TALENT SHOW AUDITION FORM
Attention all talented Sanford Creek Elementary School students! It is time for the Fifth Annual talent
show, CLUB ALLEGRO! If you would like to be a part of the show, please review the following
guidelines and rules. It is important that you read this information carefully.
All students who wish to audition must fill out the following form and have their parents sign the
permission section in order to attend the audition. Each student is permitted to audition in one act. It
can be as a soloist or in a group.
Remember:
Form: Permission and act description forms are due by 9 AM on the following dates. NO late
forms will be allowed! Please turn them in sooner rather than later!
 Tracks 1, 3, and 4 Wednesday, November 30. Audition time and dates will be given by end of
school December 2, 2016.
 Track 2 forms are due Thursday, November 10. Track two will receive their audition times and
dates December 5, 2016 upon returning from track out.
Audition dates:
Tracks 2, 3 and 4 – Monday, December 12 and Tuesday December 13
Tracks 1 – Monday, January 9 and Tuesday, January 10
Please note these are the ONLY audition dates! If you plan to be a server, Master of
Ceremony, or sound/light technician, you will receive a separate form!
All students must audition in the exact clothing/costume that will be worn in the talent show.
Judges will be checking for costume appropriateness.

Approximately 20 talent show performers/groups will be selected from the top auditions. This
number is subject to change. Groups/individuals will be judged accordingly using the following
criteria.

Show date: January 31, 2017, 6:30 – 8:00 PM. Please do not audition unless you can commit to this
evening.
On the following pages of this form, fill in each section completely. Indicate the type of act you
will perform. All talent performed in good taste is welcome. Acts should be no longer than 4 minutes.
Songs and lyrics must have approval before the audition. Attach the lyrics of songs to the audition
form if they are sung, danced to, or played at all in the act. Due to equipment limitations, please bring
in your accompaniment before the audition date to be sure it will play on what we have available. If
you would like to have Mrs. Mainey accompany you on the piano that is an option.
IMPORTANT REMINDER:
Student auditions must be exactly as the performance would be including necessary costumes and/or
props. If you are chosen to participate, no changes are permitted after auditions.

Students are responsible for their own music. Bring a CD with your name and the track listed so it is

If your act is using music of any kind, please supply the name of the song, composer, and a copy of the
words if used in the song. Attach a copy of the lyrics to the permission form. Only appropriate
lyrics will be accepted!

Sanford Creek Elementary Talent Show, CLUB ALLEGRO. I understand he/she will be assigned an
audition time slot which will be given to the student no later than Friday, December 2nd for Tracks 1,
3, and 4. Track 2 students will be notified upon returning from track out on Monday, December 5,
2016. The judges will try very hard to stick to the posted schedule, but often there are changes at the
last minute.
Students should plan to be at the audition when it starts at 7:45 AM or 4:00 PM and will be released
to their classes or breakfast for the AM auditions. Students at the PM audition will be released to their
parents after their audition. If your child is in afterschool care, they will be released to Mrs. Wocjik as
soon as their audition is over. Any student not picked up by 5:05 will forfeit their
opportunity to be in the Talent Show.
