfer hours will be counted as both attempted and earned hours.
Job Placement Policy
While the School cannot guarantee employment for graduates, assistance in finding suitable employment is provided by posting area job
openings on a career opportunities bulletin board for students to review. Students also receive the Nuts and Bolts Business Training course that
provides training in professionalism and job search skills including how to write a resume, complete an employment application and prepare for an
effective interview. The curriculum places a great deal of emphasis on how to obtain and retain employment after graduation. Graduates are
encouraged to maintain contact with the School and follow-up with the School on current employment or special employment needs of the Student. In
addition, the School maintains a network of relationships with local professionals and employers who provide mentoring to students while they are in
school.

Disciplinary Policy
The Institute's Administration reserves the right, at its sole discretion, to suspend or terminate any Student for insubordination, refusal to
cooperate with Faculty or School Administration, inability to follow instructions or schedules, failing to observe the Grievance Policy and Procedure,
failure to observe the Standards of Conduct, or in any situation where the Administration feels that the Student’s continued attendance jeopardizes the
learning environment and educational mission of the Institute. Disruption of an Educational Institution is a crime in the State of Arizona.
Any infraction of the Standards of Conduct, General Policies, or the Enrollment Agreement could result in any of the following disciplinary
actions:
PROBATION: A Student may be placed on probation for a specified time for violating the Standards of Conduct or General Policies or Rules of the
Institute. If the Student does not correct the problem, he/she will be placed on a second probation. If the problem is not corrected during the second
probationary period, he/she may be suspended for three days or dismissed permanently at the discretion of the School Administration.
SUSPENSION: Enrollment may be immediately suspended at the discretion of School Administration for up to three days for violation of the Standards
of Conduct or General Policies. Students may receive in-house suspension at the discretion of the Administration.
DISMISSAL – TERMINATION: Enrollment may be terminated at the discretion of School Administration for any reason deemed necessary to maintain
the positive educational environment and general objectives of the institution for any of the following reasons:








Non-conformance with the Standards of Conduct, General Policies, or Enrollment Agreement.
Non-conformance with the state laws and regulations governing schools and students.
Non-compliance with the school’s Satisfactory Academic Progress Policy.
An action which causes or could cause bodily harm to a client, student, or employee of the school.
Engaging in the manufacture, distribution, dispensation, possession, or use of drugs.
Being under the influence of drugs or alcohol while on the premises.
Immoral or improper conduct.
Theft or Willful destruction of school or student property.
Disciplinary decisions may be appealed by submitting a clear and concise written statement containing all pertinent data and facts. The appeal will be
reviewed by the school administration and the student will be notified of the decision within 30 days of the receipt of the appeal.
Student Grievance Policy & Procedure
The Hair Nail & Skin Institute will make every effort to resolve any student grievance that is not frivolous or without merit. The student must
first try to resolve the problem though the Institute’s Student Grievance Policy and Procedure. The student must register the grievance in writing to the
attention of the Institute’s Administration within 30 days of the date that the act which is the subject of the grievance occurred. Depending on the
extent and nature of the complaint, interviews with appropriate staff and/or other students may be necessary to reach a final resolution of the
complaint. The grievance will be reviewed by the Administration and a response will be sent to student in writing within 15 days of receiving the
grievance. The initial response may not provide for final resolution of the matter, but will notify the student of continued investigation and actions
taken. Should the student find the response to be unsatisfactory, the student may appeal the decision by contacting the Arizona State Board of
Cosmetology at 1721 East Broadway, Tempe, Az. 85282-1611. Phone (480-784-4539).
Dress Code
Students are required to be in dress code at all times. Students must arrive in the morning ready for the day. Hair must be styled, makeup
and nails to be done. Any student not in dress code will be sent home to change, and is expected to return in proper attire in a timely fashion.

