Before performing any work, both Property Owners and Plumbers must complete and sign the form below. The
property owner and plumber shall contract directly for the lead water service l i n e replacement, but the Utility will
make direct payment to the property owner’s plumber for up to $2500.
The form below is an addendum to the agreement between the property owner and plumber, and lists the conditions of
the Utility’s rebate.

1. Property owners have entered into a contract with Plumber for replacement of the lead water service line at
owners’ property.
2. A copy of one bid is attached to this addendum.
3. The Sheboygan Water Utility has approved including the replacement of Property owners’ lead water
service line in the Utility‘s Lead Water Service Replacement Program.
4. The replacement of Property owners’ lead water service line is subject to the City of Sheboygan Plumbing
Inspector or the Sheboygan Water Utility certifying that the replacement of the lead water service line is
complete and was done in compliance with all applicable laws, ordinances and regulations. An itemized invoice
shall be included with certification.
5. The Sheboygan Water Utility agrees to pay Plumber the lesser of $2,500.00 or the Contract Amount if the
Contract Amount is less than $2,500.00, within 30 days after the Utility receives notification that the
certification described above was obtained. The Plumber agrees to apply the payment received from the
Sheboygan Water Utility against the Contract Amount.
6. If the Contract Amount exceeds $2,500.00, the Property owners agree to pay the Plumber the difference
between the Contract Amount and $2,500.00.
7. The Property owners understand that the maximum amount the Sheboygan Water Utility will pay for the
replacement of Property owners’ lead water service line is $2,500.00. Any amount the Plumber charges beyond
that amount, including any amount the Plumber charges above the Contract Amount, is the responsibility of the
Property owners and not the Sheboygan Water Utility.
8. The Property owners understand and agree that the Property owners are solely responsible for the selection of
the Plumber and have contracted with the Plumber for the replacement of the Property owners’ lead water
service line independently from the Sheboygan Water Utility. Accordingly, in consideration for the amount paid
by the Sheboygan Water Utility as described above, the Property owners agree to release the Sheboygan Water
Utility, and its employees and officials, from any loss, damage or unexpected costs or expenses arising from
replacement of Property owners’ lead water service line.
9. The Sheboygan Water Utility reserves the right to deny reimbursement for failing to comply with all applicable
laws, ordinances, and regulations.

The Utility’s rebate program makes use of Federal funds and therefore prequalified plumbers must adhere to Use of
American Iron and Steel requirements. The plumber/company owner signing below certifies that all materials used
are/will be in full compliance with the Use of American Iron and Steel requirement as mandated in the EPA’s State
Revolving Funds program.
