The program starts Wednesday, January 8, 2014 at Summit Central Area and continues for six consecutive weeks finishing on February 12, unless
weather forces postponement. If a cancellation occurs, we automatically extend the program the number of trips missed.
The bus loads at school at 2:15 PM and returns at approximately 7:30 PM in the evening. We need at least 35 students in order to pay for the program without raising fees. Adult chaperones will be on the bus.
Classes begin at 3:30 P M and are 2 1/2 hours long. Lessons are taught by Alpine West Ski & Snowboard School with thirty one years of teaching
experience in the greater Seattle area. Snow Sports Consulting Service will be coordinating this program for Alpine West Ski & Snowboard School.
To enroll submit the completed form and tuition in a sealed envelope marked “PTSA: Ski Club” and place the envelope in the PTSA mailbox.
Please do NOT send cash. Enrollment is secured on a first come, first served basis, and registration must be accompanied with payment to secure your space.
Registration deadline is Monday, November 18, 2013.
To receive a discounted seasons pass (which is required for all students) sign up for this program. Season Pass applications will be emailed to you once
SSCS has received your tuition and registration form. Please be aware passes are purchased through the Summit @ Snoqualmie ONLY! Alpine West,
SSCS and Elizabeth Blackwell Elementary will not be responsible for your passes. Direction for buying passes are enclosed with this registration form.
Deadline for ordering passes is Sunday, December 15, 2013.

(___) Beginning skier - can go straight and stop. Some turning. Needs help on the beginner chairlift hill. Green Circle runs.
(___) Wedge turn skier - links wedge turns. Skis independently on the beginner chairlift hill. Green Circle & some Blue Square runs
(___) Christie skier - can sideslip both ways. Finishes last half of turn with matched skis. All Blue Square & some Black Diamond runs
(___) Carving skier - can match skis in most turns with pole plants. Skis most advanced slopes. All Black Diamond runs
(___) ATAC skier - skis all terrain and all snow conditions including, deep snow and moguls.
Snowboarders must be 9 years or older.

(___) Beginning boarder - can side slip on toe and/or heel edge. Rides independently on the beginner chairlift hill. Green Circle runs
(___) Turning boarder - does toe/heel turns. Rides easier slopes on big chairs. All Blue Square & some Black Diamond runs.
(___) Fakie boarder - switches from regular to fakie while linking turns. Rides most advanced slopes. All Black Diamond runs.
(___) ATAC boarder - boards all terrain and snow conditions including, deep snow and moguls.

School Participation Agreement & Release I (we) recognize that skiing/snowboarding are hazardous sports that can results in serious injury or death. I (we) voluntarily made a choice to participate in this activity and accept the risks
inherent in skiing/snowboarding and in the ski area/mountain environment. I (we) accept my (our) responsibility to be informed, to behave prudently, to ski safely and to read and abide by the Skier’s Responsibility Code and all other
posted behavior notices. I (we) agree that I will not sue or make a claim against Elizabeth Blackwell Elementary School, Elizabeth Blackwell Elementary School PTSA, Lake Washington Public School District, Alpine West Ski and
Snowboard School, Snow Sports Consulting Service, LLC, Boyne USA Inc., or any of its owners, officers, agents, or subsidiaries, including but not limited to, Ski Lifts Inc., Crystal Mountain, Inc., and the U.S. Government or any of
(their/its) employees, agents, contractors, subsidiaries, officers (“Released Parties”) for any loss, injury or damage result ing from any cause including negligence, which arises out of my participation in any activity at the ski area,
including but not limited to, use of the slopes, equipment, or any of the facilities or services on the premises. I (we) further agree to RELEASE, HOLD HARMLESS, and INDEMNIFY Elizabeth Blackwell Elementary School, Elizabeth
Blackwell Elementary School PTSA, Lake Washington Public School District, Alpine West Ski and Snowboard School, Snow Sports Consulting Service, LLC, Boyne USA Inc., or any of its owners, officers, agents, or subsidiaries,
including but not limited to, Ski Lifts Inc., Crystal Mountain, Inc., and the U.S. Government or any of (their/its) employees, agents, contractors, subsidiaries, officers (“Released Parties”) for any loss, injury or damage which arises out of
my participation as described above. This release is also binding as to any other person, including all family members, heirs, and executors.
If I am signing on behalf of a minor: I accept full responsibility for all medical expenses and claims related to the minor’s participation in any activity as described above. I agree to RELEASE, HOLD HARMLESS, and INDEMNIFY
the Released Parties from all claims brought by or on behalf of the minor.
Students will be expelled from this ski/snowboard program without refund for being willfully disobedient, possession, use or sale of tobacco, alcohol, or controlled substances, vulgarity, stealing, gambling, fighting, leaving the ski
area or for using non-approved transportation. If the ski/snowboard program is canceled for a day(s), the program is automatically extended until the program is completed. If the program is not completed by March 26, 2014 any
remaining on-snow lessons and transportation will be refunded minus the trips taken. All other refunds are only considered if a student must withdraw based upon a doctor’s written recommendation.
Because of preseason costs, a $50.00 processing fee is charged to issue all refund checks. This program provides employment, services, and privileges regardless of race, color, creed, sex, religion, age or national origin.
I (we), the undersigned have carefully read and understand this “Participation Agreement & Release” and agree it is binding on me (us). Student sign and/or parent/guardian if the student is under 18.

Choose which option works best for you.
1. You can purchase your pass on line at www.summitatsnoqualmie.com/ecommerce. Once you have turned in your
registration form with full payment a Group Name and Password along with other directions will be emailed to you.
This is for Alpine West/SSCS students ONLY.
2. If you had a pass last year you can send your completed and signed application with payment to The Summit @ Snoqualmie
by December 15, 2013. Write on the application “use last years photo” pass will be mailed back to the student.
3. If you did not have a pass last year, send a school photo or any picture that is wallet size or larger with your completed and
signed application with payment to The Summit at Snoqualmie by December 15, 2013. Pass will be mailed back to student.
4. If you had or did not have a pass last year you can drive up to The Summit West Season Pass Office with your completed
and signed application and payment. Get your picture taken or use last years photo and they will give it to you right then.
They are open everyday from 9-5 after October 21.