Clothing must be solid black free of print, emblems or logos. This includes dots, stripes, lace, flowers etc. This includes pants, skirts shirts,
dresses, under shirts, undergarments, outerwear, and stockings. Denim is not permitted at any time. (no color is permitted at any time)
Aesthetics students will be provided a black smock, and this must be worn at all times.
Shirts must have sleeves that completely cover the under arm area. Cap sleeves are not permitted and are a reason to be sent home.
Shoes and accessories ie. Jewelry, belts may be of any color. Shoes must have complete closed toe and back heel. Heels on shoes must not
be more than 1’ in height. All shoes must be without holes, be clean, neat and in good repair. (slippers, flip flops, sandals are not
permitted) shoes must be worn with hose or socks at all times.
Stockings and leggings are only permitted to be worn, when worn under dresses or skirts. Leggings are not pants and may not be worn
as so.
Skirts and dresses may not be any shorter than 2’ above the knee. And stockings or leggings must be worn. (No bare legs at any time)
Name tags must be worn every day, no exceptions will be made. If your name tag is at home, you may use a loaner for 1 day. If it is lost
you must purchase a new name tag from the director. (please ask for current pricing)
Hats and scarfs are not permitted to be worn.

Facility Duty Responsibilities
Cleanliness and sanitation are essential aspects of our profession. Students will be asked to participate in daily and weekly clean-up tasks
around the Institute, including laundry tasks, floors, and bathroom facilities. This training will help develop skills and habits that will contribute to future
success.
Visitors/Clients Must Remain in Reception Area
If a visitor or client is not currently receiving a service, they must remain in the reception/lobby area. Visitors are not allowed on the clinic
floor, classrooms, or break room without the explicit permission of the Institute’s administration.
Student Obligation to Know Pricing, Service Policies, and Products
Students should know all pricing and service policies of the Institute, and the name, purpose, benefits, procedures and cost to the client of
products used. This information will be taught throughout your training. Product knowledge and professional demeanor is essential to client
confidence. Practice courtesy and professionalism at all times when dealing with other students, clients, staff, and visitors to the School. Never
complain or show discontent about performing a service in the presence of, or within earshot, of your clinic client. Always make your client feel
comfortable and welcome. If you have comments, please save them for later when Staff would be able to talk with you about them.
Time Clock Rules and Procedures
The School is a clock hour program. It is vital that students follow the Time Clock Rules and Procedures. Failure to do so will result in the
loss of hours. Student’s hours are recorded on a time clock each day. It is the responsibility of students to use the time clock correctly. Students are
responsible for clocking in and out. Students are assigned a mandatory ½ hour lunch which will automatically be deducted each day. Students must
sign in and out for this time. Students are required to notify the Front Desk Coordinator or floor instructor when they leave and return from lunch.
Students not clocked in and out for the day will not receive hours for the day. Under no circumstances should a student ever leave the
premises during their scheduled hours without the permission of the Director. Students who leave the premises without notifying the Director may be
sent home for the day.
Physical Demands and General Requirements of the Cosmetology Profession
The Hair Nail & Skin Institute is concerned with your success in the Beauty Industry. Students interested in pursuing a career as a
Cosmetologist, Aesthetician or Instructor should be aware of the requirements for success. Students should be able to perform the day-to-day functions
noted below:
1.
A student must have ability to do tasks, using their hands and fingers to grasp, handle, control, move, assemble, or feel objects or tools.
2.
A student must use stomach and lower back muscles to support the body. Students must have the ability to bend and twist the body while
standing or sitting when servicing clients.
3.
A student must enjoy interacting with the public and able to follow a client’s direction. A student must be able to communicate well and help
educate the client on the care of hair, nails or skin.
4.
A student must be willing and able to work long hours while building a personal clientele in order to earn the desired income.
5.
Strive to continually upgrade your abilities through educational classes, attendance at trade shows, seminars, and practice, practice, practice.

Standards of Conduct
(The standards of conduct are modeled after accepted Industry standards to prepare students for a successful career)
1.
It is the student’s responsibility to clock in/out. Students leaving the Institute must be clocked out. Lunch break is 30 minutes (signed out),
and students are allowed two ten minute breaks per day (signed out). Instructor or staff must be notified when taking lunch or break. (See
Time Clock Rules and Procedures)
2.
Students must comply with the dress code and a name badge must be worn at all times.
3.
Discuss only ethical and professional subject matter while serving clients and during school hours. Refrain from using profanity.
4.
Be fair, honest, and never steal. This can result in termination.
5.
Training involves sanitation, cleanliness, and care of equipment and facility. Students are responsible for personal work stations and work areas
daily. Daily assigned sanitation duties must be evaluated before clocking out each day. State Board requires all students to follow sanitation
rules and practices at all times
6.
All students required to attend classes as scheduled. Students are to clock in by 8:30 A.M. If a student is tardy they may not clock in until 9:45
A.M. If a student is unable to attend, the Student must notify the Director at 928-910-1002 between the hours of 8:00-8:30 am. If the phone
isn’t answered promptly you will need to call back as we may be attending to other students at that time. A no call / no show will not be
tolerated by the Institute. A student must call in person, and must speak to the Director; a message on the voice mail will not be
accepted.
7.
Saturdays and Tuesdays are mandatory. Students may not be absent the day before or the day following a weekend or a holiday (unless
originally contracted to do so). This will result in a 1 to 3 day suspension.
8.
Smoking is only allowed in designated area outside of the school building.
9.
Cell phones of any type, iPods, or any other electronic devices are not allowed inside the school unless they are turned off and locked in a
locker. Violations will result in phones being confiscated for the day. Phones may be used outside during breaks or lunch in designated area.
Repeated violations will result in the stipulation that the Student’s phone not be allowed in the School under any circumstances. Important
messages or emergencies can be left with school staff.
10.
Gum chewing is not allowed.
11.
No eating or drinking except in the break area. (not in classroom)
12.
After a student has clocked in, they must be involved in curriculum related activities at all times.

13.
Staff, clients, and students must be treated courteously and professionally at all times. Clients should never be left waiting. Breaks and lunch
must be adjusted around the client’s needs.
14.
It is rude to speak in a foreign language in the presence of a client, instructor, administrator, or another student who does not speak and
understand that language.
15.
Students may not refuse a client service. If this occurs, the student will be clocked out for the day and may receive a 1 to 3 day suspension.
16.
Personal services must be approved by Instructors and paid for PRIOR to service.
17.
Practical operations are required to be signed off on a daily basis and are the sole responsibility of the student.
18.
Students may receive oral counseling and/or a student advising sheet for any inappropriate conduct or other actions not adhering to the
standards of conduct.
19.
Client consultations must be done with an Instructor on every client. Consultation sheets must be filled out and completed on every service
rendered by all students. All consultation sheets must be signed by an Instructor.
20.
No arguing with an instructor or student in the presence of clientele or other students.
21.
Types of behavior that inhibit the learning process of other students are not acceptable.
22.
Destruction of property or violent behavior will result in immediate termination. Disruption of an Educational Institution is a crime in the State of
Arizona.
23.
Any student with a complaint needs to register the complaint with the Director/Administrator.

Use, possession, or sale of drugs or alcohol is not allowed on the premises. Students whose behavior is influenced by drugs or alcohol will be
asked to leave the premises and disciplinary action will result.
26.
The school is not responsible for a student’s personal equipment or property. Lockers are provided for all students.
27.
Students must park in areas designated for student parking.
28.
Absences are excused only when related to medical issues (with a Doctor’s note), court appearances, or bereavement.
29.
It is unfair to expect to receive an education for free. Our school has reserved space, equipment, and Instructors for your education. Failure to
keep your tuition payments current can result in your suspension or termination from the Institute.

The Institute reserves the right to change or amend the Standards of Conduct or General Policies at any time without prior notice.
Cancellation/Refund Policy
Applicants denied admission in the Hair Nail and Skin Institute (the “School” or "Institute" ) will receive a full refund of any money paid. If a
student (or in the case of a student under legal age, their parent, or legal guardian) cancels enrollment and demands in writing the money back within 3
business days of the signing of the Enrollment Agreement, all money collected by the School will be refunded even if the student has begun classes. If
a Student cancels the Enrollment Agreement more than 3 business days after signing the Enrollment Agreement but prior to starting classes, a refund of
all monies paid to the School less the applicable application fee (not to exceed $100.00) will be made.
Formal Cancellation Date is the earlier of the postmark on written notification, or the date said cancellation is delivered to the school director
in person; the last date of attendance if a student is terminated by the Institute, 30 days after the last day of attendance, the scheduled return date on
an Approved Leave of Absence, or the date the Student notifies the School that they are not returning from an Approved Leave of Absence.

Enrollment time is defined as the time elapsed between the student’s start date and the last physical attendance at the Institute. Any
student terminated by the Institute should refer to the Cancellation/Refund Policy table for determination of any refund due the Student or Tuition
owed to the Institute based on the percentage of scheduled time enrolled, not solely the hours clocked. Any refund due to the Student will be paid
within Forty- Five days (45) after a request for refund, in writing, is received and acknowledged by to the Institute’s Administration. Charges for books,
tools and supplies are non-refundable after being issued to a student. The school will assess a termination fee of $150.00.
If permanently closed or no longer offering instruction after a student has enrolled, the School will provide a pro-rata refund to the Student of
tuition paid. If the course is cancelled subsequent to a Student’s enrollment, the School will either provide a full refund of all monies paid, or arrange
for completion of the Course at a mutually agreeable location. Any student who has attended class and is requesting withdrawal from any one of the
School programs offered and a refund of monies paid to the school will refer to the Cancellation/Refund Policy. The Institute’s refund policy is modeled
after accepted Industry standards.

GUEST SPEAKERS: The Institute regularly schedules guest speakers and educators. They include instructors, professional stylists, salon owners, and
major manufacturer’s representatives. Students gain insight into “real world” of the Beauty business and receive valuable information on what it takes
to be successful from these industry experts. Students also have access to information about local, national, and international beauty industry trade
shows and educational seminars.

Graduation Requirements for all offered Courses
Students must meet the following requirements of study to qualify for graduation. A diploma and certificate of graduation will be issued upon
completion of the course of study. The graduation requirements are as follows:







Completion of all course hours required by the State Board of Cosmetology as outlined in the school catalog.
Completion of all written and practical requirements. Successful completion of all phases of study, required exams, practical projects, and
clinic assignments for the Course.
Full payment of all fees and tuition, and/or compliance with Financing Agreement.
Completion of School Board examinations (written and practical) with a passing score. Results must be reviewed with Instructor. Practical
School Boards will be given once monthly, in the first full week of the month, to be determined by the Director. Final Written School Boards
exams may be administered prior to the Practical School Boards. In order to be eligible to take the final exam, Students must have clocked
the hours needed for graduation by the end of the previous month, and have registered with the school to take the final exams on the next
available date.
Completion of school exit paperwork and interview requirements. All debts owed to the School are paid in full or arrangements made before
testing out for graduation.
Students must continue clocking hours until all graduation requirements are met.
Diploma will be awarded after all graduation requirements are complete.
Requirements for Licensure
Requirements for admission to the licensing examination for the Arizona State Board of Cosmetology:
An applicant must be a graduate of a school of Cosmetology, and complete the required hours for the specified course. Applicant must make
application and pay the required examination fee for the specific examination. Arizona administrative code R-4-10-108 states “The board shall
conduct all examinations in English and applicants shall submit answers in English.”
All education and learning situations at The Hair Nail & Skin Institute are conducted in English, including classroom and clinic.
Employment Assistance
While the Institute cannot guarantee employment for graduates, assistance in finding employment will be facilitated by networking with
salons, spas, resorts, and manufacturer’s representatives. Students will receive training in how to write a resume, complete an employment
application, prepare for an effective interview and professional appearance.
Re-entry Students
Former students of the Institute who wish to re-enter, must request approval from the Institute’s Administration. The request will be
reviewed and a decision made with 30 days of the request. Students who are re-admitted within six months of the original official withdrawal date will
be re-admitted and their hours honored. The school does not recruit students already attending or admitted to another school offering a similar
program of study.
Transfer Students
Students should arrange for a personal interview. Adding to the Institute's standard admission requirements, a transfer student must also
provide a complete transcript of training at the previous school or licensing agency, including attendance information, and a statement explaining the
reasons why they chose to leave the previous school and have chosen to attend The Hair Nail & Skin Institute.
In addition to the above, transfer hours accepted will be determined by the Institute at its discretion. Tuition is based on the total number of
hour plus kit requirements, books, and administration fees. Students are required to have purchased all equipment and supplies needed to complete
the course prior to the first day of class. The Institute does not recruit students already attending or admitted to another school offering a similar
program of study.
Overtime Policy
The Hair Nail & Skin Institute provides students with a sufficient and appropriate amount of time to complete their program and most students can
complete hours on or before their ending date on their enrollment agreement. No penalty will be charged for medical reasons with a valid doctor's
excuse given for each absence. Bereavement for the immediate family, or court appearances with sufficient documentation will be excused.
However, some students fail to satisfactorily complete the program in the period of time specified because of poor attendance. Any student not
completing hours by the ending date on the enrollment agreement will be subject to OVERTIME CHARGES at the rate of $15.00 per hour.

Leave of Absence
Students may request a leave of absence in writing for approval by the administration. If approved, the official leave extends the contract
period by the same number of days designated in the leave document or actually used by the student. No re-entry fee will be charged if the student
returns to class on or before the first class day scheduled after the termination of the leave of absence. A leave of absence will be approved for no
more than 60 calendar days. An extended leave of absence may be approved based on mitigating circumstances. Payments must continue as
scheduled during a student’s leave of absence. The request for a leave of absence must be made in writing and approved by the administration.
Withdrawal Requirements
Students who withdraw from enrollment prior to course completion must complete all required State Board and school paperwork and
immediately pay all charges owed to the School for training according to the Enrollment Agreement. An Administrative fee of $150.00 will be charged
for early withdrawal.
Right to Privacy and File Access Policy
In compliance with the Family Educational Rights and Privacy Act of 1974 (FERPA), the school follows policies that:

Guarantee each student (or parent or guardian if the student is a dependent minor) access to that student’s records.
Require written consent from the student or guardian for release of records in response to each third party request unless
otherwise required by law.
The school does not publish “directory information” about students.
Access to records must be arranged and requested in writing previously and a staff member must be present while the records are being
reviewed. Copies of file documents may be obtained at a cost of fifty cents per copy. Cumulative education records are maintained for a minimum of
five years after graduation or termination.
Drug Policy
The Hair Nail & Skin Institute actively supports the prevention of drug and alcohol abuse. We are concerned with having a safe and drug free
environment. Penalties to be imposed on students and employees for drug abuse violations occurring in the school include:



Notification of the abuse to the proper authorities.
A leave of absence from enrollment/employment during which time the individual must consider the responsibilities reinstated that he/she will
no longer participate in abuse activities affecting performance.
Termination will be considered based on the circumstances surrounding the violation.
Crime Awareness
The Prescott Police Department will visit with students about self protection, personal safety and rape prevention. This will be scheduled
periodically.

Sexual Harassment Title IX Policy
Students, Instructors, and Staff are entitled to learn and work in an environment free of sexual harassment. Sexual harassment is prohibited in any
School-related activity. Sexual harassment is defined as unwelcome sexual advances (including sexual assault), requests for sexual favors, and/or
physical , verbal, or written conduct of a sexual nature when:
i) Submission to such conduct is made explicitly or implicitly a term or condition of an individual's employment, education, or participation in
the School's programs or activities, or
(ii) Submission to or rejection of such conduct by an individual is used as a basis for decisions pertaining to an individual's employment,

OSHA
The United States Department of Labor of the Occupational Safety and Health Administration (OSHA) requires the school to advise its
students of the chemicals used in training. Students will learn about the importance of safety in the workplace and how to use and follow the Material
Safety Data Sheets (MSDS) for chemicals used in the school. A notebook containing Material Safety Data Sheets for the chemicals used at the school is
available in the administrative office.

Check Charges
A $42.00 fee will be collected for any check written and returned to the school for insufficient funds or account closed. Students
are not allowed to write checks or use credit card machine for cash.
Fire Drill
The Hair Nail & Skin Institute is equipped with fire alarms, sprinklers and fire extinguishers. Fire drills with safe exit plans will be conducted
regularly.

Sheri Rogers
School Executive Director
and Educational Director
Mrs. Rogers is the managing member of The Hair Nail and Skin Institute LLC. Mrs. Rogers has been a professional administrator and Director of
Cosmetology Schools for over 25 years. Among the schools she directed, a Paul Mitchell School, Toni & Guy Hairdressing Academy, and a Sassoon
Connection School to name a few. Mrs. Rogers earned her cosmetology license in 1986. Mrs. Rogers is a lifelong licensed salon stylist, beauty industry
executive, and a master career educator. Her many associations with industry professionals and advanced education provide a new level of excitement
and opportunities for training of her students. Mrs. Rogers is a certified team leader of the Nuts and Bolts training program.
Jill Rene' Hassen
Cosmetology and Aesthetics Educator
Founder of The Hair Nail & Skin Institute in 2007. She obtained her Cosmetology license in 1968 in Phoenix, Arizona, and has been a licensed
Cosmetologist for over 48 years.
Mrs. Hassen has been a licensed Arizona Cosmetology Instructor for almost 30 years, a cosmetology School Director for a national company, as well as
Director of The Hair Nail and Skin Institute in Prescott, Arizona from 2007 to 2015. Mrs. Hassen is a Master Educator of hair styling, aesthetics, and nail
technology. She has been a Salon Owner and a successful Salon Stylist. Mrs. Hassen is a certified team leader of the Nuts & Bolts Business Training
Program.
Sandra A. Cassino
Cosmetology and Aesthetics Educator
Sandra Cassino assisted with opening the school in 2007. For more than 40 years, Ms. Cassino has been a dedicated professional of the Cosmotology
Industry. Ms. Cassino has created and implemented the freshman aesthetic curriculum, and is currently instructing freshmen in theory and practical
classes daily, as well as being an Aesthetics instructor on the clinic floor.
Ms. Cassino holds State of Arizona licenses in Cosmetology, and Cosmetology Instructor; State of California licenses in Cosmetology, Cosmetology
Instructor, and holds a California Vocational Teaching Credential.

Ms. Murphy obtained her student teacher training at The Hair Nail & Skin Institute in 2011. She holds State of Arizona Cosmetology and Instructors
licenses, as well as a State of California Cosmetology license. She has been a Salon Stylist since 1994 and a Salon Owner. Ms. Murphy is a clinic floor
instructor for the Aesthetic and Cosmetology programs at the School. Ms. Murphy is a certified team leader of the Nuts & Bolts Business Training
Program.
Debra Cregger

Mrs. Cregger has held cosmetology licenses in California and Arizona for 32 years. She has been a Cosmetology Instructor for 4 years. She currently is
also our clinic floor educator. Mrs. Cregger is a certified team leader of the Nuts & Bolts Business Training Program.
Maria Cardenas
Cosmetology and Aesthetics Clinic Floor Educator
Mrs. Cardenas obtained her cosmetology instructor license in 2016. She is a licensed cosmetologist and salon owner. Mrs. Cardenas is a certified Nuts
& Bolts Business Training program team leader. Mrs. Cardenas is a graduate of The Hair Nail & Skin Institute cosmetology program as well as the
cosmetology instructor program. Mrs. Cardenas instructs both cosmetology and aesthetic students.
Elizabeth Clevenger
Cosmetology Instructor
Ms. Clevenger has been a licensed cosmetologist for over 5 years and a licensed cosmetology instructor for over a year. Ms. Clevenger is a cosmetology
clinic floor instructor as well as the cosmetology phase 1 instructor.
